On February 13, 2026, Licensing Program Analyst (LPA) Eboni Bentley conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Bentley introduced self and was granted entry into the facility by staff, after stating the purpose of the visit. Licensee (LI) Eric Calisher was contacted via telephone and arrived shortly to assist with the visit. The facility is licensed to operate for six (6) non-ambulatory residents of which one (1) can be Bedridden, with a Hospice waiver for four (4) residents. The building is a single-story home located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, two (2) bathrooms, living room area, dining area, kitchen, an outdoor covered seating area, and an attached two car garage. During the visit, LPA toured inside and outside of the physical plant with LI, and the following was observed: There were no bodies of water or obstructions on the premises. All resident bedrooms were clean, well-organized, and had all necessary items and storage spaces. Linens, comforters, and bath towels were adequately stocked at the time of visit. A comfortable temperature of 75 degrees F was maintained in the facility. Bathrooms were found to be clean and operational with water temperatures measured between 107.4 degrees F and 112.2 degrees F. The kitchen was clean and organized. All knives and sharp objects were locked in a drawer, near the stove. A two-day supply of perishable food items and seven-day supply of nonperishable food items was observed. The backyard was clean and free of clutter and debris. The side exit gate was clear and accessible. The garage is being used for storage and a Technical Violation was provided. CONTINUE TO LIC809-C...... The smoke alarms and carbon monoxide detectors were tested and operable. First aid kit is maintained and contains all the necessary elements. Emergency safety drills were last conducted on January 9, 2026. The facility has one (1) fire extinguisher that was charged, mounted, and last inspected on November 17, 2025. Current Liability Insurance was provided during the visit with a policy start date of March 8, 2025 through March 8, 2026. A working telephone (714)593-2687 remains available and the facility has a device that can be used for video teleconference purposes. LPA conducted an audit of five (5) resident files (R1-R5), three (3) staff files (S1-S-3), and medication and medication administration record review. Resident and staff interviews were conducted. On February 10, 2025, at 10:15am, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim was greeted and granted entry by Caregiver (CG) Annabelle Cendana. Administrator (AD) Eric Calisher arrived at the facility around 11:00am and LPA Kim explained the purpose of the visit. The facility is licensed to operate for five (5) nonambulatory residents, of which 1 may be bedridden, and has a hospice waiver for four (4) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: six (6) resident bedrooms, two (2) bathrooms, living area, dining area, kitchen, outdoor covered patio, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with AD Calisher. There were no bodies of water or obstructions in the facility. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, Resident Room 4, Resident Room 5, and Resident Room 6. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 106.0 degrees F to 118.5 degrees F. A comfortable temperature of 78 degrees F was maintained in the facility. LPA Kim observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food both were available and maintained properly. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). The smoke detectors and carbon monoxide detectors were operable. A working telephone (714-593-2687) and video teleconferencing laptop and tablet dedicated to the residents both remain available. Last emergency drill was conducted on January 9, 2025, and are conducted quarterly. First aid kit is maintained and contains all the necessary elements. Evidence of Liability is effective March 8, 2024, and expires March 8, 2025. Emergency food, emergency water, and emergency supplies are stored in the garage. The facility has one (1) fire extinguisher that is charged and mounted in the garage. LPA Kim conducted an audit of resident files (R1-R5), staff files (S1-S7), and medication and medication administration record were in all in order and complete. LPA Kim conducted two (2) resident interviews. No deficiencies were cited during this visit. On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz conducted an unannounced visit for the purpose of conducting a required Annual inspection. LPA was greeted and granted entry into the facility by Caregiver 1 (CG1) and met with House Manager (HM) Mel Rodriguez and discussed purpose of the visit. LPA Quiroz called Administrator (AD) Eric Calisher via telephone and explained the nature of the visit. This facility is licensed to provide services to age range 60 and over, capacity of 6 which 1 can be bedridden, Hospice waiver approved for 4(four) hospice residents . There are five (5) residents in care of which (0) zero residents are receiving hospice care services. Administrator Eric Calisher has an Administrator Certificate with expiration date of 8/9/2025. LPA Rosie Quiroz along with (HM) Rodriguez toured the interior and exterior of the facility. During today's inspection tour, LPA Quiroz observed three of four residents in the living room with staff supervision and 1 of four residents in their room resting. LPA Quiroz interacted and interviewed with staff, visitor and residents during today's visit. LPA Quiroz inspected resident's bedrooms and bathrooms. Water temperatures were recorded to be between 108.5-109.8 degrees Fahrenheit. LPA Quiroz inspected resident’s bedrooms and appeared to be clean. Facility temperature in resident's bedrooms and throughout the facility was recorded to be within normal limits. LPA Quiroz observed the emergency and disaster and evacuation plan. Facility has a supply of emergency food, water and PPE in garage area readily available for staff and residents. LPA Quiroz toured the outside of the facility and observed seating and shaded area backyard for residents and visitor's enjoyment in backyard area. During today's visit, LPA Quiroz provided Consultation on Title 22 and Infection control.
