Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. LPA Tea was greeted and granted entry into the facility by Administrator (AD) Alper Ozdemir and explained the reason for the visit. The facility is licensed for two ambulatory residents and four non-ambulatory with a hospice waiver for two. Currently there are three residents. LPA Tea reviewed three resident files and 2 staff files. Resident files and staff files contained all required documentation. Upon review of records, the facility is up to date with required quarterly emergency disaster drills, which was last conducted on January, 5 2026. AD Ozdemir's administrator certificate has an expiration date of April, 25 2026. LPA toured the physical plant, checked food service, and the first aid kit. The facility is a one-story home, that has four resident bedrooms, two bathrooms, living room, dining area,, kitchen and garage. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, and showers were free of mold/mildew. Water temperature measured between 117. 6 to 120.5 Fahrenheit degrees. LPA suggested to AD Ozdemir to lower the hot water. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards; doorways were free of obstructions. First aid kit had all the required elements including dressing, bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps locked and (Annual continuation on LIC809-C) inaccessible to residents in a kitchen drawer. LPA also observed toxin substances secured underneath the kitchen sink and a storage shed in the backyard. The fire extinguisher in the kitchen is fully charged. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample seating with shade in the backyard. There is a barbecue grill in the backyard where residents and their family can use. There are two exit gates that are self-latching and operational on both sides of the house. LPA observed PPE supplies, emergency food and water stored in the backyard storage sheds and garage. Facility provides activities based on their personal choice. The staff likes the residents to be healthy by doing exercises and having them walk around the facility. At the time of the visit, LPA observed residents lounging in the living room watching television and one resident walking around in their walker in the facility. LPA reviewed medication storage and administration. Medications are stored in a locked kitchen cabinet. Medications are being administered per physician order. LPA interviewed residents regarding their quality of care and spoke to staff present regarding care provided. Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. On this day Licensing Program Analysts (LPAs) Fred Arias and Michael Tea made an unannounced visit to conduct a required annual visit. LPAs were greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for 6 residents of which 4 can be non-ambulatory residents and a hospice waiver for 2. Administrator (AD) Alper Ozdemir arrived shortly to conduct facility tour. AD Ozdemir has a valid certificate that expires on 4/25/2026. AD provided updated liability insurance that expires on 3/31/2025. LPAs along with Administrator Ozdemir toured the facility at 2:00 PM. LPAs toured the physical plant, checked food service, facility documentation and the first aid kit. The home consists of 4 resident bedrooms, living room, dining room, and kitchen as well as 2 restrooms. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured 110.6 degrees F in one restroom. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Auditory exit alarms were operational during today's visit. LPAs toured the kitchen and observed sharps locked in a cabinet during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Smoke detectors tested operational during today's visit. Fire extinguisher was fully charged. LPAs reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility conducts quarterly emergency drills with the last drill conducted on 10/2/2024. Outside grounds were toured. Walkways around the home were clear of hazards. There are no security bars or weapons on the premises. First aid kit contained all required items including tweezers, scissors and thermometer. Facility conducts activities in the form of exercise. There is shaded outdoor seating for residents. Exit gates are unlocked and operational. LPAs observed the emergency food and water supply. LPAs reviewed four resident files and three staff files. CONTINUED ON LIC 809C DATED 1/3/2025 All resident files contained required documentation including admission agreements, physician reports, resident appraisals, and physician orders for bed rails as indicated. Resident files were missing completed Needs and Services Plans. Staff files reviewed contained required documentation including required annual training, medical assessment/ TB, criminal record clearance and proof of CPR training. LPAs reviewed medication storage and administration. Medications are stored in a locked cabinet. Some medications are not being documented correctly in the medication administration report for two out of four residents. Based on the observations made during today’s visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced plan of correction (POC) visit to follow up on citation issued on 01/19/2024. LPA was greeted and granted entry into the facility and explained the reason for the visit. Deficiency cited under Title 22 Regulation 87608(a)(5)(B) pertaining to postural supports has NOT been cleared. Resident 1 has full rails on the bed. Licensee has not complied with the terms of the POC. CIVIL PENALTY ASSESSED. Licensee has been advised to maintain all items especially those that were previously deficient in the facility in accordance with Title 22 Regulations. Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to Alper's Care Home. The purpose of today’s visit was to conduct the Annual Required inspection. LPA was allowed entry into the home and met with Administrator Alper Ozdemir. Facility has an approved hospice waiver for 2 residents and the home currently has 6 residents. There is 1 resident on hospice during today's visit. Alper Ozdemir has an Administrator Certificate expiring on 04/25/2024. LPA Lyman along with Administrator toured the facility at 10:15 AM. LPA toured the physical plant, checked food service, and the first aid kit. Facility appears to be clean, safe, and sanitary. The home consists of three resident bedrooms, one resident bathroom, one shared hall bathroom, living room, dining room, and kitchen. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. At 10:22 AM, LPA observed Resident 1 (R1) has full bed rails and all residents have a full rail on the inside of the beds facing the wall. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 118 and 119 degrees F in all facility bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Resident hygiene supplies are locked and inaccessible to residents. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements. The entry door into the garage is alarmed. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed the locked medication cabinet. Smoke detectors and Carbon Monoxide detectors tested operational during today's visit. Fire extinguishers are fully charged. Kitchen appliances are operational during today's visit. LPA toured the outside grounds and there is ample shaded seating for residents. LPA observed emergency food and water supply in the garage. Facility has a generator on site. LPA reviewed the emergency disaster plan during the visit. Plan is thorough and complete. Facility provided documentation of last fire drill conducted on 1/03/2024. Facility provides activities in the form of cards or exercise. At 11:00 AM, LPA reviewed six resident files and three staff files. Resident files contained required documents CONTINUED ON LIC 809C DATED 01/19/2024 including admission agreements, physician reports and resident appraisals. Staff files reviewed contained required documentation of annual training, health screen/TB, and criminal record clearance. LPA observed evidence of current liability insurance. At 12:15 PM, LPA observed medication administration and storage. Facility uses a medication administration record and medications are secured. Based on the observations made during today's visit, the following violation is being cited per California Code of Regulations, Title 22, Division 6, Chapter 8.
