Type of visit: Required - 1 Year
Licensing Program Analyst (LPAs) Tena Herrera and Gabriela Castro conducted the required annual inspection. LPA arrived unannounced and was greeted by Maria (Caregiver) and the purpose for today’s visit was explained. Shortly after Assistant Administrator Thomas Trice arrived to assist with visit. The facility is licensed to serve 6 Non-Ambulatory residents ages 60 and above, with a hospice waiver for 2. The facility is a single story home located in Bellflower, Ca. A tour of the facility includes: living room, dining area, kitchen, 5 resident bedrooms (1 shared room), 1 staff bedroom, 4 bathrooms (1 staff, 1 visitor, 1 resident, 1 private in room 4), laundry room, detached garage, tool shed, front yard and back yard. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit todays visit and the initial visit and observed the following: Infection Control: The facility maintains the required Infection Control Plan. Operational Requirements: The facility maintains the required liability insurance. Physical Plant & Environment Safety: LPA toured facility, residents’ bedrooms were checked and closet/drawer space to accommodate each resident comfortably was available. The front yard is free of debris/hazards and the outdoor and passageways are free of obstruction. No bodies of water were observed at the facility. There are no security bars or weapons on the premises. Hygiene products are readily available. Storage areas for cleaning solutions, toxins, knives, and hazardous items are kept in a locked and are inaccessible to residents. The hot water temperature was tested throughout the facility and measured within the required range of 105-120 degrees F. Smoke detectors and carbon monoxide detectors are operable and in compliance. There fire extinguisher was observed and is fully charged. Staffing: There appears to be sufficient staffing at all times in the facility. With night staff that is trained and able to assist in care and supervision of the residents in the case of an emergency(Continued on LIC809-C) Personnel Records-Training: Staff has criminal record clearance, current First-Aid/CPR/AED training along with training in postural supports, medication assistance, dementia, hospice and other ongoing training are documented in personnel files. LPA reviewed 4 staff files with no issues. Administrators Thomas Trice and Liza Trice have valid certificates that expire in Dec. 2026. Resident Records-Incident Reports: Resident files are kept in a secure location and have the following documents in their files - Pre-admission appraisal/Appraisal Needs & Services Plan, Admission Agreements, Identification & Emergency Information and current Physician's Report. LPA reviewed 6 Resident Files with no issues observed. Residents Rights-Information: Residents are provided with telephone and internet at the facility. The facility has the following posters posted: Residents Rights, Complaint Poster, and Ombudsman. Planned Activities: Facility provides scheduled activities and have a variety of activities to choose from within the facility. There is an outdoor activity area available for the residents. Food Service: The kitchen was observed for the ability to prepare and serve food. LPA observed an appropriate food supply of two (2) days of perishables and one week (7 days) of non-perishables. Incidental Medical & Dental: Medication is properly labeled and are centrally stored in a closet and are in their original containers. Disaster Preparedness: The facility has an Emergency Disaster Plan with contact numbers and at least 2 relocation sites. Last Fire Drill was conducted on 9/2/25. Residents with Special Health Needs: Facility has the required "Oxygen in Use" signs posted. The facility has 1 Resident using Hospice Services and all information is properly documented. Per California Code of Regulations, Title 22, and California Health and Safety Code, there were no deficiencies observed during the visit. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Tena Herrera conducted the required annual inspection. LPA arrived unannounced and was greeted by Percy (Caregiver) and the purpose for today’s visit was explained. Shortly after Assistant Administrator Liza Trice assisted with visit. The facility is licensed to serve 6 Non-Ambulatory residents ages 60 and above, with a hospice waiver for 2. The facility is a single story home located in Bellflower, Ca. A tour of the facility includes: living room, dining area, kitchen, 5 resident bedrooms (1 shared room), 1 staff bedroom, 4 bathrooms (1 staff, 1 visitor, 1 resident, 1 private in room 4), laundry room, detached garage, tool shed, front yard and back yard. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit todays visit and the initial visit and observed the following: Infection Control: There is sufficient PPE supplies and Infection Control Plan is maintained at the facility. Operational Requirements: The facility has an approved fire clearance, there is a plan of operation with required Infection Control Plan and facility maintains the required liability insurance. Physical Plant & Environment Safety: LPA toured facility, residents’ bedrooms were checked and closet/drawer space to accommodate each resident comfortably was available. The front yard is free of debris/hazards and the outdoor and passageways are free of obstruction. No bodies of water were observed at the facility. There are no security bars or weapons on the premises. Hygiene products are readily available for clients. Administrators Thomas Trice and Liza Trice have valid certificates that expire in Dec. 2024. Resident Records-Incident Reports: Resident files are kept in a secure location and have the following documents in their files - Pre-admission appraisal/Appraisal Needs & Services Plan, Admission Agreements, Identification & Emergency Information and current Physician's Report. LPA reviewed 6 Resident Files with no issues observed. Residents Rights-Information: Residents are provided with telephone and internet at the facility. The facility has the following posters posted: Residents Rights, Complaint Poster, and Ombudsman. Planned Activities: Facility provides scheduled activities and have a variety of activities to choose from within the facility. There is an outdoor activity area available for the residents. Food Service: The kitchen was observed for the ability to prepare and serve food. LPA observed an appropriate food supply of two (2) days of perishables and one week (7 days) of non-perishables. Incidental Medical & Dental: Medication is properly labeled and are centrally stored in a closet and are in their original containers. Technical violation was issued as the fire drill record was not at the facility, per Assistant Administrator the log was taken to office to scan for record keeping as drill was conducted just last week, all previous logs for drills were documented only the current log was missing. Residents with Special Health Needs: There are no bedridden or residents with postural supports at this facility. Facility has the required "Oxygen in Use" signs posted. Per California Code of Regulations, Title 22, and California Health and Safety Code, the deficiencies observed during the visit will be detailed on the LIC809-D. Mar 29, 2024Complaint investigation reportUnsubstantiated
Allegation investigated: Staff do not give resident medication as prescribed. Staff do not rotate resident as needed.
Licensing Program Analyst (LPA) Luis Mora conducted an unannounced initial complaint visit to determine the validity of the above-mentioned allegations. LPA met with Lisa Trice (Secondary Administrator) and explained the reason for the visit. The investigation consisted of the following: LPA Mora obtained copies of the resident and staff rosters, interviewed Administrator, Staff 1 - Staff 3 (S1 - S3) and Resident 2 - Resident 7 (R2 - R7), and reviewed medication and rotation log for all residents. R1 moved out on December 9th, 2023. The investigation revealed the following: regarding the allegations "staff do not give resident medication as prescribed", it is alleged that facility did not administer R1's medication as needed. Administrator and staff denied the allegation and stated that medication is given as prescribed for all residents. Residents interviewed could not corroborate the allegation. LPA reviewed medication for R2 - R7 for the month of March 2024 and observed that medication is given as prescribed. (Continued to LIC 9099-C) Unsubstantiated Regarding the allegations "staff do not rotate resident as needed", it is alleged that R1 is bed bound and not being rotated every 2 hours. Administrator and staff denied the allegation and stated that R1 was not bed bound and spent most the time on a wheelchair in the living room and when R1 was on the bed they provided rotation every 2 hours. Review of R1's physician report dated 04/21/2023 shows that R1 was not bedridden instead R1 was non-ambulatory. LPA observed 4 out of the 6 residents in wheelchairs in the living room. The other 2 residents were in bed and stated that staff do come and rotate them. All residents interviewed could not corroborate the allegation. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. Allegation investigated: Staff are billing resident for TV services not provided.
Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced 10-day complaint visit to investigate the allegation listed above. LPA met with Caregivers, Marites Padolina and Percy Veranda Ramos and explained the purpose of the visit. Administrator Thomas Trice arrived at 1:30pm and assisted LPA with the investigation. The investigation consisted of the following: LPA toured the facility and obtained copies of the Staff & Resident Rosters, Cable Invoice (Sep. 2023-Dec. 2023) and R1's Itemized Invoice (Nov.1, 2023). LPA obtained and reviewed Resident #1 (R1) files such as; Identification and Emergency Information (FACE Sheet), Admission Agreement (01/15/2022), Physician's Report (04/01/2023), Resident Appraisal (01/15/2022), and Needs and Services Plan (09/24/2023). LPA conducted interviews with Staff #1 (S1) - Staff #3 (S3) and Resident #1 (R1) - Resident #5 (R5). *****CONTINUED ON LIC9099-C***** Unsubstantiated The investigation revealed the following. In regards to the allegation: “Staff are billing resident for TV services not provided.” It is alleged that staff invoiced resident for TV services not provided. It is also alleged that the staff are limiting the cable channels. Interviewed staff denied the allegation. S1 stated that there are 125 TV/cable channels available for all the residents to watch. S1 stated that R1 accessed and watched pay-per view channels at least 3x since Sep. 2023. When S1 received the cable bill for September 2023, S1 told R1 that only basic cable TV was included in their agreed services. S1 stated that R1 continued to watch pay per view shows 2 more times (Oct 2023-Dec 2023) and S1 reminded R1 again. S1 stated that R1 was only talked to but never invoiced for the extra cable charges incurred. Interview conducted with W1 revealed that she told R1 that Netflix and Amazon Prime are also available to her, but R1 refused to watch those shows. LPA reviewed the itemized invoice dated 11/01/2023 listing the services provided such as care and supervision, food services, assistance with ADL, medication management, assistance with mobility tasks and and cable TV (no extra charge billed). However, according to S1 the cable TV included on the invoice was for basic cable TV only and not pay-per-view shows. S1 stated that was also explained to R1. S1 also stated that S1, W1 and R1 met recently to discuss the TV services and R1 has agreed to pay extra for any future shows that R1 watch on pay per view channels. (4) of (5) residents interviewed stated that they are satisfied with the TV channels provided by the facility and have no concerns. During the tour, LPA observed that R1 has access to numerous cable channels on her TV, including other streaming services. LPA reviewed R1's documents and showed that R1 was admitted to the facility on 01/15/2022. Records revealed that the staff provided R1 the services paid for based on R1's level of care listed on the Admission Agreement which R1 signed. It also revealed on the reviewed documents that the optional service to be provided was incontinence care, but did not include R1's access to pay per view channels. Based on statements and interviews conducted with residents and staff as well as reviewed files and documentation, there was not enough supportive evidence to corroborate the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Bennette Pena conducted the required-1 year inspection. LPA was allowed entry by Jaime Patena, Caregiver and Perseveranda Ramos and explained the purpose of today's visit. Administrators, Thomas Trice and Liza Trice arrived at 10am and assisted LPA with the inspection. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following: Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were observed. The facility has submitted a COVID-19 Mitigation Plan and Infection Control Plan and was reviewed. Common area surfaces are being cleaned and disinfected on a regular basis. Bathrooms have soap and paper towels. Staff are adhering to infection control requirements. Operational Requirements: A current Plan of Operation was reviewed. The Infection Control Plan has been added to the Plan. A fire clearance is in place. Liability Insurance policy in the amount of $1,000,000.00 each occurrence and #3,000,000.00 in the total annual aggregate is valid and will expire on 09/01/2024. Administrator Thomas Trice stated that facility does not handle cash resources for the residents. The last fire drill was conducted on 12/07/2023. Care and supervision to meet the residents needs was observed. Special equipment and supplies to meet the persons with special needs were observed. Physical Plant/Environment Safety: The facility is a single story home located in a residential neighborhood that is licensed to serve 6 non-ambulatory residents ages 60 and over. Facility has hospice waiver approved for 2. Current census is six (6) non ambulatory. Home consists of five (5) resident bedrooms, one (1) staff bedroom, (4) bathrooms, living room, dining room, family room, kitchen, backyard, and a detached garage. The interior and exterior physical plant was inspected. Exit doors are free of any obstruction and there are no pools or large bodies of water. LPA observed broken flooring supplies, trash and other miscellaneous items in the side yard. Smoke and carbon monoxide detectors are operational. LPA observed the fireplace in the living room was not adequately screened. One (1) fire extinguisher located in the kitchen entrance was serviced on 12/08/2023. Administrator stated that the laundry dryer is broken, but have already contacted a service technician to fix it. Service technician is scheduled to come in today, 12/09/2023 or tomorrow, 12/10/2023. All bathrooms toured were observed to be fully stocked with hand soap, and paper towels, and had the required grab bars and nonskid mats in place. All showers in bathrooms accommodate non-ambulatory clients. At 10:15am, hot water temperature readings were measured which are within the required 105-120 degrees Fahrenheit. Medication was observed to be centrally stored in the kitchen cabinet. ***CONTINUED ON LIC 809-C** Staffing: A total of seven (7) caregivers including the two (2) Administrators provide care and supervision to the residents. Staff employed are over the age of 18 and have criminal background clearance, fingerprint cleared, have the required training and associated to the facility. Personnel Records-Training: Administrator certificate is valid and will expire on 12/01/2024. Three (3) staff files were reviewed for criminal background clearance and training. Personnel records have health/TB screenings and First Aid/CPR training. Resident Records-Incident Reports: Three (3) resident files were reviewed containing admission agreements, Physician's Report, Medical/Functional assessments, Needs and Services Plans, TB clearance, Personal rights, Medical Consent, Medication Records, Restricted Health Care Plans and Hospice Notes/Records were reviewed. Resident Rights-Information: Resident personal rights are posted. Physician order for use of 1/2 half and full bed rails were reviewed in (6) resident's files. One (1) resident did not have a written order from the Physician indicating the need for 1/2 (half) bed rail. Planned Activities: There is sufficient space to accommodate both indoor and outdoor activities. Administrators stated that the facility does not have dementia residents. However, dementia is part of the training for direct care staff and is included in the Plan of Operation. Food Service: The kitchen was inspected and has sufficient supply of 2-day perishable & 7 day non-perishable food. All sharps were observed to be stored in a kitchen drawer which was locked and inaccessible to residents. Kitchen, food preparation area, and storage areas were observed to be clean and sanitary. The food is properly stored in the refrigerator. Pesticides and cleaning supplies are kept away from the food preparation areas. LPA observed that the kitchen sink base cabinet is not kept clean and close to breaking. Additionally, the base molding on one side of the wall in the kitchen is broken. LPA observed cleaning supplies and hazardous materials were stored in the food storage area in the detached garage. Plates, cups and utensils are kept cleaned and stored properly. Incident Medical and Dental: All residents have Restricted Health Care Plan and Needs and Services Plan on file. Currently, Home Health personnel service five (5) out of six (6) of the residents in the facility. Residents medication are centrally stored in a locked cabinet in the kitchen. Four (4) residents' medications were reviewed to confirm medication is given as prescribed and is documented properly. Disaster Preparedness: The facility has a complete Emergency Disaster and Mass Casualty Plan containing emergency evacuation, storage and preservation of medications, operation of manual assist devices. The facility conducts emergency drill on a monthly basis for all staff and residents. Residents with Special Health Needs: Five (5) out of the six (6) residents are receiving home health services. Postural support physician orders are on file. Two (2) half bed rail and two (2) full bed rail for mobility assistance was observed in four (4) residents in bedrooms #1, #3, #4 and #5. One (1) resident did not have a written order from the Physician indicating the need for 1/2 (half) bed rail. LPA observed that there are no oxygen in use signs posted in bedrooms #4 and #5 and both residents in the rooms are using oxygen. Individual Service Plans and Appraisals for residents are on file.
