On July 13, 2026, Licensing Program Analyst (LPA) Eboni Bentley arrived unannounced for the purposes of conducting a required 1-Year annual visit using the CARE Inspection Tool. LPA Bentley was greeted and granted entry into the facility by staff, after stating the reason for the visit. Administrator (AD) Brian Canseco arrived shortly to assist with the visit. The facility is licensed to operate age 60 and over for (6) non-ambulatory only residents of which one (1) may be bedridden in Bedroom 1 only, with a Hospice waiver for six (6). The building is a single story structure located in a residential neighborhood, which consists of the following: five (5) resident bedrooms, two (2) bathrooms, living area, dining area, kitchen, an outdoor covered seating area, and an attached two car garage. The facility currently has a census of five (5) residents in care with all are receiving home health services. LPA Bentley toured the inside and outside of the physical plant with AD Canseco. All rooms were inspected and the facility was observed to be appropriately furnished at the time of visit. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Additional linens, comforters, and bath towels were adequately stocked and available. Bathrooms were found to be clean and operational with water temperatures measured between 109 degrees F to 109.6 degrees F. The kitchen was observed clean, all appliances were operational, and there was a two-day supply of perishable and seven-day supply of non-perishable food available. Toxins, disinfectants, sharps, and medications were secured. The backyard was observed with a shaded seating area for residents and the facility has one exit gate that was operational. Evaluation Report Continues on LIC 809-C..... The smoke alarms and carbon monoxide detectors were operable. An emergency safety drill was last conducted on June 1, 2026 and are conducted quarterly. Emergency food, emergency water, and emergency supplies were stored in the garage. The facility has one (1) fire extinguisher that was charged, mounted, and last serviced on June 24, 2026. First aid kit is maintained and contains all the necessary elements. A working telephone (657-227-3338) remains available and there is a device that can be used by residents for teleconference purposes. Liability Insurance is effective August 20, 2025 and expires on August 20, 2026. LPA Bentley conducted an audit of five (5) resident files (R1-R5), four (4) staff files (S1-S4), and medication and medication administration records review. Interviews were conducted with staff and residents. Based on today’s observations, no deficiencies are being cited during the visit, per Title 22, Division 6, Chapter 8 of the California Code of Regulations. Licensing Program Analyst (LPA) Michael Tea made an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA Tea met with designated Licensee (LE) Tin Le. An application to operate a Residential Care Facility for the Elderly (RCFE) was received by our agency on December 27, 2024, for a total capacity of five non-ambulatory residents and one bedridden. The fire clearance was approved by a fire inspector from the Orange County Fire Authority on March 5, 2025. The facility is a one-story home with five bedrooms, two bathrooms, a living room, a kitchen, a dining area, and an attached two car garage. There is a one exit gate on the left side of the house that is self-latching and unlocked. There is a shaded seating area for clients in the backyard and LPA did not observe any obstacles or hazards. There are activities such as games and puzzles for residents to play and participate in. LPA also observed reading materials. LPA observed Administrator certificate, Facility sketch and Emergency Disaster Plan in the facility mounted on the wall in the living room area. The See Something, Say Something poster (PUB 475), Long Term Care Ombudsman poster, and Personal Rights were posted up on the wall in the living room. Resident bedrooms had the required furnishings. Bedrooms with sliding doors had working door alarms. LPA observed all beds had linens and blankets. Smoke detectors and carbon monoxide detectors tested operational. Hot water temperatures in the bathrooms measured around 112.8 to 113.7 Fahrenheit degrees. Toxins were secured and locked in a storage closet by the entry way. Medications will be stored in a locked cabinet in the living room alongside with facility record files. The first aid kit with all the required elements and first aid guidebook was observed in the living room. Records will also be stored alongside medication in the locked cabinet. A phone is in the kitchen for clients to use. Pre-licensing Report continuation on LIC809-C A supply of perishable and non-perishable food was observed. Electric stove burner, dishwasher, refrigerator, microwave, washer, and dryer are operational. Emergency food and water and additional supplies were observed to be well stocked in the kitchen pantry and garage. There is a laptop in the kitchen for the residents to use. The fire extinguisher is in the kitchen and is fully charged. The facility is ready to be licensed. LPA conducted the Component Three Orientation with Licensee Tin Le. Licensee was notified that the final application approval will be issued by the Centralized Applications Bureau in Sacramento. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
