Licensing Program Analyst (LPA) Debbie Correia made an unannounced visit to the facility to conclude the required annual licensing inspection that commenced on October 30, 2025. LPA was met by Administrator Derafora, identified herself, was granted entry into the facility, and stated the purpose of today’s visit. During today's visit LPA conducted a review of facility, staff, and resident records that were complete and up to date. Staff training, disaster drills, administrator certification, liability insurance, and required licensing forms were present and current and/or active. Additionally, a review of resident medication administration records was accurate and complete. The facility is equipped with Personal Protective Equipment (PPE) and a first aid kit and manual. LPA Correia, accompanied by Administrator Derafora, conducted a tour of the remaining portion of the interior of the facility, and a toured the exterior of the facility. The taps at faucets used by residents measured within compliance. The facility pool is surrounded by a locked gate and is inaccessible to residents in care. There is a shaded area for outside activities, and no obstructions or trip hazards were observed. Based on today's visit, there were no deficiencies observed at this time in the areas evaluated. Licensing Program Analyst (LPA) Debbie Correia conducted an unannounced required annual inspection. LPA identified herself, was granted entry, and discussed the purpose of the visit with Administrator Derafera. The facility is licensed for a maximum capacity of six (6) residents, all of whom may be bedridden. The facility's current census is six (6) residents, four (4) of whom are bedridden and receiving hospice care, and two (2) residents that are non-ambulatory, who were all present. During today's visit LPA Correia, accompanied by Administrator Derafera, conducted a tour of the interior of the facility. The facility was equipped with smoke alarms, fire extinguishers, carbon monoxide detectors, and a sprinkler system that were all up to date and operational. Cleaning solutions and toxins are housed in a locked room, medication is maintained in a locked medication cart, and sharps are in a locked drawer. LPA observed all the required furnishings in resident bedrooms, and the resident bathrooms were equipped with grab bars and non-slip flooring, and nightlights in the facility hallways. The facility’s ambient temperature at the time of visit was 75 degrees Fahrenheit. LPA observed the facility to be sanitary and in good repair. There is a large common area to facilitate visitation and resident activities. There is a landline and internet available for residents’ use. Per the facility Administrator there are no firearms or ammunition on the premises. During today’s visit LPA also secured facility records. Due to time constraints the comprehensive annual inspection began, however, will be continued later. No deficiencies were cited during today’s visit. Licensing Program Analyst (LPA) Ryan Fulton conducted an unannounced Required Annual Inspection. The facility file was reviewed prior to the visit. LPA was welcomed by and discussed the purpose of the visit to Administrator Tess Derafera. The facility's license shows a maximum capacity of Six (6) residents. During today’s inspection there were Six (6) Residents in care. LPA and Administrator Tess Derafera toured the interior and exterior of the facility and inspected each room. The facility was clean, sanitary, and in good repair. Pathways were free of obstruction and slip hazards. resident bedrooms contained the required furnishings. Doors, windows, screens, toilets, and showers were in working order. Extra linens and hygiene supplies were present, as well as Personal Protective Equipment. The facility had sufficient space and equipment to facilitate dining, laundry, visitation, meetings, and client activities. There was at least 2 days of perishable food, and at least 7 days non-perishable food present, all of which are safely stored. Cooking/dining equipment and utensils were present. Toxic chemicals/poisons were locked and inaccessible to residents. Medications were labeled, as required, and stored in locked areas. The facility’s ambient internal temperature was compliant. Hot water temperature at taps accessible to residents were all compliant: Kitchen sink was 105 F; bathroom #1 sink was 105 F and bathroom #2 sink was 105 F. Pool was Locked and secure with a 6 foot fence around the entire perimeter. Per administrator, no firearms or ammunition are kept at the facility. Carbon monoxide/Smoke detectors, emergency lighting, and facility telephone were all in working order. Fire extinguisher's .First aid kit(s) were complete and readily accessible. Required licensing postings were observed in visible areas of the facility. Licensing Program Analyst (LPA) Rebecca Ruiz conducted an unannounced case management - annual continuation visit. LPA was greeted by, identified herself to, and explained the purpose of the visit to Licensee Philip Butzen. During today's visit, LPA observed residents in care and toured the facility LPA reviewed multiple resident and staff records. Each resident record was complete and contained a signed admission agreement, updated physician’s report and medical assessment, documents regarding safeguarding personal property, and personal rights. Each staff file was complete and contained a personnel record, first aid certificate, fingerprint clearance and association, and a health screening. The facility’s water temperature was measured at 105.3 degrees Fahrenheit in non-private bathroom and 106.5 degrees Fahrenheit in a private resident bathroom. LPA provided the Licensee with technical assistance regarding the facility's emergency disaster plan and the Infection Control plan. The Licensee will submit a copy of their current liability insurance to the Department. No deficiencies were cited on today’s date. Licensing Program Analyst (LPA) Rebecca Ruiz conducted an unannounced Required 1-Year visit. The facility file was reviewed prior to the visit. LPA was greeted by, identified herself to, and explained the purpose of the visit with Licensee Philip Butzen. The facility is licensed for a maximum capacity of 6 residents, all of which may be bedridden. The facility has a waiver for 6 hospice residents. During today’s visit, the facility had a census of 4 residents, 2 of which were bedridden and receiving hospice services, 1 was non-ambulatory, and 1 was ambulatory. The facility does not have a clearance for delayed egress or secured perimeter and LPA did not observe any aspects of delayed egress or secured perimeter. The Administrator for the facility is Lyndon Derafera and their certificate was valid and current. During today’s visit, LPA toured the facility and inspected each room of the facility, including resident rooms, bathrooms for resident and staff use, kitchen, garage, common areas, and outside space. The facility has a swimming pool on the premises that is located down a flight of stairs. During today's visit, LPA observed two sections of the fence surrounding the pool had been removed, resulting in the swimming pool being unsecured. Per the Licensee, the fence had been removed approximately 3 months ago. A deficiency and an immediate civil penalty of $500 was assessed during today's visit. According to the Licensee, no firearms or weapons are stored on the premises. The facility was found to be clean, safe, and in good repair with no pathway obstructions. LPA observed locked storage for all hazardous and/or toxic chemicals and were stored separately from food supplies. Continued on LIC809-C page... LPA observed a 2-day supply of perishable food and a 7-day supply of non-perishable food present at the facility. LPA observed linens and hygiene products provided to the residents that are in good repair and sufficient to meet their needs. Staff present at the facility during the time of the inspection had a criminal background clearance. Due to time constraints, the LPA was unable to complete the Required-1 year inspection during today's visit and an annual continuation will be conducted at a later date. The following deficiency is cited per CA Code of Regulations Title 22 and noted on the attached LIC809-D page. The $500 civil penalty was noted on the LIC421IM form.
