LPA Correia conducted the required one-year inspection. Upon arrival, LPA was greeted by Caregiver Janet Balingit, identified herself, and explained the purpose of the visit. The facility is licensed to serve six residents aged 60 and above, all of whom may be non-ambulatory and/or receiving hospice services, with one (1) resident permitted to be bedridden. Administrator Jeremy Bagaoison arrived and joined the visit. During today’s inspection, all six residents were present. Three residents are currently receiving hospice services, and the remaining three receive Home Health services. A review of resident records confirmed that required documents including the current Physician’s Report, Resident Appraisal, Needs & Services Plan, Identification and Emergency Information, and Admission Agreement were complete and up to date. Personnel files were also reviewed and contained Criminal Record Clearance, required training, Health/TB Screening Reports, First Aid and CPR certification, and qualifications, all of which were current and complete. The Administrator’s certification is valid until March 13, 2026. The facility was in compliance with disaster drill requirements and maintains a current lease agreement and liability insurance. [Continued on LIC 809C] [Continuation of LIC 809] LPA, accompanied by Administrator Bagaoison, conducted a tour of the facility, inspecting both the interior and exterior. The facility was observed to be clean, sanitary, and in good repair. Pathways were free of obstructions and slip hazards. Required postings were visible. Resident bedrooms contained the required furnishings. Resident showers were equipped with non-skid flooring and grab bars and were in working order. Doors, windows, and screens were in good condition. Extra linens, hygiene supplies, and Personal Protective Equipment (PPE) were available. LPA observed a landline telephone; operational smoke and carbon monoxide detectors; a current fire extinguisher; and a First Aid kit and manual. The facility includes an adequately sized common area and a shaded outdoor space for activities and visitation. The pool area is secured with a locked 5 foot gate. Per Administrator Bagaoison, there are no firearms or ammunition on the premises. Medications were stored in a locked medication cart. Toxic substances and hazardous materials were secured in locked cabinets. Based on today’s inspection, no deficiencies were cited. An exit interview was conducted, and a copy of this report, along with Licensee Rights (LIC 9058, rev. 01/16), will be provided to Administrator Bagaoison. Licensing Program Analyst (LPA) Debbie Correia made an unannounced visit to the facility to conduct an annual licensing inspection. LPA identified herself to Caregiver Janet Balingit and was granted entry into the facility. Administrator Bajaoisan was contacted and arrived at the facility to join the inspection. The facility is licensed to serve six (6) residents; of which all can be non-ambulatory and/or receiving Hospice Services, and 4 residents may be bedridden. During today's visit all 6 Residents were present. LPA Correia conducted a resident records review and conducted a partial facility tour. Per today's facility visit resident records were complete and up to date. During the facility tour LPA observed smoke and carbon monoxide detectors were present and operable, as well as a fire extinguisher. The internal ambient temperature was 78 degrees Fahrenheit. The require posting were appropriately displayed, and the facility had a working landline. Sharps, toxins, and medications were locked and inaccessible to Residents in care. An overall inspection of the facility began today however due to time constraints LPA was unable to complete the visit and will return later to conduct the remaining portion of this inspection. No deficiencies were cited during today's visit. This report was discussed with Administrator Bajaoisan. Type of visit: Case Management - Annual Continuation

Licensing Program Analyst (LPA) Debbie Correia made an unannounced visit to the facility to conduct a case management - annual continuation visit to complete the annual inspection commenced on February 13, 2024. LPA was greeted and granted entry into the facility by Administrator Rivera, to whom LPA discussed the purpose of the visit. During today’s visit, LPA, accompanied by Administrator Rivera, toured the interior and exterior of the facility, and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. Resident bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. Hot water temperature at taps accessible to residents in care were all with in regulation measuring at 110.F, and the facility temperature was 70 F. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all safely stored and labeled. Cooking/dining equipment and utensils were present. There were no sharp objects, toxic chemicals/poisons, or open-faced heaters accessible to residents. Medications and toxins are stored in locked areas inaccessible to residents in care Faiclity has a pool on the premesis that has a 5-foot locked gate, no other bodies of water were present. Per Administrtor Rivera there was no ammunition kept at the facility. Smoke alarms, carbon monoxide detectors, and facility telephone were all operable. Fire extinguisher was serviced within the last 12 months. Required licensing postings were observed in visible areas of the facility. All required records for residents and staff were present and current. No deficiencies were cited during today's annual inspection. The facility is licensed to serve six (6) residents; of which all can be non-ambulatory and/or receiving Hospice Services, and 4 residents may be bedridden. During today's visit all 5 Residents were present. LPA Correia conducted a resident records review, briefly spoke to resident in care, and, accompanied by Administrator Rivera, toured of facility. Per today's facility tour resident bedrooms and bathrooms were equipped with required furnishings, and precautionary. Smoke and carbon monoxide detectors, as well as a fire extinguisher were all present and operable, and up to date. An overall inspection of the facility began today however due to time constraints LPA was unable to complete the visit and will return at a later date to conduct the remaining portion of this inspection. Allegation investigated: Staff did not feed resident in care

Licensing Program Analyst (LPA) Renita Hall, conducted an unannounced visit to initiate a complaint investigation regarding the above-mentioned allegation and deliver findings. LPA were allowed entry by Lawrence Vergara, Caregiver. LPA identified herself and disclosed the purpose of the visit and elements of the complaint with the Caregiver. The site Manager, Anafe Rivera and Administrator, Von Rivera joined the interview a few minutes later. On October 31, 2023 an allegation was made that staff members at Anabella Home did not feed a resident in care, which raised concerns about potential neglect. The resident in question was no longer living at the facility as family came to pick up their belongings and decided to care for resident themselves. Continued on 9099C Unsubstantiated The Reporting Party: RP was interviewed to obtain their account of the events and stated that resident was on Hospice and did not know what type of diet the resident was on at the time of reporting the complaint. Residents Interviewed: During the interview with the resident, they confirmed that they had received their meals regularly and did not experience any instances of not being fed by the staff. Staff Interviewed: All staff members denied the alleged allegation and provided consistent statements that the residents were served meals as per their dietary requirements. Other outside sources stated that they witnessed staff feeding the resident and the resident was only in care for a few days before they "medical out." Documentation Review: Relevant documentation, including resident care records, meal schedules, and any incident reports, were reviewed to identify any discrepancies or patterns. No discrepancies were found Based on the review of records, and statements the allegation of: Staff did not feed resident in care is Unsubstantiated due to a lack of conclusive evidence. A finding that is unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. An exit interview was conducted with Von Rivera, Administrator.
