Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Licensee Lily Nguyen and Administrators Brenda and Kristine Truong and discussed the purpose of the visit. The facility is a one story home with five resident bedrooms, two bathrooms, living room, dining room, kitchen, and attached two car garage. LPA observed all resident rooms had the required components and furnishings. LPA observed 1 of 4 residents has a bed rail with no physicians order on file. LPA observed the bathrooms to be stocked with toilet paper, paper towels and slip mats in the showers. The water was tested between 105 and 106.6 degrees Fahrenheit. LPA observed a clean supply of linens in the hall closets for resident use. The centrally stored medication is stored in a cabinet in the corner of the living room and is locked making them inaccessible to residents in care. LPA observed the first aid kit to have all the required components. LPA observed the facility to have a two day perishable and seven day non perishable food supply on hand. LPA observed the stove to have 2 of 5 burners not ignite. The knives are stored in a drawer by the fridge that was unlocked upon LPAs inspection. The toxins and chemicals were found to be unlocked under the kitchen sink upon LPAs inspection. Staff immediately locked the knives and toxins upon LPAs discovery. LPA observed the garage to be used as extra facility storage. LPA observed the emergency food and water supply to be stored in the garage. LPA observed the backyard to be free of debris and obstructions with a shaded seating area for resident use. The carbon monoxide and smoke detectors were found to be operational. LPA observed a fire extinguisher in the facility charged and with a service date of June 24, 2026. Continue on LIC809C LPA reviewed staff files and no discrepancies were observed. LPA reviewed resident files and no further discrepancies were observed. LPA reviewed resident medications and observed 3 of 4 residents to have over the counter medications and vitamins without a physicians order on file. LPA reviewed the last disaster drill was conducted on May 14, 2026. All staff present are background cleared and associated to the facility. Based on today’s inspection deficiencies are being noted per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Michael Tea made an unannounced visit for the purpose of conducting a Plan of Corrections Inspection for deficiencies issued on 07/08/2025 during the required annual inspection conducted by LPA Tea. LPA was greeted and granted entry by caregiver staff. The Administrator (AD) Kristine Truong arrived shortly after to assist during the visit. LPA discussed the purpose of the inspection with the administrator. LPA Tea toured the facility to check the deficiencies that have been corrected with the administrator. The broken dresser in one of the resident’s rooms was replaced with a new one. There is a new toilet paper holder affixed on the bathroom wall. LPA checked the garage and observed the facility with enough emergency water supply for residents in care. LPA verified completed physician's reports for residents. Based on LPA Tea's observation on today's visit, the Plan of Corrections has been fulfilled by the assigned POC due date of 07/29/2025, thus clearing the Type B deficiencies CCR 87458(c)(1), 87303(a) and HSC 1569.695. Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. LPA Tea was greeted and granted entry into the facility by a caregiver staff and explained the reason for the visit. Administrators (AD) Kristine Truong and Brenda Truong arrived shortly to assist with the visit. The facility is licensed for a capacity of six, approved for six non-ambulatory, with a hospice waiver of four. Currently there are six residents, and there are two on hospice during today's visit. LPA Tea reviewed six resident files and four staff files. Resident files contained most of the required documents, except four of the residents does not have updated physician’s report. Staff records met all requirements. The administrator certificate expires on January 17, 2026. LPA Tea along with staff toured the facility. LPA toured the physical plant, checked food service, and the first aid kit. The home is a one-story facility that consists of 5 resident bedrooms, 2 full bathrooms, living room, kitchen, dining area, and attached garage. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, and shower was free of mold/mildew. Water temperature measured between 108.1 Fahrenheit degrees and 115.5 Fahrenheit degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements Annual Inspection continued LIC809C including dressing, bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps secured in a kitchen drawer. LPA also observed toxin substances to be secured and locked and inaccessible to clients underneath the kitchen sink and garage. The fire extinguishers throughout the facility are fully charged. The facility’s last fire drill was conducted on January 27, 2025. Kitchen appliances are operational during today's visit. LPA observed emergency food and water supply in the kitchen and garage. There was not enough water for residents in case of emergency. LPA toured the outside grounds. There is a shaded patio area for residents to sit outside. The gates on both sides of the house are self-latching and operational. Facility provides activities based on resident’s personal preference and health condition and limitations. There are activities like coloring and reading provided for residents. There are group discussions, and an activities person comes to the facility from time to time. At the time of the visit, LPA observed residents watching TV and enjoying lunch. LPA reviewed medication storage and administration. Medications are stored in a locked cabinet in the kitchen. Medications are being administered per physician orders. LPA advises facility needs to list PRNs on their centrally stored medications list. LPA interviewed clients regarding their quality of care and spoke to the staff present regarding care provided. Based on the observations made during today’s visit, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Michael Tea made an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA Tea met with designated Administrators (ADs) Brenda Truong and Kristine Truong. An application to operate a Residential Care Facility for Elderly (RCFE) was received by our agency on January 31, 2024 for a total capacity of six; six non ambulatory and four maybe on hospice. Fire clearance was approved by a fire inspector from the Orange County Fire Authority on March 14, 2024. The facility is a one story home with five resident bedrooms, one of the five is a shared room, two bathrooms, a living room, a kitchen, a dining area, and an attached two car garage. All exiting doors had alarm notifications. There are two backyard exit gates on each side of the house that is self-latching and unlocked. There is a shaded seating area and LPA did not observe any obstacles or hazards in the backyard. There is a lovely garden area with plants and fruit trees. LPA observed both Administrator certificates, See Something, Say Something poster (PUB 475), Personal Rights, Ombudsman poster, Facility sketch and Emergency Disaster Plan in the facility mounted on the wall around the facility entryway. Fire extinguisher is located in the front entryway fully charged with an inspection date of June 25, 2024. Resident bedrooms had the required furnishings. LPA observed all beds had linens and blankets. All toxic chemicals, cleaning solutions, and disinfectants are inaccessible to residents and will be stored and locked underneath the kitchen sink. Medications will be stored in a locked cabinet in the living room. The first aid kit is also stored in the locked cabinets in the living room and has all the required elements. Staff and resident records will also be stored and locked in the cabinets in the living room. Reading materials and games were observed in the living room with a phone and computer station for residents can use. Continuation of Pre-licensing on LIC 809-C A supply of seven-day of perishable and non-perishable food was observed and will be maintained on hand. Smoke detectors and carbon monoxide detectors tested operational. Gas burner stove, dishwasher, refrigerator, microwave, washer, and dryer are operational. There are cameras placed in the common areas of the facility without audio for the safety of residents in care. Hot water temperatures in the bathrooms measured between 116.7 and 112.6 degrees F. Emergency food and water and additional supplies were observed to be well stocked in the garage. The facility is ready to be licensed. LPA conducted the Component Three Orientation with ADs. ADs was notified that the final application approval will be issued by the Centralized Applications Bureau in Sacramento.
