Allegation investigated: Licensee did not maintain adequate emergency supplies as required

On 12/30/2025 at 1:20 PM, Licensing Program Analyst (LPA) Ardalan Gharachorloo arrived unannounced to deliver findings in regard to the allegation above. LPA met with Administrator Ho Fung and explained the purpose of the visit. During the course of the investigation, the LPA toured the facility and confirmed that W1 had previously visited the facility . W1 stated that at the time of that visit, the facility did not have an emergency water supply available for residents. During the LPA’s visit, S1 was interviewed and stated that after W1’s visit, the facility purchased and obtained an emergency water supply. Based on LPAs observations and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Type of visit: Required - 1 Year

On 11/06/2025 at 1:15 PM, Licensing Program Analyst (LPAs) Ardalan Gharachorloo and Greg Clark arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Ho Lim Fung, Administrator and explained the purpose of the visit. LPAs toured the facility including but not limited to 5 residents’ rooms, bathrooms, activity rooms, kitchen, common area and courtyard. There are no bodies of water observed. LPAs observed lighting in all rooms are adequate for the comfort and safety of the residents. Hallway temperature was maintained at 72 degrees F.The hot water temperature in a residents’ shared bathroom was measured at 107 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 11/15/2024. Emergency Disaster Plan was last reviewed and posted on 12/02/2024. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 10/28/2025. LPAs reviewed 5 residents records and 5 staff records, and all were complete. LPA met with Administrator, Ho Lim Fung, and explained the purpose of the visit. LPA toured the facility including but not limited to 4 residents’ bedrooms, bathrooms, activity room, kitchen, common area and backyard. There are no bodies of water observed. LPA observe lighting in all rooms are adequate for the comfort and safety of the residents. Hallway temperature was maintained at 72 degrees F. The hot water temperature in a residents’ shared bathroom was measured at 108 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care. On 11/07/2023 at 10:35 AM, Licensing Program Analyst (LPA) K. Nguyen arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Jessica Ching, Administrator and explained the purpose of the visit. LPA toured facility with Jessica including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of 14 total bedrooms which 12 bedrooms are occupied by the residents and 0 bedroom is occupied by staff. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 110.9 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid tiles. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 11/2/2023. Emergency Disaster Plan was last posted on 1/1/2018. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 11/02/2023. LPA reviewed 4 residents records. LPA reviewed 4 staff records and 4 of 4 have current first aid training and associated to the facility. LPA reviewed a sample of resident’s medications. No deficiencies cited during visit.
