On 12/30/25 LPA Yi Sam Jian made an unannounced annual visit to the facility. LPA met with staff Joseph Sunga, and Administrator, Irene Mehta arrived later during the visit. LPA explained the purpose of the visit. Facility is a 4 bedrooms one level house and 2 bathrooms. LPA toured the facility inside and outside including all of resident rooms, common areas & kitchen. Residents are currently resting in the bedrooms and living room. Backyard was fenced, secured, and in good condition. All outdoor and indoor passageway were free and clear of obstruction. No accessible bodies of water or fire safety hazards observed. Kitchen was inspected, sufficient supply of food observed. Medications, toxins and sharps stored appropriately and inaccessible to clients, a comfortable temperature was maintained, hot water temperature inspected to be compliant, furnishing and lighting was sufficient for comfort and safety. Carbon monoxide detector and smoke detector system inspected and met the requirements. fire extinguisher checked and fully charged. Facility has a written emergency disaster plan. Licensee has at least one completed first aid kit located in the living room. Criminal record clearances or exemptions for facility staff or other individuals who have client contact have been reviewed. No deficiencies are cited at this time. Allegation investigated: Staff did not issue an appropriate refund to resident or resident's authorized representative

LPA Jeung met with administrator to discuss refund of pro-rated monthly fee for client #1, who moved out of facility on 4/8/25. LPA also reviewed client file and obtained copies of relevant documents, including proof of delivery of certified mail to son of client #1 on 9/20/25. Receipt of check has been acknowledged by client's daughter. However, the payee is client. Son/DPOA claims that it cannot be cashed and mailed the check back to facility. Administrator denies receiving the check. Based on records reviewed and information from administrator, this allegation is determined to be unsubstantiated. Although the allegation may have occurred or is valid, there is not enough evidence to prove the alleged violation did or did not occur. Allegation investigated: Residents are not accorded dignity and respect Staff used restraint for resident

On 9/10/2025, LPA Grace Donato conducted an unannounced complaint investigation visit to deliver findings. LPA met with Administrator, Irene Mehta and explained the purpose of this visit. Regarding the allegation of Residents are not accorded dignity and respect and staff used restraint for resident, Responsible party (RP) stated that ADM informed a witness (W1) that the staff used mitten restraints while changing and feeding the resident (R1). W1 told ADM that restraints were not allowed. During the investigation, the ADM mentioned using mittens whenever they change R1s diapers because R1 is very combative. The mittens was provided by the family members. While there was a consent provided by the family members, there was no Doctors order when LPA requested it from the facility. Based on interview, the above allegations are determined to be SUBSTANTIATED. Deficiencies of the California Code of Regulations, Title, 22 cited on the LIC9099-D. Failure to correct the deficiencies may result in civil penalties. Report is reviewed and copy of the report and appeals rights are provided. Substantiated According to the interview, ADM provided the information requested during this time. The EMTs didn’t want to talk to the family, but kept asking about the DPOA. The ADM of whom was on the phone with the family during this time with a three way with the case worker (CW) during the incident. The family was asking why the R2 needed to go to the hospital. LPA tried to reach DPOA but has not gotten any reply. CW confirmed that they were in a phone call about the resident being sent to the hospital. Based on records review, LPA obtained a police report, and it states that the Fire Captain were waiting for pertinent patient information that ADM was refusing to provide and acting aggressive. Police asked ADM to retrieve the paperwork but ignored the request. ADM was uncooperative, and verbally aggressive, providing several reasons why she refused to provide the necessary information. She eventually obtained and provided the information to the Fire Captain, but only after requesting the information from her numerous times. LPA has also obtained message exchanges between ADM and Social worker, working on managing R2s behaviors. Regarding the allegation of residents are not allowed privacy, RP states that staff said residents doors must remain open. Based on observations, LPA observed that there are rooms where the doors remain open. One resident (R3) had the door closed and just rings the bell if he/she needs anything. At the same time the staff does rounds and checks on them from time to time. LPA attempted to interview 3 residents but didn't want to be interviewed. Based on interviews, records review and observations, the department has determined that although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED. On 1/09/25 LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Administrator Irene Mehta. LPA explained the purpose of the visit. Facility is a 4 bedroom house with 2 shared rooms. LPA toured the facility inside and outside including all of resident rooms, common areas & kitchen. The indoor and outdoor passageways were free of obstruction. Residents are currently resting in the bedrooms and living room. The residents have adequate amount of linens and all personal belongings are intact. While touring the facility it was observed that the room temperature was at 70 deg F. Hot water was also tested in the bathrooms and the temperature was 108 deg F. Carbon monoxide monitor is working properly. All fire extinguishers have been checked and current. Client bathrooms were observed to be in good repair equipped with grab bars and non-skid floors. LPA checked the food supply and there is adequate amount of food, 2 days for perishables and & 7 days non-perishable. Emergency drills are done quarterly. Four resident records and four staff records were reviewed. Everything is complete and updated. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. Three client records and three staff records were reviewed. Resident records are updated, complete and signed. LPA requested the Liability Insurance and LIC500 to be emailed. No deficiencies are cited at this time. Allegation investigated: Facility retained a resident with prohibited health condition who is not under hospice.

On 7/23/24, Licensing Program Analyst (LPA) Grace Donato conducted an unannounced complaint investigation visit. LPA met with Administrator Irene Mehta and explained the purpose of the visit. Regarding the allegation of facility retained a resident with prohibited health condition who is not under hospice. Reporting party (RP) stated that resident (R1) had stage 4 pressure injury when sent to the hospital. Resident was not in hospice. Based on records review, there is only one resident in the facility who has pressure injury. R1 was last checked on January 2024 and had a diagnosis of a small pressure sore. Nursing care was recommended. Home Health visited and trained staff to care for the wound. Wound was being maintained with care based on the progress notes provided. page 1 of 2 Unfounded LPA interviewed the Administrator (ADM) and it was mentioned that the staff was trained to care for the wound. Wound was healing and then comes back again despite facility staff caring for it and constant repositioning of R1. When R1 was sent to the hospital for abdominal pain last May 6, 2024, the wound was at stage 2. When R1 was released from the hospital, R1 went on hospice. LPA was also able to interview the resident (R1) and the resident stated that he/she is happy in the facility and that the staff takes good care of him/her and wants to stay here in the facility. Based on interviews and records review, the department has determined that that the allegations were false, could not have happened and/or is without a reasonable basis, therefore the allegations are UNFOUNDED. On 1/23/24 LPA Grace Donato made an unannounced pre-licensing visit to the facility. LPA met with Administrator Irene Mehta. LPA explained the purpose of the visit. LPA toured the facility inside and outside including all of resident rooms, common areas & kitchen. The indoor and outdoor passageways were free of obstruction. The residents have adequate amount of linens and all personal belongings are intact. While touring the facility it was observed that the room temperature was at 69 deg F. Hot water was also tested in the bathrooms and the temperature was 110 deg F. Carbon monoxide monitor is working properly. All fire extinguishers have been checked and current. Client bathrooms were observed to be in good repair equipped with grab bars and non-skid mats. LPA checked the food supply and there is adequate amount of food, 2 days for perishables and & 7 days non-perishable. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. Three client records and three staff records were reviewed. Resident records are updated, complete and signed. Facility is clean and in good repair based on observations made today. Facility is in compliance with Title 22 regulations. Component III is also conducted on this day. No citations are issued. No deficiencies are cited at this time.
