On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. Facility is licensed for six non-ambulatory residents. Facility has an approved hospice waiver for four residents and the home currently has six residents, with three residents on hospice. Administrator (AD) Imelda Caro arrived shortly to assist with facility inspection. AD provided updated liability insurance that expires on 10/15/2026. LPA along with staff toured the facility at 9:00 AM. LPA toured the physical plant, checked food service, and facility documentation. The home consists of six resident bedrooms, staff room, living room, dining room, and kitchen, three bathrooms and an attached garage. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 105.2 degrees F and 110.6 degrees F in all bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. Auditory exit alarms were operational during today's visit. LPA toured the kitchen and observed sharps locked in a cabinet during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Smoke detectors tested operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control and emergency disaster plans and plans are complete and thorough. Facility conducts quarterly emergency drills with the last drill conducted on 09/01/2025. Outside grounds were toured. Walkways around the home were clear of hazards. There is shaded outdoor seating for residents. Exit gate is unlocked and operational. LPA observed the emergency food and water supply. First aid kit contained all required items including tweezers, scissors and thermometer. Facility conducts activities in the form of games. LPA reviewed six resident files and three staff files. All resident files contained required documentation including admission agreements, physician reports, and resident appraisals. Staff files reviewed contained required documentation including required annual training, medical assessment/ TB, criminal record clearance and proof of CPR training. LPA reviewed medication storage and administration. Medications are stored in a locked closet. Medications are being administered per physician order. Based on the observations made during today’s visit, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Allegation investigated: Facility is understaffed

On this day Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to deliver complaint findings. LPA Mendivil was greeted and granted entry into the faciltiy and explained the reason for the visit. Administrator/Licensee Imelda Caro was available via telephone. The Department received a complaint on 04/05/2021 and LPA Tirre conducted the initial 10 day visit on 04/09/2021. During LPA Mendivil's visit LPA Mendivil interviewed staff and residents. Regarding the allegation that facility is understaffed, the investigation revealed the following: It was alleged the facilty was understaffed. Per interviews with Administrator/Licensee Imelda Caro in 2021 around the time the complaint was filed she had 4 caregivers staffed at the house. Imelda stated she cannot remember the exact number of residents but would have been between 5-6 residents. Imelda stated that they were able to meet all of the residents needs in 2021. CONT on LIC 9099-C dated 08/12/2025 Unsubstantiated Imelda stated they rotate shifts so everyone should have 2 days off, but can work overtime if needed. Imelda stated currently they have a staff of 3 caregivers and they are rotated and given 2 days off each. The facility has 2 live in caregivers that are available as needed overnight. Based on interviews with 2 out of 3 caregivers stated they are able to meet the residents needs with 2 caregivers. The final caregiver was not available for interviews. LPA Mendivil has observed 1-2 caregivers in addition to Imelda on multiple visits to the facility on 08/02/2022, 10/14/2022 and 12/16/2024. Based on interviews with 3 out of 5 residents stated the staff meet their needs. One resident was not oriented to time and space and could not answer LPA Mendivil's questions and the final resident was asleep at the time of the visit. Therefore based on the preponderance of evidence through observations and interviews the allegation that facility is understaffed is determined to be UNSUBSTANTIATED meaning that although the allegation may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violation occurred. This agency has investigated this complaint. No deficiencies cited. On this day Licenscing Program Analyst (LPA) Andrea Mendivil made an unannouced visit to conduct a required annual visit. LPA Mendivil was greeted and granted entry into the facility by Caregiver Leo M and explained the reason for the visit. Administrator Imelda Caro arrived at 8:48am. At 8:40 am LPA Mendivil toured the facility with Caregiver Leo. The facility is 6 bedrooms, 3 bathroom, one story home with an attached garage. LPA Mendivil inspected the residents’ bedrooms. LPA Mendivil observed that the bedrooms were clean, lights were operational, and furniture were in good conditions. LPA Mendivil observed that the bathrooms; toilets and sinks were clean. LPA Mendivil tested the hot water in three (3) bathrooms. The hot water temperature was recorded at: 105.5 Fahrenheit degrees,106.5 Fahrenheit degrees, and 111.0 Fahrenheit degrees. LPA Mendivil inspected the kitchen. LPA Mendivil observed that the facility had a two-day supply of perishable food and a supply of seven-day of non-perishable foods as required. LPA Mendivil tested the facility smoke detectors and carbon monoxide detectors the detectors were operational. LPA Mendivil observed that the administrator has a current administrator certificate. LPA Mendivil observed the poster PUB 475 Let Us No and LTCO (Long Term Care Ombudsman) in hallway, visible to the public. LPA Mendivil observed that toxic chemicals were kept in a locked cabinet and the chemicals were not accessible to residents. LPA Mendivil inspected the garage and observed that emergency food and water.. LPA Mendivil observed secured and locked medication cabinet. LPA Mendivil observed that the resident’s medications were labeled. LPA Mendivil reviewed residents’ records and files. Therefore, and based on the observations made during this annual inspection conducted per Title 22 of the California Code of Regulations, no citations were issued.
