Type of visit: Required - 1 Year

On May 19 2026, at 8:00am, Licensing Program Analyst (LPA) Eboni Bentley conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Bentley was greeted and granted entry by staff after stating the purpose of the visit. Administrator (Admin) Joey Encarnacion was arrived a short time later to assist. Admin Joey Encarnacion has an administrator certificate with an expiration date of April 14, 2028. The facility is a single level structure, licensed for six (6) non-ambulatory residents and has a hospice waiver for five (5) residents. The home consists of the following: Four (4) resident bedrooms, two (2) staff bedrooms, three (3) resident bathrooms, a living room area, den, dining room area, kitchen, outdoor shaded seating area, and an attached two car garage. During the visit, LPA Bentley toured the interior and exterior of the physical plant with staff and the following was observed: There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be clean and operational. The water temperatures in three bathrooms measured between 111.5 degrees F to 118.4 degrees F. A comfortable temperature of 73 degrees F was maintained throughout the facility. LPA Bentley observed the facility to be sanitary and appropriately furnished at the time of visit. Storage areas for cleaning supplies, toxins, and sharps objects were stored and locked. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available. CONTINUE TO LIC809-C...... There is a two car garage with a refrigerator and additional supply of perishable items. The washer and dryer were observed to be in working condition. Facility has a sufficient amount of emergency food supply and water supply. The backyard was clean and free of clutter and debris. A shaded patio area with tables and chairs was observed. Emergency safety drills was last conducted on April 4, 2026 and are being conducted quarterly. First aid kit is maintained and contains all the necessary elements. Smoke and carbon monoxide alarms were tested and observed operational. A working telephone (714-481-0743) remains available, and the facility has a device that can be used for video teleconference purposes. The facility has two (2) fire extinguishers that were charged, mounted, and last serviced on November 11, 2025. Liability Insurance is effective June 5, 2025 through June 5, 2026. LPA Bentley conducted an audit of six (6) resident files (R1-R6), four (4) staff files (S1-S4), and interviews were conducted. A review of the Medication and Medication Administration Record (MAR) was also conducted. Based on today’s observations, no deficiencies are being cited during this visit as per Title 22 Division 6 Chapter 8 of the California Code of Regulations. Type of visit: Required - 1 Year

On May 22, 2025, at 8:00am, Licensing Program Analyst (LPA) Eboni Bentley conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Administrator (AD) Joey Encarnacion was out of town and communicated via telephone during the visit. AD Joey Encarnacion has an administrator certificate with an expiration date of April 14, 2026. The facility is a single level structure, licensed for six (6) non-ambulatory residents and has a hospice waiver for five (5) residents. The home consists of the following: Four (4) resident bedrooms, two (2) staff bedrooms, three (3) bathrooms, a living room area, den, dining room area, kitchen, outdoor shaded seating area, and an attached two car garage. During the visit, LPA Bentley toured the interior and exterior of the physical plant with CG Encarnacion and the following was observed: There were no bodies of water or obstructions on the premises. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available. CONTINUE TO LIC 809-C There is a two car garage with a refrigerator and additional supply of perishable items. The washer and dryer were observed to be in working condition. LPA Bentley conducted an audit of six (6) resident files (R1-R6), four (4) staff files (S1-S4), three (3) staff interviews, and three (3) resident interviews. A review of the Medication and Medication Administration Record (MAR) was conducted. Based on today’s observations, one Type A deficiency and two technical violations were cited during this visit as per Title 22 Division 6 Chapter 8 of the California Code of Regulations. Type of visit: Required - 1 Year

On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz made an unannounced visit on this day for the purpose of conducting an Annual Required-1 year inspection visit. LPA was greeted by Administrator (AD) LIsa Encarnacion and discusses purpose of today's visit. Licensee (L) Joey Encarnacion arrived during today's visit The facility is a one story home which consists of: 2 shared resident bedrooms, 2 private residents bedrooms, 2 caregiver bedrooms, 3 bathrooms designated for residents, 1 bathroom designated for visitors and staff, living-room, family room, kitchen with dining area, back yard with ample shade and outdoor furniture, garage with operational washer and dryer. The facility is licensed for age range 60 and over, approved for capacity of 6 Non-Ambulatory residents and has a hospice waiver for five (5) residents. There are currently (4) one resident receiving hospice care services. (AD) Lisa Encarnacion has a current Administrator's certificate which expired 3/5/2023. (AD) Lisa Encarnacion indicated completed AD renewal, pending certificate and agreed to submit copy of AD Certificate to CCLD upon receiving. LPA along with AD Lisa Encarnacion toured the interior and exterior of facility. LPA observed kitchen for proper sanitation and cleanliness. LPA observed sharp objects locked and secured in cabinet in kitchen area, and observed toxin substances to be locked and inaccessible to residents in care in locked and secured closet in garage area. The food was observed to meet the minimum requirement of two days perishable and seven days nonperishable food supply for residents in care. Resident bedroom areas were observed to have required furnishings including but not limited to: lamp, bed, mattress, night stand and clean linen. LPA interacted and interviewed with residents and staff during today's facility tour. The water temperature in resident bathrooms were measured to be within normal limits. The Grab bars and non-slip mats were observed in resident bathrooms and shower room. Room temperature inside the facility was recorded to be 72 degrees F. The outside grounds were inspected during facility tour, and no bodies of water were observed. No pool available at this facility. CONTINUE LIC 809-C PAGE... CONTINUED... Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. LPAs observed and tested functional and operational carbon monoxide and smoke alarms. LPA Quiroz observed two fire blankets. (L/AD) Joey Encarnacion reports fire drills are conducted quarterly with staff and residents. Last facility fire drill was conducted on 1/5/2024. LPA Quiroz reviewed whether the facility is operating within capacity limitations and reviewed repeated violations, no repeated violations assessed. During today's visit, LPAs reviewed six of six resident records, centrally stored medications and four personnel files. Resident and personnel files were found to be within Title 22 California Code of Regulations. Based on the observations made during today’s visit; there are no deficiencies being cited per Title 22 Division 6 of the California Code of Regulations.
