On June 15, 2026, an office meeting was held to discuss possible additional civil penalties for Complaint 11-AS-20230119162436 regarding the allegation “Resident sustained a fracture while in care”. Attendees included Licensing Program Manager (LPM) Ulysses Coronel, Licensing Program Analyst (LPA) Mario Leon, and Simla Mehta - Licensee/Administrator, Meenesh Mehta - Manager and Jennifer Bobadilla - Assistant Administrator. The following topics were discussed: • Possible additional civil penalties • Appeal rights for the additional civil penalties • The Department’s offer of the Technical Support Program (TSP) There were no deficiencies cited during today’s meeting. Type of visit: Case Management - Annual Continuation

On 10/15/25, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced required – annual continuation inspection and met with Administrators Simla Mehta and Jeniffer Bobadillia and Staff Elenita. The facility is licensed to serve six (6) non-ambulatory residents age 60 and above. The facility has an approved hospice waiver for six (6) residents and two (2) residents are currently receiving hospice services. Annual Fees are current. The facility is a two-story home. The first floor is where the residents reside and consists of the following: six (6) resident bedrooms, three (3) full bathroom, one (1) staff room and one (1) staff restroom, den, living room, kitchen, dining room, laundry room, attached garage and an outdoor shaded patio and gazebo area. The facility is clean, sanitary, and in good repair. On 10/10/25, Staff accompanied LPA inside and outside the facility during this inspection. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. Continue to LIC809-C. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. There are no security bars or weapons on the premises. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place, hot water temperature properly measured between 106 degree Fahrenheit. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Doorways were free of obstructions. LPA toured the kitchen area and observed a two-day supply of perishable and a seven-day supply of non-perishable food. Knives and toxins were kept in locked storage cabinet. First Aid kit was available. Two fire extinguishers, last serviced 06/18/25 was observed in the kitchen and in the hallway. Hedrick Fire Protection inspected the fire alarm in 06/25. The last fire drill was conducted on 09/09/25. On 10/15/25, five (5) staff records were reviewed, five (5) out of five (5) staff records had current first aid certificates and required criminal record clearances or criminal record exemptions. On 10/10/25, five (5) resident records were reviewed. On 10/15/25, two residents’ medication was reviewed. No deficiencies are being cited. Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced visit to the above facility. The purpose of today’s visit was to conduct the one-year inspection. LPA met with Jennifer Bobadilla, Administrator ,and the purpose of the visit was discussed. The Facility is licensed to serve 6 non- ambulatory residents ages 60 and above. Currently there are 5 residents in placement. One (1) resident is receiving hospice services, Two (2) residents have dementia. The facility does not handle any of the residents’ money. This home is a two story home. The 1st floor is where the residents reside and consists of the following: (6) resident bedrooms, (3) Full bathroom, (1) staff room and (1) staff restroom, den, living room, kitchen , dining room, laundry room , attached garage and an outdoor shaded patio and gazebo area. LPA toured the Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place, water temperature measured between 112F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards; doorways were free of obstructions. Kitchen was checked and observed to be within Title 22 regulations. Perishable and non-perishable food supply was checked. All cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. Smoke detectors were working properly and fire extinguisher was fully charged. Carbon monoxide detector was operational. First Aid kit was available. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. During todays visit LPA did not observe any deficiencies. On 10/30/2023 at 1:00 PM, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced required – annual inspection and met with Jennifer Bobadilla, Administrator. Five (5) residents and two (2) staff were present during this inspection. Facility is licensed to serve six (6) non-ambulatory residents. The facility currently has 5 non-ambulatory residents. The facility is a two-story structure located in a residential neighborhood. It has a ramp going alongside front of home. The 1st floor is the facility and 2nd floor is for staff. 1st floor It consists (6) bedrooms, (3) full bathrooms, 1/2 bathroom. Sun room, book/activities area, living room/office area, dining room, kitchen/office area, ramp alongside of living room & dining room also ramp alongside of hallway to bedrooms, shaded back yard, front yard, gazebo, laundry room and 2 car garage. Staff accompanied LPA inside and outside the facility during this inspection. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. There are no security bars or weapons on the premises. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place. Resident bath towels and toiletries were adequately stocked. Common areas were clean and clear of hazards, doorways were free of obstructions. LPA observed one fire extinguisher last serviced August 16, 2023. 5 staff records were reviewed, 5 out of 5 staff records had required criminal record clearances. 2 staff were interviewed. 5 resident records were reviewed and, 5 out of 5 resident records had Admission Agreements, Medical Assessments, and Needs & Services Plans. 3 resident medication records were reviewed. 2 residents were interviewed. Deficiencies are not being cited based on LPA observation, interviews conducted and record review in accordance with the California Code of Regulations, Title 22. An exit interview was conducted.
