On 03/27/2026 at 8:30AM, Licensing Program Analyst (LPA) Andrew Christy arrived unannounced to conduct the 1-Year Annual Required inspection. LPA met with Administrator, Helen Herbert, and explained the purpose of the visit. The facility currently is currently at max capacity with six (6) residents. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All outdoor and indoor passageways are kept free of obstruction. No bodies of water were observed. A comfortable indoor temperature is maintained at 69.0 degrees Fahrenheit. The hot water temperature in the residents’ shared bathroom was measured at 109.9 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of non-perishable and 2 day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 06/24/2025. At 9:45AM, LPA reviewed five (5) resident files and five (5) staff files, all found to be complete. The emergency disaster plan was last reviewed 01/30/2026. Quarterly emergency drills were last conducted 01/23/2026. A review of resident medications and the Medication Administration Record (MAR) found no outstanding errors. No deficiencies cited during visit. On 02/05/2024 around 09:15 AM, Licensing Program Analyst (LPA) L. Holmes arrived unannounced to conduct a required annual inspection. Helen Herbert, Administrator (ADM) was notified. LPA met with Araceli Emerick, Staff (S1) and explained the purpose of the visit. ADM who currently holds a Standard Certificate #(7000752740) that expires on 06/12/25. The facility’s fire clearance was approved for six (6) ambulatory clients; three (3) may be non-ambulatory and three (3) may be bedridden. Upon arrival, LPA observed two (2) staff attending to four (4) residents dining and watching television. LPA toured the facility with S1 including, but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of five (5) total bedrooms which four (4) are occupied by the residents and one (1) by staff. All outdoor and indoor passageways were free of obstruction. There are not any bodies of water. A comfortable temperature was maintained at 71 degrees Fahrenheit (F). LPA observed lighting in all rooms to be adequate for the comfort and safety of the residents. Hot water temperature in the shared residents bathroom was measured at 116.2 degrees (F). Toilet, hand washing, and bathing areas were safe, sanitary and in operating condition. Hand washing poster, paper towels, and soap observed at all hand washing stations. Linen and hygiene products were available for residents. PPE, sanitizer, and paper goods are sufficient. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher last inspected 06/13/24. Emergency Disaster Plan is updated. First aid kit observed complete. Fire drill was last conducted on 12/2024. Six (6) resident and four(4) staff records were reviewed, all staff have criminal record clearance. No deficiencies cited during visit. LPA met with Caregiver, Carolina Darcen and explained the purpose of the visit. Carolina phoned Licensee/Administrator, Helen Herbert, to inform. Administrator, Helen, arrived shortly after. The facility’s fire clearance was approved for capacity six (6) residents. In which three (3) may be Bedridden. Hospice waiver approved for two (2). Administrator Certificate #6003256740 Expired 06/12/2023. LPA toured facility with Helen including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 6 total bedrooms which 4 bedrooms are occupied by the residents and 2 bedroom is occupied by staff. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 69 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 120.8 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. LPA reviewed 4 staff records and 3 of 4 have current first aid training and associated to the facility.
