On 07/16/2026 at 12:15 PM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct a 1-Year Annual Required Inspection. LPA met with Licensee/Administrator Ritchie Gonzales and explained the purpose of the visit. The facility's fire clearance is approved for thirty-two (32) non-ambulatory residents, of whom five (5) may be bedridden. Administrator Certificate #7013761740 expires on 07/18/2027. LPA toured the facility with Mr. Gonzales, including, but not limited to, four (4) resident apartments, resident bathrooms, activity rooms, the kitchen, dining room, common areas, and the courtyard. No bodies of water were observed on the premises. LPA observed that lighting throughout the facility was adequate to ensure the comfort and safety of the residents. The hallway temperatures measured 73°F and 74°F, and the activity room temperature measured 76°F. The hot water temperature was tested in a sample of shared resident bathrooms and measured 110.1°F, 109.5°F, and 108.1°F. Resident bathrooms were equipped with grab bars and non-skid mats. The facility maintained at least a seven-day supply of nonperishable food and a two-day supply of perishable food. Centrally stored medications, sharps, and toxic substances were secured in locked storage and were inaccessible to residents in care. LIC809-C (Page 2) LPA reviewed five (5) resident records and six (6) staff records. All six (6) of six (6) staff members reviewed had current First Aid training and were properly associated with the facility. Smoke detectors and carbon monoxide detectors were tested and found to be in operating condition during the inspection. The fire extinguisher was last serviced on 07/02/2026. The Emergency Disaster Plan was last updated and posted on 07/16/2026. The first aid kit was complete. Documentation showed the last emergency disaster drill was conducted on 05/28/2026. On 5/7/2025, at 9:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required Annual Inspection of the facility. Upon arrival, LPA stated the purpose of the visit to Medication Technician Randy Gonzales. Licensee / Administrator (ADM) Richie Gonzales arrived at approximately 9:15 AM. The LPA toured the interior and exterior of the facility, inspecting the kitchen, food and emergency supply storage areas, dining area, shared restrooms, community living spaces, client rooms, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. The maximum hot water temperature was 111.3 degrees Fahrenheit and the temperature in the common area was 77.5 degrees Fahrenheit. The carbon monoxide and smoke detectors were fully operational. The fire extinguishers were last serviced on 7/24/2024. The LPA observed postings in the facility that included a complaint poster, Ombudsman, and Personal Rights posters. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA reviewed facility records, records of 5 staff members, and records of 5 residents. No citations were issued during the inspection. On 11/07/2024 at 1:30 PM, Licensing Program Analyst (LPA) L. Alexander conducted an unannounced subsequent complaint visit, to deliver investigation finding of above allegation. LPA met with Licensee/Administrator, Ritchie Gonzales, and explained the purpose of the visit. During the investigation RSO has been identified as not being present in or associated to facility. This allegation is deemed UNFOUNDED. This agency has investigated the complaint and have found that the complaint was UNFOUNDED, meaning that the allegations were false, could not have happened and/or is without a reasonable basis. On 07/24/2024 at 1:00 PM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Activities Director, Helen McCauley and explained the purpose of the visit. Staff phoned Licensee/Administrator, Ritchie Gonzales, to inform. Ritchie arrived approximately an hour later. The facility’s fire clearance was approved for 32 (thirty-two) non-ambulatory residents. There is an approved hospice waiver for 10 (ten) residents. Administrator's certificate #6044801740 expires 07/18/2025. LPA toured the facility with Helen including but not limited to six (6) residents’ apartments, bathrooms, multiple activity rooms, kitchen, common area and courtyard. There are no bodies of water observed. LPA observe lighting in all rooms are adequate for the comfort and safety of the residents. Hallway temperature was maintained at 76 and 73 degrees F. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in a sample of residents’ shared bathroom were measured at 109.7, 111.7, 107.0 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care. On this day at around 12:00PM Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct 10-day investigation and met with Licensee/Administrator, Ritchie Gonzales. LPA explained to Licensee/Administrator the purpose of visit. During the visit, LPA requested the following records: Certificate of Liability Insurance. LPA received Premium Finance Agreement and Certificate of Liability Insurance with Effective Date of Policy 08/01/2023. Based on the information gathered during the course of investigation, the 1 allegation is closed as Unfounded. A finding that the complaint is Unfounded means that the allegation is false, could not have happened, and/or is without a reasonable basis.
