On 04/16/26, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Licensee Leia Dimalanta. LPA explained the purpose of today’s visit. The facility is licensed to serve six residents ages 60 and above. Of which six may be non-ambulatory. During today's visit, LPA toured the facility, reviewed five resident records, reviewed two resident medications, and facility documents. Deficiencies were observed, see LIC809-D. During the facility tour, LPA measured the water temperature between 128 - 130 degree Fahrenheit for both bathrooms. During record review, Resident #3's (R3) medical assessment (02/15/26) revealed R3 as bedridden. R3 is non-weight bearing for a total of six weeks. Licensee indicated R3 was temporarily in brace/cast and is non-ambulatory. R3 has not been reassessed. Due to insufficient time, an annual continuation is required. Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced visit to the above facility. The purpose of today’s visit was to conduct the one-year inspection. LPA met with Leia Dimalanta, Administrator and the purpose of the visit was discussed. Facility is licensed to serve (6) non- ambulatory residents age 60 and over . Currently the facility has 6 residents. Two of the residents are diagnosed with dementia, no residents are receiving home health or hospice care services. The facility does not handle any of the residents’ money. This home is a single story home consisting of: (6) resident bedrooms, (2) Full bathroom, living room, kitchen with dining area, laundry area and an outdoor shaded patio area. LPA toured the Resident bedrooms which had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place, water temperature measured between 120F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards; doorways were free of obstructions. Kitchen was checked and observed to be within Title 22 regulations. Perishable and non-perishable food supply was checked. All cleaning solutions, hazardous items, and medications were securely locked and inaccessible to residents. Smoke detectors were working properly and fire extinguisher was fully charged. Carbon monoxide detector was operational. First Aid kit was available. Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. On 2/29/2024, Licensing Program Analysts (LPA) Darneisha Cross and Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Leia Dimalanta / Licensee. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) residents ages 60 and above. Of which (6) may be non-ambulatory . Facility has an approved hospice waiver for (1) and Bedridden for (1) but it was placed on the duplicate facility. The facility is a single-story structure located in a residential neighborhood. It consists of (6) bedrooms, (2) full bathrooms, shaded back yard, front yard, laundry room, and unattached 2 car garage. LPA toured the physical plant with staff. There were no bodies of water or obstructions on the premises. A total of (5) rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident’s personal belongings was observed. Bathrooms were found to be within Title 22 regulations and were operational. LPA inspected the carbon monoxide detectors combo were in operable conditions. The water temperature measured: Kitchen 126.4°F, Bathroom #1:125.°F. Evaluation Report Continues LIC 809-C LPA Iniguez observed the facility to be clean, sanitary, and appropriately furnished at the time of the visit. Storage areas for personal hygiene were observed, cleaning agents found unlocked underneath bathroom sink. Sharps objects were stored and not accessible to residents. The kitchen was inspected and there is sufficient perishable and non-perishable food available maintained properly. All fire extinguishers were charged and were operable. A review of (3) residents' service files, (3) staff personnel files were checked. (3) Medication Administration Records (MAR) were reviewed no discrepancies were found. First AID kit was checked. Last fire disaster drill was on:2/13/2024. LPA observed the facility's infection control practices. Liability insurance was provided to LPA during visit. Facility Annual Fees Current. During annual inspection, LPAs discovered that they are two different facility numbers under the name of Americare Assisted Living. LPA advised the licensee to follow up with her LPA caring case about separating both facilities. Deficiency cited under California Code of Regulations, Title 22, Division 6, Chapter 8.
