On January 12, 2026, Licensing Program Analyst (LPA) Eboni Bentley conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Bentley introduced self and was granted entry into the facility by staff, after stating the purpose of the visit. Licensee (LI) Mai Nguyen was contacted via telephone and granted permission of Caregiver Tuyet Nguyen to sign the facility report in her absence. The facility is licensed to operate for six (6) non-ambulatory residents, with a Hospice waiver for two (2) residents. The building is a single-story home located in a residential neighborhood. It consists of the following: five (5) resident bedrooms, two (2) bathrooms, living room area, dining area, kitchen, an outdoor covered seating areas, and an attached two car garage. During the visit, LPA toured inside and outside of the physical plant with staff and spoke with LI via telephone. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. A comfortable temperature of 70 degrees F was maintained in the facility. Bathrooms were found to be clean and operational; however, the water temperature in two resident bathrooms measured between 131.2 degrees F and 139.1 degrees F. The kitchen was inspected and LPA observed four non-operational range burners on the stove, used for preparing meals for residents in care. Deficiencies were being cited. Continues on LIC 809-C ... The smoke detectors and carbon monoxide detectors were operable. First aid kit is maintained and contains all the necessary elements. Emergency safety drills were last conducted on October 12, 2025. The facility has one (1) fire extinguisher that was charged and purchased on January 12, 2026. Current Liability Insurance was provided during the visit with a policy start date of June 17, 2025, through June 17, 2026. A working telephone (714)274-9802 remains available. No emergency food, emergency water, or emergency supplies were observed. A deficiency was cited. The facility does not currently have a device that can be used for video teleconference purposes. LPA provided Technical Assistance. LPA conducted an audit of five (5) resident files (R1-R5), three (3) staff files (S1-S), and medication and medication administration review. Resident and staff interviews were conducted. Based on record review, the facility currently has one bedridden Resident #1 (R1) with a fire clearance for six (6) non-ambulatory only residents. A deficiency was cited during this visit, as per Title 22 Division 6 Chapter 8 of the California Code of Regulations. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Administrator Mai Nguyen. Administrator Catherine Truong Administrator's certificate expires on August 24, 2025. Facility is licensed for 6 non-ambulatory residents and a hospice waiver for 2. Facility is a single story home with 5 bedrooms, 2 bathrooms, kitchen, living room, dining room and a two car garage. LPA observed the fireplace in the living room is screened. LPA observed the PUB 475 poster (See Something, Say Something poster) is 8 1/2 by 11 inches in size and is posted in the entry way of the facility. LPA observed a 2 day perishable and a 7 day non-perishable food supply on hand in the kitchen. LPA observed a 3 day emergency food and water supply stored in the garage. Knives and cleaning supplies are kept locked under the kitchen sink. LPA observed the 4 burner gas stove lights unassisted. LPA observed medications are kept locked in a kitchen cabinet. LPA and the Administrator toured the resident rooms. LPA observed all 5 resident rooms are clean and organized. Smoke detectors/carbon monoxide detectors tested operational. LPA observed both bathrooms are clean and operational. Hot water measured 120.0 degrees Fahrenheit in both bathrooms. LPA observed a clean supply of linen stored in the hall closet. No obstacles or hazards observed inside of the facility. LPA and the Administrator toured the backyard. There are 3 storage sheds in the backyard. All 3 sheds are kept locked and used to store extra supplies and furniture. No bodies of water observed in the backyard. There is a covered patio with chairs to sit outside. The exit gate is operational. LPA reviewed 2 staff files. No discrepancies observed. Both staff members have the required training and CPR training. Both staff members are associated to the facility. LPA reviewed 6 resident files and medications, no discrepancies observed. LPA inspected the first aid kit. The first aid kit has all the required elements. The last emergency drill was conducted on November 23, 2024. The fire extinguisher in the living room is fully charged. No deficiencies are being cited as a result of this visit. LPA consulted with the Administrator concerning reporting requirements and restricted/prohibited conditions. Type of visit: Case Management - Incident

On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz conducted an unannounced visit to the facility for the purpose to address a Special Incident Report (SIR) with occurrence date of 10/8/2023 received in Community Care Licensing Orange County Regional Office on 11/9/2023. LPA Quiroz was greeted and granted entry by Caregiver Tuyet Nguyen. LPA Quiroz called and spoke to Licensee/Administrator Mai Nguyen and discussed purpose of today's visit. (L/AD) Mai Nguyen arrived on or about 10:58am. On or about 11:03am, LPA Quiroz along with (L/AD) Mai Nguyen toured the interior and exterior of the facility premises. Documentation review of SIR dated 10/8/2023 indicates alleged mistreatment to Resident (1) from Caregiver 1 (CG1) and Caregiver 2 (CG2). L/AD Nguyen indicated "Staff no longer work at the facility. (R1) was relocated to another facility. I don't know where the caregivers are working now." LPA Quiroz provided consultation of California Code of Regulation (CCR) 87211-Reporting Requirements(a)(1)(D).(a)Each licensee shall furnish to the licensing agency such reports as the Department may require, including, but not limited to, the following:(1)A written report shall be submitted to the licensing agency and to the person responsible for the resident within seven days of the occurrence of any of the events specified in (A) through (D) below. This report shall include the resident's name, age, sex and date of admission; date and nature of event; attending physician's name, findings, and treatment, if any; and disposition of the case.(D)Any incident which threatens the welfare, safety or health of any resident, such as psychological abuse of a resident by staff or other residents, or unexplained absence of any resident. Type of visit: Case Management - Deficiencies

On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz conducted a subsequent visit after conducting a Case Management- incident inspection visit to the facility. LPA Quiroz was greeted and granted entry by Caregiver Tuyet Nguyen. LPA Quiroz called and spoke to Licensee/Administrator Mai Nguyen and discussed purpose of today's visit. (L/AD) Mai Nguyen arrived on or about 10:58am. On or about 11:03am, LPA Quiroz along with (L/AD) Mai Nguyen toured the interior and exterior of the facility premises. On or about 11:04am while inspecting kitchen area, LPA Quiroz observed three bottles of medication containers on kitchen island. Resident 1 (R1) and Resident 2 (R2) were observed to be in close proximity sitting in dining room area. The following three medications were identified to be: Oyster Shell 500-Vitamin D, Olmesartan HCTZ 12.5mg and Fucoiden Pure Extract which were identified to belong to Caregiver 1 (CG1) as evidenced by (CG1)1 saying "Sorry, sorry," and (L/AD) stating "The caregiver was just going to take them." This was verified with (L/AD) Mai Nguyen. LPA Quiroz provided consultation on California Code of Regulation (CCR) Incidental Medical and Dental Care 87465(a)(h)(2). Facility cited during today's visit under California Code of Regulation (CCR)87465(a)(h)(2)Incidental Medical and Dental Care(a) A plan for incidental medical and dental care shall be developed by each facility.
