Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Becky Mann made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator Delcie Mucha, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of 20, a current census of 19. LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility does not have a swimming pool or similar bodies of water. The facility has sufficient lighting and is maintained at a comfortable temperature of 73 degrees Fahrenheit. The facility has sufficient indoor and outdoor space for resident activities. The facility is equipped with operating smoke detectors/carbon monoxide, working laundry equipment, and telephone service. Residents showers, toilets, and hand washing areas were operating properly. The hot water temperature in three (3) resident bathrooms measured between 112, 113 and 114 degrees Fahrenheit. Resident bedrooms are equipped with beds, mattresses, linens, chairs, dresser, storage space and sufficient lighting. The facility has sufficient linens, towels, and personal hygiene items for residents. The facility has posted in a common area, facility license, administrator certificate, facility sketch, emergency telephone numbers, activities calendar, personal rights, Admissions Agreement, CCLD complaint poster and Ombudsman poster. Overall, the facility is clean, in good repair, and operating in a safe condition for residents in care. Food Service: Facility kitchen and dining area are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents. Sharps and chemicals were kept locked and inaccessible to residents in care. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. Record Review: LPA reviewed (4) resident files for admission agreements, updated physician reports, and needs and services plans. Resident files are maintained and up to date. LPA reviewed (3) resident medications. Medications are labeled and administered as prescribed. Medications are kept locked and inaccessible to residents in care. LPA also reviewed (4) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings. Facility staff records are up to date. Based on the observations made during today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator Delcie Mucha, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (20), a current census of (17). LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility does not have a swimming pool or similar bodies of water. The facility has sufficient lighting and is maintained at a comfortable temperature. The facility has sufficient indoor and outdoor space for resident activities. The facility is equipped with operating smoke detectors/carbon monoxide alarms, last checked by San Bernardino Fire on 2/04/25, working laundry equipment, and telephone service. Resident’s showers, toilets, and hand washing areas were operating properly. The hot water temperature in five (5) resident bathrooms measured between 108.8 and 115.6 degrees F. Five (5) resident’s bedrooms had beds, bed linen, chairs, dresser, storage space and sufficient lighting. The facility has sufficient linens, towels, and personal hygiene items for residents. The facility has posted in a common area, facility license, administrator certificate, facility sketch, emergency telephone numbers, activities calendar, house rules, personal rights, Admissions Agreement, staff schedule, CCLD complaint poster, and Ombudsman poster. Food Service: Facility kitchen and dining area are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents in care. Sharps and chemicals were kept locked and inaccessible to residents in care. Continuation on LIC – 809C: Care & Supervision: Facility has 24-hour/7days a week care staff. Facility staff have current CPR/first aid training. Medical Related Services: Resident’s medications are labeled and centrally stored in a locked cabinet. Record Review: Five (5) Staff files reviewed were observed to be complete. Five (5) Resident files reviewed were observed to be complete. Based on observations and record review no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. Jan 29, 2025Complaint investigation reportUnsubstantiated
Allegation investigated: Staff are mismanaging residents medications Staff left residents in soiled diapers for an extended period of time Staff are not meeting residents hygiene needs Staff are not providing adequate food service to residents Staff are not providing residents with activities
Licensing Program Analyst (LPA) Bernadette Allen made an unannounced visit to the facility to commence a complaint investigation and deliver findings. LPA identified herself to Marlene Santillanes support staff who was informed of the purpose of the visit. Marlene then called Delcie Meadows-Administrator who arrived about 10:05 AM. Delcie was informed of the purpose of the visit and allegations. The investigation consisted of interview with clients, staff members, records review and observations. During the investigation LPA conducted a random audit of the Medication Administration Records (MARS) and found that staff are administering medications as prescribed by the physicians. LPA observed staff ensuring that medications are swallowed at the time given. Interviews with both clients and staff indicated that clients are receiving assistance with their hygiene needs and are not left soiled for extended periods. LPA also observed clients finishing breakfast in the dining area, with clients commenting that the food was good, and portions were adequate. Additionally, LPA observed lunch being prepared and served to clients, with portion sizes appearing sufficient. Unsubstantiated The LPA reviewed records showing that food purchases are made weekly, which appear adequate for the number of clients in care. A tour of the kitchen revealed a variety of food items are available, with a 5-day supply of perishables and a 7-day supply of non-perishables. Based on the evidence gathered during the investigation, the above allegation is found to be Unsubstantiated; meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Magda Malcore conducted and unannouced required annual inspection. LPA met with Elizabeth Mahan, Administrator, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (20) and a current census of (16) residents in care. LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility has no swimming pools or similar bodies of water. Indoor and outdoor activity space is sufficient for resident in care. The facility's outdoor activity space is gated and protected from traffic. The facility has sufficient lighting and is maintained at a comfortable temperature. Resident’s showers, toilets, and hand washing areas were operating in a safe and sanitary condition. The bathrooms were equipped with grab rails and non-skid flooring. The hot water temperature in residents' bathrooms measured at 110 degrees F. Nine (9) Resident’s bedrooms inspected were equipped with beds, bed linen, chairs, and lighting. The facility has operating carbon monoxide alarms, telephone service, laundry equipment and signal system. The facility has sufficient linen and personal hygiene items for residents. The facility has posted in a common area, Community Care Licensing complaint poster, Ombudsman poster, Personal Rights, Theft and Loss Policy, disaster evacuation exit plans and emergency telephone numbers. Sharps, disinfectants, and cleaning solutions were kept locked and inaccessible to residents in care. Care & Supervision: Facility has 24-hour, 7 days a week care staff. Food Service: Facility kitchen and dining areas are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents in care; However, one of the facility's freezers used to store perishable food items was not maintained at a temperature of 0 degree F. The freezer tested at 28 degrees F. The Administrator moved the perishable items out of the freezer into another properly operating freezer and stated the she will have the refrigerator serviced. Deficiency cited. Record Review: LPA review of five (5) staff files and five (5) resident files reveals the following: staff #1 (S1) did not have a health screening signed by the examining physician. Resident #1 (R1) did not have a current physician's reports or medical assessment conducted within 12 months on file as required due to R1's cognitive condition. The facility’s liability insurance is current. The facility’s last fire drill was conducted on 12/21/23. Medical Related Services: Resident’s medications are labeled and centrally stored in a locked room. The facility has a complete first aid kit. Based on observations and record review, deficiencies are being cited per Title 22, of the California Code of Regulations.
