On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a required annual. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. The facility is licensed for 6 residents, 5 of which can be non-ambulatory and 1 can be bedridden. The facility has a hospice waiver for 2 residents. This is a single story home with an attached garage. The facility has 5 bedrooms and 3.5 bathrooms. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors and testing hot water temperature in the bathrooms. The hot water temperature measured 99.8 and 102.5 degrees Fahrenheit and all smoke detectors were operational. The facility conducted an emergency drill on 4/10/2025. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed secured medication storage in kitchen. LPA observed emergency food was stored in laundry room which is attached to the kitchen. LPA reviewed one out of one staff training and fingerprint records and conducted a complete review of 5 out of 5 residents files. Administrator has a current administrator certificate which expires on 11/30/2025. Based on observations the following is being cited per Title 22. Type of visit: Case Management - Other

On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a case management visit. LPA was greeted and granted entry into the facility and explained the reason for the visit. Administrator Oana Abrudan arrived at 8:45 am. Per review of approved facility sketch dated 02/01/2016 there was no indication there was a designated "Rec Room" and it was listed as Resident Room #1 at 11 X11 for measurements. Based on the current submitted facility sketch dated 10/28/2024 Resident Room #1 is 12 X11 with an interior wall connected to the "Rec Room". Per previous floor plan neither Rec Room or Resident Room 1 had a window or door labeled. Per review of facility sketches Resident Room #3 was previously designated as Caregiver Room, facility Administrator Oana will confirm if room was designated as non- ambulatory at initial fire clearance approval. Based on observations made during today's visit deficiencies are being cited. Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Staff #1 at 9am. During today’s visit, LPA met with Oana Abrudan, Administrator. The facility is a single story, six bedroom building with an approved fire clearance of six non-ambulatory residents of which one may be bedridden. The facility has a hospice waiver for two and currently has a census of four residents in care with one on hospice. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in three of three resident bathrooms, and testing auditory devices on all exits. The hot water temperature measured between 110.3 and 111.2 degrees Fahrenheit and all smoke detectors were operational. The fire extinguisher is charged and was serviced on June 3, 2024. The facility’s last fire drill was conducted on June 10, 2024. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed medication storage and reviewed the centrally stored medications. Per review medications are being given as prescribed. LPA reviewed three of three staff training and fingerprint records and conducted a complete review of resident records. LPA interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPA confirmed that administrator has a current administrator certificate which expires on January 7, 2025. Two former front hall closets were converted into another caregiver room. Both rooms were not noted on the facility sketch that was used for fire clearance. An immediate civil penalty was assessed for $500. The following deficiency is being cited per Title 22 Division 6 of the California Code of Regulations.
