On July 15, 2026, Licensing Program Analyst (LPA) Taylor Simerly and Licensing Program Manager (LPM) Alfonso Iniguez conducted an unannounced annual visit. LPA and LPM were greeted and granted entry by staff. LPA and LPM met with Imelda Cunningham, with a valid administrator certificate from July 23, 2025 to July 22, 2027. LPA and LPM explained the purpose of today’s visit. The facility is licensed to serve (6) elderly adults ages 60 and above, of which (6) can be non-ambulatory. The facility has an approved hospice waiver for (4). Currently the facility has (5) residents and (1) is currently on hospice. The home consists of 5 resident bedrooms, 1 staff room, living room, dining room, kitchen, two bathrooms and an attached garage. LPA Simerly, LPM Iniguez and the executive director toured the physical plant. There were no bodies of water or obstructions on the premises. LPA and LPM inspected residents’ bedrooms and bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The water temperature ranged from 107.8°F to 109 F, and a comfortable room temperature throughout the facility. The evaluation Report continues on the next page, LIC 809-C, providing further details of the inspection findings During the visit, LPA Simerly and LPM Iniguez observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies, toxins, and sharp objects were stored in a way that made them inaccessible to residents in care. The kitchen was inspected, and there were sufficient perishable and non-perishable food available, which was adequately maintained. The first aid kit contains all of the required materials. All fire extinguishers were charged and operable. The last Fire/Disaster Drills were conducted on July 5, 2026. A review of (5) residents' service files and (4) staff personnel files was conducted. LPA reviewed (5) Medication Administration Records (MARs) and found no discrepancies. LPA and LPM observed the facility's infection control practices. All mandated inspection control posters were displayed throughout the facility. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did not observe deficiencies; therefore, no citations were issued at this time. On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents. Facility has an approved hospice waiver for 4 residents and the home currently has 6 residents, with 1 resident on hospice. Administrator (AD) Imelda Cunningham arrived shortly to conduct facility tour. AD provided updated liability insurance that expires on 06/01/2026. LPA along with staff toured the facility at 1:40 PM. LPA toured the physical plant, checked food service, and facility documentation. The home consists of 5 resident bedrooms, staff room, living room, dining room, and kitchen as well as 2 bathrooms. LPA toured the kitchen and observed sharps locked in a cabinet during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. LPA observed 3 expired cans of food in the kitchen and 4 expired cans of food in the garage. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured 105.6 degrees F. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Smoke detectors tested operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility last conducted emergency drills on 01/20/2025. Outside grounds were toured. Walkways around the home were clear of hazards. First aid kit contained all required items including tweezers, scissors and thermometer. Facility conducts activities in the form of games. There is shaded outdoor seating for residents. Continued on LIC809-C dated 07/25/2025 Exit gates are unlocked and operational. LPA observed the emergency food and water supply. LPA reviewed six resident files and two staff files. All resident files contained required documentation including admission agreements, physician reports, and resident appraisals. Staff files reviewed contained required documentation including required annual training, medical assessment/ TB, criminal record clearance and proof of CPR training. LPA reviewed medication storage and administration. Medications are stored in a locked closet. Medications are being administered per physician order. Based on the observations made during today’s visit, a deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. At around 8:00 AM, LPA Tea was greeted and granted entry into the facility by house manager, Angeline Delacruz and explained the reason for the visit. The administrator (AD) Imelda Cunningham arrived shortly to assist with the visit. The facility is licensed for 6 non-ambulatory residents with a hospice waiver for four. Currently there are six residents, of which three are on hospice during today's visit. Around 8:20 AM, LPA Tea reviewed six resident files and three staff files. Resident files and staff files contained all required documentation. Upon review of records, the facility is up to date with required quarterly fire drill, which was last conducted in July 5, 2024. AD Cunningham administrator certificate is currently expired however administrator has complied with all course work and is pending renewal at this time. LPA Tea along with AD Cunningham toured the facility at 9:19 AM. LPA toured the physical plant, checked food service, and the first aid kit. The facility is a single-story home that consists of 5 resident bedrooms, 1 staff room, 2 bathrooms, living room, dining room, kitchen and attached garage. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, and shower was free of mold/mildew. Water temperature measured around 110 F degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements including bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps locked in a kitchen drawer. LPA also observed toxin substances to be locked and inaccessible to clients in care underneath the kitchen sink and a shed in the backyard. The fire extinguisher was fully charged in the kitchen area. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample seating underneath a shaded patio area in the backyard. Annual continuation on LIC809-C There are two exit gates that are self-latching and operational on both sides of the house. LPA observed PPE supplies, emergency food and water stored in the garage. Facility provides activities based on their personal choice. Residents walk around the house and do some gardening in the yard. At the time of the visit, LPA observed residents having breakfast and lounging in the living room watching television and movies. At 9:39 AM LPA reviewed medication storage and administration. Medications are stored in a locked cabinet in the kitchen. Medications are being administered per physician order. LPA interviewed residents regarding their quality of care and spoke to staff present regarding care provided. Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations.
