Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to the facility to conduct an Annual Required inspection. LPA was greeted and granted entry by staff and explained the reason for the visit. The facility is a one story building with an approved fire clearance for six non-ambulatory residents of which six may be on hospice. The facility currently has a census of six residents with two residents on hospice. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature. The hot water temperature measured between 110 and 116.7 degrees Fahrenheit and all smoke detectors were operational. The facility’s last fire drill was conducted on March 4th 2026. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed secured medication storage in kitchen drawer. LPA observed unsecured medications in kitchen cabinet to the right of the sink in a Kaiser paperbag. LPA observed four Bic lighters in the drawer on the right of the stove. LPA observed middle burner of the 5 burner stove is non operational. LPA reviewed two out of two staff training and fingerprint records. LPA conducted a complete review of resident records. LPA confirmed that administrator has a current administrator certificate which expires on November 18th 2026. Based on observations made deficiencies are being cited per Title 22. Allegation investigated: Staff handled resident in a rough manner Staff spoke to resident in an inappropriate manner. Staff denied resident water. Staff did not respond to resident's call button.

On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to deliver complaint findings. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. The Department received a complaint on 11/15/2023 and the initial 10-day visit was conducted on 11/20/2023. During the initial visit LPA Mendivil interviewed staff and residents. LPA Mendivil obtained copies of pertinent documents such as resident's physician report, emergency contact information and staff training records. Regarding the allegations Staff handled resident in a rough manner, Staff spoke to resident in an inappropriate manner, Staff denied resident water and Staff did not respond to resident's call button, the investigation revealed the following: Unsubstantiated Based on interviews with 3 out of 3 staff they deny the allegation that they handled residents in a rough manner. Per interview with 1 out of 3 residents, resident denied staff handled them in a rough manner. LPA Mendivil was unable to interview 2 out of 3 residents due to 2 resident being unavailable. Based on interviews with 3 out of 3 staff deny the allegation that staff spoke to resident in an inappropriate manner, interviews with staff indicate they have to speak loudly with residents as 2 are hard of hearing. 3 out of 3 staff deny the allegation that they have denied water. Per LPA Mendivil’s observation LPA Mendivil observed 3 out of 3 residents had their own water cups and they were filled up on 11/20/2023. Per interviews with 3 out of 3 staff stated they answer all calls even at nighttime as the staff sleeps in a room near all residents. Based on interviews with 1 out of 3 residents stated staff answers their call buttons within 2 minutes. Due to the lack of corroboration of the allegations by staff and residents the allegations Staff handled resident in a rough manner, Staff spoke to resident in an inappropriate manner, Staff denied resident water and Staff did not respond to resident's call button are determined to be UNSUBSTANTIATED, meaning that although the allegation may have happened or are valid there is not a preponderance of evidence to prove the alleged violations did or did not occur. No deficiencies cited. The facility is a one story building with an approved fire clearance for six non-ambulatory residents of which six may be on hospice. The facility currently has a census of four residents. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature. The hot water temperature measured between 105 and 109 degrees Fahrenheit and all smoke detectors were operational. The facility’s last fire drill was conducted on April 7th, 2025. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed medication storage and reviewed the centrally stored medications. Per review completed medication appear to be being given as prescribed. LPA observed a recliner chair placed on the bottom half of Resident 1 (R1) bed. LPA observed food item that has a label of "Keep Refrigerated" in a kitchen cabinet. LPA reviewed two out of two staff training and fingerprint records. LPA conducted a complete review of resident records. LPA confirmed that administrator has a current administrator certificate which expires on November 18th 2026. Based on observations made deficiencies are being cited per Title 22. Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility today to conduct an Annual Required Evaluation. LPA was greeted and granted entry by Maria Janet Manlutac, Caregiver. During today’s visit, LPA met with Lise Brick, Licensee (LE) and Rose Manabat Palma, Administrator (AD). The facility is a single story building with an approved fire clearance of six non-ambulatory residents of which six may be on hospice. The facility currently has a census of three residents in care with one resident on hospice. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperatures in three of three resident bathrooms, and testing auditory devices on all exits. The hot water temperature measured between 106.1 and 109.0 degrees Fahrenheit and all smoke detectors were operational. Fire extinguisher was charged and serviced on February 8, 2024. The PUB 475 poster was displayed and the correct size. The facility does not have a fire drill on file for 2024. AD Palma will conduct a fire drill the first week of June. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed medication storage and reviewed the centrally stored medications. Per review completed medications appear to be being given as prescribed. LPA reviewed three of four staff training and fingerprint records and conducted a complete review of resident records. LPA interviewed alert residents regarding their quality of care and spoke to staff present regarding care provided. LPA confirmed that both administrators have current administrators' certificates. LE and AD rotate staff from other nearby facilities and will associate all staff to all three of her homes. All staff were fingerprinted and cleared. The following deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
