On 03/5/2026 at 02:15 PM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. LPA introduced herself to Flor Famisan- Caregiver who allowed LPA entry into the facility. The facility is licensed to serve 6 non-ambulatory clients ages 60 and over. One (1) may be bedridden in room 2 only. The facility has a hospice waiver approved for 3. The facility has four (4) clients and two (2) clients are receiving hospice care. The facility is a single-story home in a residential area which consists of four (4) bedrooms, two (2) bathrooms, living room, dining room, kitchen, washer and dryer/ storage area, backyard with table and chairs. At 3:00 PM, LPA reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings which were current. LPA also reviewed four (4) clients’ files for admission agreements, updated physician reports, needs and services plans all of which were current. At 3:45 PM, LPA and Rosemarie toured the physical plant inside and outside of the facility and there were no obstructions or bodies of water on the premises. LPA inspected a total of four (4) bedrooms and two (2) bathrooms. LPA observed the bedrooms to have the required furniture which appeared to be in good repair. The rooms also have adequate lighting, and storage for the clients’ personal belongings. The bathrooms also were observed to be operational with handrails and shipmates. Continued LPA toured the kitchen and dining area and there was sufficient seating for dining. LPA observed a 5-day supply of perishables and a 7-day supply of non-perishable food available for the clients in care which were adequately maintained. The smoke/carbon monoxide detectors were in operable condition and tested. The fire extinguishers were fully charged. The water temperature ranged from 105.5°F to 120°F. LPA observed cleaning supplies, toxins, and sharp objects stored inaccessible to residents in care. There were also additional cleaning supplies, and linens for clients in care. Based on the observations made during today’s visit, no deficiencies were cited. On 05/01/2025 at 08:00 AM, Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced annual inspection visit at the Luxury Assisted Living LLC Facility. LPA Calderon was allowed entry into the facility by Administrator Dmitry Malyshev. Facility is licensed to serve 6 non-ambulatory residents aged 60 and above. One may be bedridden in room 2 only. The facility has a hospice waiver approved for 3. Currently, the facility has ZERO (0) residents and ZERO (0) in hospice care. LPA Calderon explained to Administrator Dmitry Malyshev, the purpose of the one-year Annual Inspection visit, and escorted LPA Calderon on a tour of the entire inside and outside facility grounds. As part of the inspection, LPA Calderon reviewed: ZERO (0) client service records, ZERO (0) client medication records, ZERO (0) staff records, and inspected the inside facility and outside grounds. The facilities’ last fire drill was conducted on Not needed at this time. The one-story residential home consists of four (4) client bedrooms, two (2) client bathrooms, living room, dining room, kitchen, staff room, office area, attached garage with washer and dryer/ storage area, backyard with table and chairs. No weapons are stored in the premises. Kitchen was inspected and observed to be clean and operational. A two-day supply perishable and seven-day supply of non-perishable foods are present in the facility. Emergency Water Storage is in the garage and kitchen area.On 05/01/2025 at 08:00 AM, Licensing Program Analyst (LPA) Jose Calderon conducted an unannounced annual inspection visit at the Luxury Assisted Living LLC Facility. LPA Calderon was allowed entry into the facility by Administrator Dmitry Malyshev. Facility is licensed to serve 6 non-ambulatory residents aged 60 and above. One may be bedridden in room 2 only. The facility has a hospice waiver approved for 3. Currently, the facility has ZERO (0) residents and ZERO (0) in hospice care. LPA Calderon observed that all facility rooms are clean and in good repair. A comfortable temperature was observed, and the facility has central air and heating. LPA Calderon observed the following during inspection of client’s rooms: mattresses are in good condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. All bedrooms contain furniture, lighting fixtures and personal storage space as required, all beds have the required amount of linen and mattress covers, LPA Calderon observed fully stocked closet with bedding, towels, and toiletries supplies. Bathroom fixtures are clean, in good repair, and working properly and contain the required nonskid mats and grab bars. LPA Calderon observed bathrooms were found to be within Title 22 regulation. Bathroom #1 hot water temperature properly measured at 108 degrees Fahrenheit, and bathroom #2 hot water temperature properly measured at 109 degrees Fahrenheit. Kitchen hot water temperature properly measured at 113 degrees Fahrenheit. Facility one (1) Carbon Monoxide and six (6) Smoke Detectors hard wired and connected were tested and are working properly. The facility two (2) Fire Extinguishers was checked and found to be fully charged and accessible. All exit doors in the facility have alarm systems. All toxins and knifes are locked/secured and inaccessible to clients. No Medications are centrally stored and in a locked storage cabinet. Facility first aid kit is fully stocked with manual was checked and in order. A working landline phone was operational. Outside grounds were toured and no bodies of water were observed. All Exits/ Walkways around the home were free of debris and hazards. Outside patio accessible to clients. ZERO (0) client files were reviewed and found to be complete. LPA Calderon reviewed ZERO (0) resident medications, and they were all found to be administered according to doctor's orders. ZERO (0) staff files were checked and have the required documents. LPA Calderon noted the Administrator Dmitry Malyshev Certification # 6065287740 expiration date of 01/23/2025 was NOT valid at time of visit. The facility does NOT handle client's money/cash resources, and a NO Surety bond is needed. Commercial General Liability Policy #00000000 policy period from 00/00/0000 to 00/00/0000 underwritten by James River Insurance Company, coverage 1,000,000/3,000,000 is NOT valid at time of inspection. Administrator Dmitry Malyshev to email LPA Calderon a full copy of the commercial insurance policy including all endorsements no later than 00/00/0000. All the required documents are posted in the facility in a clearly visible area. LPA Calderon advised the Administrator Dmitry Malyshev to continuously monitor the Centers for Disease Control (CDC) website and Community Care Licensing Provider Informational Notices (PIN) for any updates relating to COVID-19 guidance. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA Calderon did not observe any deficiencies therefore no citations were issued at this time. Annual Licensing Fee is CURRENT. On 03/06/2024 at 9:45 AM, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced required – Annual Inspection and met with Administrator Dmitry Malyshev. Annual Facility Fees are current. Facility is licensed to serve 6 non-ambulatory residents age 60 and above. One may be bedridden in room 2 only. The facility has a hospice waiver approved for 3. Facility is a single story 4-bedroom, 2 common bathroom, 1 common area, a kitchen, a 2 car de-attached garage used for storage, laundry area, linen closet and a large outdoor shaded patio area. Administrator accompanied LPA inside and outside the facility during this inspection. Outside grounds were toured and no bodies of water were observed. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure, shower was free of mold/mildew and a non-skid mat was in place, and warning signs are prominently identified in the kitchen and in the bathroom. The second shower is under currently under construction. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Knives and toxics were kept in locked storage cabinet. First Aid kit was available. Licensee tested all carbon monoxide detectors and smoke detectors located in the facility. Both devices were functional and interconnected. Deficiencies are being cited based on LPA observation and interviews conducted in accordance with the California Code of Regulations, Title 22, see LIC809D. A violation regarding 87204 Limitations - Capacity and Ambulatory Status. The Administrator rented three of the bedrooms out to private individuals. There are no residents in care. An exit interview was conducted, plans of corrections were developed and reviewed, and technical assistance provided.
