Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator Bien Tandoc, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6), a current census of (3). LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: Indoor and outdoor passageways are free of obstruction. The facility does not have a swimming pool or similar bodies of water. The facility has sufficient lighting and is maintained at a comfortable temperature. The facility has sufficient indoor and outdoor space for resident activities.The facility is equipped with operating carbon monoxide alarms, however no fire alarms; deficiency issued. Facility has working laundry equipment and telephone service. Resident’s showers, toilets, and hand washing areas were operating properly. The hot water temperature in two (2) resident bathrooms measured at 116.7 degrees F. Three (3) resident’s bedrooms had beds, bed linen, chairs, dresser, storage space and sufficient lighting. The facility has sufficient linens, towels, and personal hygiene items for residents. The facility has posted in a common area, facility license, administrator certificate, facility sketch, emergency telephone numbers, activities schedule, menu, personal rights, CCLD complaint poster, and Ombudsman poster. Food Service: Facility kitchen and dining area are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents in care. Sharps and chemicals were kept locked and inaccessible to residents in care. Continuation on LIC – 809C: Care & Supervision: Facility has 24-hour/7days a week care staff. Facility staff working have current CPR/first aid training and background cleared. Medical Related Services: Resident’s medications are labeled and centrally stored in a locked cabinet. Three (3) residents medication were audited, medication was pre-poured into clear containers, deficiency issued. Record Review: Two (2) Staff files reviewed were observed to be complete. Three (3) Resident files reviewed were observed to be incomplete, Resident #1 (R1) had missing needs and service plan, deficiency issued. Last disaster drill was conducted Jan 2026. Based on observations and record review deficiencies were cited per Title 22, Division 6, of the California Code of Regulations. An exit interview was conducted, licensing reports (LIC 809/C, LIC 809D, LIC 811) along with appeal rights were discussed and provided to Administrator Bien Tandoc at the conclusion of the visit.the state’s words, verbatim · CDSS document, Apr 9, 2026

Licensing Program Analyst (LPA) Magda Malcore conducted a follow-up visit in relation to Redlands Fire Department inspection results submitted to the Regional office on 5/06/24. LPA Malcore met with Bienvenida Tandoc Jr, Administrator, and discussed the purpose of the visit. On 5/01/2024, the Redlands Fire Department conducted an inspection due to a change of Ambulatory status submitted by the facility. The facility currently has an approved fire clearance for 6 ambulatory and the facility requested a fire clearance for 6 nonambulatory status. Based on Redlands Fire Department inspection report, structural changes would need to be made before a change of ambulatory status maybe cleared by the Fire Department. The Administrator stated that on 5/06/24, she consulted an contractor to have french doors with direct access to the backyard constructed in bedroom #1 and bedroom #2. The Administrator stated that bedroom #1 and bedroom #2 will be shared rooms. Bedrooms #3 and #4 will remain ambulatory. The Administrator stated she will have an updated medical assessment for resident #1 (R1) to confirm their ambulatory status as the report on file is dated 10/11/2019, prior to a possible eviction. The Administrator was advised all construction documentation and status shall be submitted to the licensing within a timely manner. Licensing Program Analyst (LPA) Magda Malcore made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Bienvenida Tandoc Jr, Administrator, and discussed the purpose of the visit. The facility is a Residential Care Facility for the Elderly (RCFE) with a license capacity of (6) and a current census of (5) residents in care. The facility has a hospice waiver for (2) residents. LPA conducted an overall inspection of the facility, which included, but was not limited to, the following: Physical Plant/Environment: LPA observed that the Licensee is not operating within the limitation of their license. The Redlands Fire Department approved the facility license for six (6) Ambulatory residents; however, the five (5) residents in care are unable to ambulate without staff assistance and/or without the assistance of a mechanical aid. Indoor and outdoor passageways are free of obstruction. The facility has no swimming pools or similar bodies of water. Outdoor shaded area is sufficient for resident activities and is enclosed with a self-latching gate.The facility has sufficient lighting and is maintained at a comfortable temperature. Resident’s showers, toilets, and hand washing areas were operating in a safe and sanitary condition. The hot water temperature in residents' bathrooms measured 107 degrees F. The facility has operating carbon monoxide alarms. The facility has sufficient linen,towels, and personal hygiene items for residents. The facility has posted in a common area, Community Care Licensing complaint poster, Ombudsman poster, Personal Rights, disaster evacuation plan and emergency telephone numbers. Sharps, disinfectants, and cleaning solutions were kept locked and inaccessible to residents in care. Food Service: The facility kitchen and dining areas are maintained clean. The facility has sufficient non-perishable and perishable food supply for residents in care. Cleaning solutions and other toxins are store away from the food supply. Care & Supervision: The facility has 24-hour, 7 days a week care staff. Record Review: LPA review of staff files reveal, staff#1 (S1) and staff#2 (S2) did not have current first aid/CPR training on file. LPA review of resident files reveal, resident #1 (R1) did not have a physician's report/medical assessment on file for review. LPA record review also reveals an emergency drill with staff has not been conducted quarterly. The facility’s last drill was conducted was on 4/1/23. Medical Related Services: Resident’s medications are centrally stored and kept locked. Based on LPA observations and record review, deficiencies are being cited per Title 22, of The California Code of Regulations and Health and Safety Codes. An immediate civil penalty is also being assessed today and fees will continue to accrue if the deficiency is not corrected.
