At approximately 9:30 AM, Licensing Program Analyst (LPA) Magdaleno arrived unannounced to conduct a required 1-year annual inspection and met with Administrator Silvana Daniel. Facility is a Residential Care Facility for the Elderly (RCFE) with three (3) residents in care. Facility has a Dementia Care Plan, a Hospice waiver for two (2), with zero (0) Hospice residents currently in care, and is approved for all non-ambulatory residents. LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 9:50 AM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one (1) story home, was a comfortable temperature, and passageways were free from obstructions. Facility's fire extinguisher was observed charged and last serviced 11/25. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperature measured 108.1 degrees F, which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, hygiene, incontinent care, and paper products available for residents. Residents' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPA observed at least a two (2) day supply of perishable and seven (7) day supply of non-perishable food. Food was found to be stored in a safe manner with open items covered. There is a shaded seating area in the backyard with outdoor space for activities. Facility has an internet access device and internet available to residents in care, and the phone was observed operational during today's inspection. Continued LIC809C... Continued from LIC809... LPA observed a supply of PPE, emergency supplies, a first aid kit, and flashlights. At approximately 10:35 AM LPA conducted a review of three (3) resident records and medication records. All required documentation present. Medication is centrally stored and locked. At approximately 11:15 AM LPA conducted review of two (2) staff records. All required documentation present. At approximately 12:00PM, Licensing Program Analyst (LPA) Felias arrived unannounced to continue a 1-Year Required Visit and met with Administrator, Steven Daniel. Licensee, Silvana Daniel, arrived during visit at approximately 12:15PM. Facility serves residents with dementia and has a plan of operation for dementia care and programming on file. Facility has an approved fire clearance and capacity for 6 non-ambulatory residents. Facility has an approved hospice waiver for 2 individuals. Upon arrival, LPA was informed that there were 5 Residents in care. LPA reviewed staff files and medication. Staff files had current first aid and CPR certification. Medications were centrally stored and secure. Administrator's Certificate for Silvana Daniel (7036051740) was current with an expiration date of 03/21/2027. Administrator's Certificate for Steven Daniel (7036052740) was current with an expiration date of 08/26/2026. LPA also cleared deficiency cited on 02/22/2024. At approximately 12:15PM, Licensing Program Analyst (LPAs) Deniz and Felias arrived unannounced to conduct a 1-Year Required Visit and met with Licensee/Administrator, Silvana Daniel. Facility serves residents with dementia and has a plan of operation for dementia care and programming on file. Facility has an approved fire clearance and capacity for 6 non-ambulatory residents. Facility has an approved hospice waiver for 2 individuals. Upon arrival, LPAs was informed that there were 5 Residents in care. At approximately 12:50PM, LPAs reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 1:15PM, LPAs conducted a walk-though of the facility with Licensee/Administrator. LPAs observed the following: The facility was found to be clean and at a comfortable temperature with all exits free from obstruction. Facility had emergency lighting. Facility is a 1 story building with 5 Resident bedrooms, 1 staff bedroom, 4 bathrooms, and common spaces. Facility has an Infection Control plan on file. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Mattress pads were in place or available for Resident use. Toxins were observed to be stored inaccessible to residents. Bathrooms were equipped with necessary grab bars, and non-slip floors/mats were present. Hot water temperatures for all sinks in facility were found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. LPAs reviewed resident files. Files were all found to be well organized. Administrator's Certificate for Silvana Daniel (6003231740) was shown to be pending with application renewal date of 01/16/2025. Facility's fire extinguisher was last inspected November 2024. Facility smoke detectors and carbon monoxide detectors were tested and operational. Facility's last emergency drill was conducted January 2025. Allegation investigated: Facility is not meeting resident's care needs

Licensing Program Analyst Leibert arrives unannounced for the purpose of delivering findings on this complaint. This investigation included the review of documents; taking of witness statements; site visit to the facility. The following determinations are made: Follow-up interview with the Complainant indicates that there was no intent to lodge a complaint against the facility and that the referral was made in hopes of obtaining assistance in locating a placement for R1 in a higher level of care; The Complainant and the Administrator indicate that the Administrator was not properly informed by the placing agency of the extend of care required for R1 and that financial issues precluded R1's placement in skilled nursing; The Physician's Assessment Report(LIC 602) provided to the Administrator does not accurately describe R1's behaviors; The Complainant and the Nurse Case Manager familiar with R1's care at the facility state that the facility provided excellent care to R1 and made every effort allowed under the Administrator's license to meet the care needs of R1. Based on statements and records, we have found the complaint to be UNFOUNDED, meaning that the allegation is false and without a reasonable basis. The complaint is DISMISSED. Upon arrival, LPA was informed that there were 4 Residents in care and 1 staff member on-site. At approximately 10:00AM, LPA reviewed the Facility's Staff Roster and found that all staff on-site were background cleared and associated to the facility per regulation. At approximately 10:15AM, LPA conducted a walk-though of the facility with Licensee/Administrator. Medication was centrally stored and secure. During Medication Review, LPA observed that the facility has been pre-pouring medication for one resident. LPA notified facility that pre-pouring medications is against regulation (See Technical Violation, LIC9102, Regulation 87465(h)(5)).
