Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with caregiver Betania Najarro and explained reason for visit. Licensee Tyrone Mcghee was notified by telephone. Facility is licensed to serve six (6) residents age 60 and over of which 3 may be non-ambulatory. Bedrooms #1 and #4 approved for non-ambulatory residents. Hospice waiver approved for 3 residents. RCFE/Dementia. The facility is in a residential area. A tour of the facility included the living room/dining area, kitchen with washer and dryer, 3 resident bedrooms, 1 resident bathroom, one staff room with bathroom, and outside shaded area for residents. LPA toured the facility and observed the following: License for facility was not posted anywhere in the home TV issued. LPA observed required PUB 475 posting but not correct size TV issued. Each resident bedroom has the required furniture and bedding. LPA observed extra clean linen and towels in storage area. The smoke /carbon monoxide detectors were observed throughout the facility and are properly operating. The facility has one (1) fire extinguisher in living room. A bottle of Awesome cleaning bleach spray was observed to be accessible to residents under bathroom sink at time of visit. The hot water temperature in the resident’s bathroom was measured at 124.4 not within the required range of 105-120 F. Freezers are maintained at a temperature of 0 degree F and the refrigerators at a maximum of 40 degrees F. Facility was observed to have a sufficient supply of 2 days perishable & 7 days non-perishable foods. LPA observed injectable medication not in lock box accessible to residents in refrigerator. There are no firearms or weapons stored at the facility. The facility does not have a swimming pool or bodies of water on the premises There is a shaded area for the residents outside with a table, but no chairs TV issued. Passageways and exits are free of obstruction. SEE LIC 809C LPA reviewed three (3) staff files and observed the following missing: Admin- LIC 501 job application with resume, LIC 503 health screening with TB and employee rights. S1-LIC 503 health screening with TB and employee rights, S2- LIC 503 health screening with TB and employee rights. LPA reviewed three (3) resident files and observed the following: missing: R1-Physicians report LIC 602 with TB, LIC 627 consent form, LIC 601 ID/emergency form, LIC 603 pre appraisal, and LIC 613 resident rights. R2-LIC 627 consent forms, LIC 603 pre appraisal, and LIC 613 resident rights. R3-Physicians report LIC 602 with TB, LIC 627 consent form, LIC 601 ID/emergency form, LIC 603 pre appraisal, and LIC 613 resident rights. LPA also observed two staff missing CPR/First Aid certificates and all required training needed before employment. LPA requested a copy of the following missing documents to be sent liability insurance, infection control plan, emergency disaster plan, and last drills conducted at facility. LPA reviewed medication for three (3) residents and observed discrepancies/deficiencies. Two residents were missing a centrally stored medication list. Medication are stored in locked file cabinet in living room. MAR log is used. Deficiencies have been noted on LIC 809D under Title 22 Regulations. On 3/17/2026 at 8:03 a.m., Licensing Program Analyst (LPA) Jewel baptiste arrived unannounced to follow up on Technical Violations issued on 2/23/2026. Upon arrival LPA spoke to the (Licensee)Tyrone Mcghee through the ring camera. The purpose of the visit was explained. At 9:25 Licensee Tyrone Mcghee arrived to assist with the visit. During the previous visit, LPA observed the following: · Purchase more cups and bowls. · Provide more comforters in the home. · Provide a shaded area for all 6 residents. · Dining room area seats all 6 residents. During today's visit the following was observed: · The purchased bowls and cups · Comforters on every bed · A shaded canopy and table and chairs for the residents · Dining table for all 6 residents. Type of visit: Post Licensing

On 2/23/2026 at 8:00 a.m., Licensing Program Analyst (LPA) Jewel baptiste arrived unannounced to conduct a Post Licensing inspection. Upon arrival LPA spoke to the (Licensee)Tyrone Mcghee through the ring camera. The purpose of the visit was explained. At 9:05 Administrator Oziomachuk Okonkwo arrived to assist with the visit. The facility is a single-story building located in a residential area. Facility approved to serve 6 residents of which 3 may be non-ambulatory, age range 60 and over. Bedrooms #1 and #4 approved for non-ambulatory residents. Hospice waiver approved for 3 residents. During today's visit, LPA inspected the physical plant inside and outside, reviewed the food supply, and tested the smoke/carbon monoxide detectors. Since the facility currently does not have any residents, LPA was unable to review staff files, resident files, and medications. Observations: LPA toured and inspected the facility. It consists of three (3) bedrooms [3 shared], one (1) bathroom, 1 office space with staff bathroom, kitchen, pull out dining room, living room with camera’s, laundry area shared with kitchen, backyard patio, and front yard. LPA was unable to observe shaded areas for residents. Sharps were observed locked and inaccessible to residents in care. No obstructions were noted in hallways or living areas. Sufficient furniture and lighting were observed throughout the facility. There are no bodies of water. Prior to admitting: · Purchase more cups and bowls. · Provide more comforters in the home. · Provide a shaded area for all 6 residents. The facility will send photo proof to LPA of those changes by March 2nd. The report was reviewed with Administrator Oziomachuk Okonkwo. Type of visit: Prelicensing

Licensing Program Analyst (LPA) Bennette Pena conducted an announced Pre-Licensing facility Evaluation visit. LPA met with Tyrone McGee, Licensee/Applicant and Oziomachukwu McGee, Administrator who assisted LPA with the visit. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and the following was inspected during the evaluation. The physical plant was toured inside and out and the following are observed: Physical Plant & Environment Safety: It consists of (4) bedrooms, of which (3) bedrooms are resident bedrooms and (1) is an office. Bedrooms were inspected and observed the required items of furniture. Mattress pads are available but have not been put in all (6) beds. There are (2) bathrooms in the home. LPA observed that bathroom #1 did not have a slip resistant mat because they installed a slip resistant tile. Only (1) set of extra linens/bedsheets are available for changing. Sufficient personal hygiene supply available. Bedrooms 1 & 2 did not have lamp shade/light. The laundry machines are located in the kitchen. Facility does not have a garage. Smoke Detectors were observed throughout the home and were tested and operable. One (1) fire extinguisher is observed in the living room which was purchased recently. Cleaning solutions and sharps were locked and stored separately from where food supplies are stored. Kitchen cabinets, refrigerator/freezer, oven and microwave are in working condition, clean and sanitary. The home does not have auditory device that will alert staff/signal system to monitor exits. Doors, exits, hallways, and passageways were clear and free of obstruction. No pools or bodies of water were observed in or around the property. The home does not have a video camera monitor system inside and outside the property. Supplies for indoor activities was observed and available for use. Hot water temperature readings met the Title 22 Regulations requirement of 105-120 deg F. ***CONTINUED ON LIC 809-C***** Operational Requirements: The Infection Control Plan has been submitted to CCL and the CAB Analyst. The facility has been granted a fire clearance. Fire clearance granted for (6) residents of which (30 may be non ambulatory and waiver/granted for hospice care for (3). The Licensee/Applicant does not have Liability Insurance in place and will be purchased once a resident has been admitted. Applicant/Administrator stated that they will not handle resident's cash resources. One (1) operating telephone was observed and tested by LPA on the premises. Telephone is easily accessible and available for residents' use. Personnel Record/Staff Training: The facility lacks a cabinet for storing and filing resident and staff records that must be maintained. Resident Rights-Information: The following policies are posted in the home: Theft and Loss policy, Labor information, Licensing Complaint Poster, Resident Personal Rights and Resident Council Rights. Food Service: Meals will be stored and prepared in a safe manner, necessary to meet the needs of residents. Toxic substances are stored in a locked kitchen cabinet. Food storage and preparation areas, which include pantries, cupboards, drawers and counters were observed to be clean and appropriate for food preparation. Appliances such as a microwave, refrigerator and stove were observed to be clean and operating properly. The refrigerator was observed to be at 45 degrees Fahrenheit and the freezer at 0 degrees Fahrenheit. Incidental Medical & Dental: First Aid supply was observed and is kept in a niche in the living room. Facility has not designated a storage of medication that require refrigeration. Licensee/Applicant did not have a safe container to dispose syringes and needles in accordance with the Title 22 Regulations 87303 (g)(1). List of emergency contacts such as Police, Fire Dept. or paramedic unit was posted in a visible location. Disaster Preparedness: The home has a complete Emergency and Disaster Preparedness Plan. There is a contact information list of local emergency response personnel, clients authorized representative or local emergency contact name posted and visible to staff and residents. Component III was also completed at the time of the visit and all required documents for Licensing were discussed. Facility did not meet the physical plant requirements/ inspection as required per California Code of Regulations Title 22 Division 6. The following corrections need to be made prior to clearing the physical plant: Facility does not have auditory device and/or signal system on exit points. Facility does not have an outdoor furnishing and activity area equipped for outdoor use. Facility does not have sufficient medication cabinet equipped for (6) residents. Facility does not have a medical and sharps bin container. The facility lacks a cabinet for storing and filing resident and staff records that must be maintained. LPA will notify the assigned Centralized Applications Bureau (CAB) Analyst of the pre-licensing facility evaluation visit report, as well as the items needing corrections. Licensee/Applicant and Administrator were informed to send proof of corrections such as photos and receipts to the LPA.
