Licensing Program Analysts (LPAs) E. Conchas and A. Martinez conducted an unannounced visit to the facility for an annual inspection. LPAs met with Licensee Maria Puraci, Administrator, Livius Puraci, and Caregiver, Mariana Baloiu. LPAs explained the purpose of the visit, conducted a tour of the facility and review staff and resident files. The facility is an 8 bedroom, 7 bathroom house. During tour of the facility, LPAs observed no bodies of water on the property. The facility is equipped with one (1) fire extinguishers purchased on 5/19/2025. The facility had operating smoke detectors and a carbon monoxide alarm. Outdoor and indoor passageways were kept free of obstruction. The outside of the facility had a shaded area with seating. LPAs observed residents’ bedrooms having the required furniture and functional lighting. LPAs toured the client bathrooms to be operating in safe and sanitary conditions. LPAs measured the hot water temperature in the bathrooms to be between 107.6-110.4 degrees F. The facility had a complete first aid kit. The facility had an current infection control plan and a current emergency disaster plan and drill which was conducted on 3/3/2026. LPAs reviewed staff and resident files. Staff 1 (S1) had the required documentation and training on file. Resident 1 (R1) and resident 2 (R2) did not have completed documentation required in file. The appraisal and needs and service plan were missing. A deficiency was cited. LPAs reviewed resident medications. LPAs observed medications for R1 and R2 had no prescription available; a deficiency was cited. *** Continued on LIC 809-C*** LPAs observed cleaning supplies, medications, and sharps were kept locked and inaccessible to the clients. Centrally stored medications were kept in a safe and locked place. The facility had sufficient food for residents in care. LPAs observed an expired container of food; a technical was issued. An exit interview was conducted where this Facility Evaluation Report was discussed and Plans of Correction were developed with Licensee. Licensing Program Analyst (LPA) Michelle Echeverria arrived unannounced to conduct the required annual visit to the facility. LPA met with Administrator/Licensee, Maria Puraci and introduced self and stated purpose of the visit. LPA was informed that there are currently 6 residents and 2 caregivers present. The facility has (8) bedrooms with private toilets, (2) bathrooms, living room, family room, (2) dining areas, kitchen, backyard, detached garage with laundry area outside. LPA completed a walk through of facility, review of records and medication audit. Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 76 degrees fahrenheit. LPA inspected resident bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, chairs and sufficient lighting. LPA inspected resident bathrooms; bathrooms were clean and appliances were found functional. Water temperatures tested at 110.4 degrees fahrenheit. The facility is equipped with operational smoke detectors, carbon monoxide alarms, fire extinguisher and first aid kit. Posters such as; the personal rights, ombudsman, license and CCL complaint poster were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for resident/staff files. Medications were observed secured and inaccessible to residents. There are no guns, ammunition, swimming pool or bodies of water in the facility. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Facility has a wide variety of food available for residents. Dishes, cups, and utensils were also stored properly. Yards/Outside: One shaded patio, a side gate with self-latching handle on the front of the house that leads into the backyard, and a detached garage with laundry area outside. All outdoor pathways were free of obstructions. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. All staff members working in the facility have criminal record clearance through the department. Record Review: LPA reviewed resident files for admission agreements, updated physician reports, and needs and services plans. LPA also reviewed staff and administrator's file for First Aid/CPR certification, criminal record clearance, trainings, and health screenings. Medications were audited at random and appeared to be dispensed appropriately by staff. LPA observed that the facility did not have a record of emergency drills. Deficiency issued. LPA reviewed the facility's emergency disaster plan, infection control plan and liability insurance. One deficiency was cited during this visit. Licensing Program Analyst (LPA) Paola Guerrero made an unannounced visit to the facility. The purpose of the visit was to conduct a required comprehensive annual inspection. LPA met with Facility Caregiver Mary Puraci and was granted entry to the facility. At the time of the visit there was two (2) staff present, and four (4) residents present. The facility is a six (6) bedroom, five (5), bathroom home, with a kitchen/dining area, living room, with open and detached garage. The facility is a Residential Care Facility for Elderly (RCFE) Licensed capacity is (6) current census (4). LPA was accompanied by Facility Caregiver Mary, to conduct a general overall inspection, which included, but was not limited to, the following: Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature. LPA inspected resident’s bedrooms; they are equipped with required furniture such as: mattresses, nightstands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. LPA measured and observed the water temperatures in the bathrooms to be 110.3 degrees F The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care. There was a designated storage space for resident/staff files. Medications are kept inside medication cabinet inaccessible to residents in care. Overall, the facility is clean, in good repair, and operating in safe conditions for residents in care. Food Service: Non-perishable and perishable food supply is sufficient for number of residents in care. Care & Supervision: Facility has sufficient care staff for coverage 24 hours a day, 7 days a week. All staff members working in the facility have criminal record clearance through the department.
