On 12/22/2025 at 9:15AM, Licensing Program Analysts (LPAs) Andrew Christy and David Doidge arrived unannounced to conduct the 1-Year Annual Required inspection. LPA met with Administrator, Cindy Chen, and explained the purpose of the visit. The facility is currently at max capacity with eight (8) residents. LPAs toured facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All outdoor and indoor passageways are kept free of obstruction. No bodies of water were observed. A comfortable indoor temperature is maintained at 70.0 degrees Fahrenheit. The hot water temperature in the residents’ shared bathroom was measured at 131.4 degrees Fahrenheit. LPAs observed lighting in all rooms are adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of non-perishable and 2 day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 01/07/2025. At 10:15AM, LPAs reviewed five (5) resident files and five (5) staff files, all found to be complete. The emergency disaster plan was last reviewed 09/02/2025. Quarterly emergency drills were last conducted 11/117/2025. A review of resident medications and the Medication Administration Record (MAR) found no outstanding errors. Continued on LIC809C..... Continued from LIC809..... On 11/26/2024 around 09:00 AM, Licensing Program Analyst (LPA) L. Holmes arrived unannounced for a required annual inspection. LPA met with Benita De Guzman, Caregiver (S1) and explained the purpose of the visit. The facility’s fire clearance was approved for eight (8) non-ambulatory residents; four (4) may be Hospice approved. Upon arrival, LPA observed one (1) staff attending to the residents and serving breakfast, and several residents visiting in the common area watching television. LPA toured the facility with S1. The areas included but were not limited to the common areas, dining room, bathrooms, kitchen, laundry room, office and backyard. The front yard and passageways were free of obstruction. A comfortable temperature was maintained at 72 degrees Fahrenheit (F). LPA observed lighting in all areas to be adequate for the comfort and safety of the residents. The hot water temperature in the shared restroom was measured at 113.6 degrees (F). The shared restroom had paper towels, soap and garbage cans; all areas were safe and sanitary. PPE, sanitizer, and paper goods remain sufficient. There was a 2-day supply of perishable foods and a 7-day supply of non-perishable foods. ...continued on LIC9099C. ...continued from LIC9099. Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was observed full and last serviced 12/29/23. Emergency Disaster Plan is updated; safety drills and training are rotational. LPA reviewed four (4) staff files, and five (5) resident files that were complete. -Around 09:37 AM, LPA witnessed 3 unlocked scissors on the cabinet mixed with pens and pencils in wooden holder; S1 corrected during visit. -Around 09:46 AM, LPA observed lysol cleaner, a can of air freshener, and clorox wipes unlocked in the hallway closet; S1 corrected during visit. -Around 09:50 AM, LPA observed R1's oxygen tank stored without it's stand in room #1; S1 corrected during visit. -Around 10:07 AM, there was a body of water present near the garage in the backyard where cement (approximately 10x10 ft. opening) had been poured due to the construction and rain but is inaccessible to residents. On 12/20/2023 at 12:30pm, Licensing Program Analysts (LPAs) L. Hall and T. Syess-Gibson, conducted an unannounced 1-Year Required inspection. LPAs met with Benita Deguzman, Caregiver and explained the purpose of the visit. Administrator, Cindy Chen, arrived at 12;53pm. The Administrator holds a certificate #6019228740 expires 8/29/2024. The facility’s fire clearance was approved for eight (5) non-ambulatory residents. LPA toured the facility including but not limited to bedrooms, bathrooms, kitchen, common area and back yard. The facility consists of six (6) total bedrooms, two (2) bathrooms, and one (1) office. All outdoor and indoor passageways are kept free of obstruction. A comfortable temperature is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 117.5 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of 7-day supply of non-perishable and 2-day of perishable foods. Smoke detectors/ carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 12/20/2022. Emergency Disaster Plan was last posted on 7/28/2023. First aid kit was observed to be complete. Continued on LIC809. Continued from LIC809. Five (5) staff records were reviewed, and all staff have criminal record clearance. All seven (7) resident records were reviewed and none contained the appraisal needs and services plan. LPA observed the following deficiencies: At 1:00pm, LPAs observed during record review none of the resident files contained an appraisal needs and services plan. At 1:48pm, LPA observed vitamins, cough medicines, and other medication in unlocked kitchen cabinet. At 1:55pm, LPA observed a wrench, screwdriver, and 3 bottles of air fresher in unlocked cabinet located over washer and dryer. The deficiencies were observed (see LIC809D) and cited from the California Code of Regulation, Failure to submit proof of corrections (POC) by plan of correction due date and/or any repeat deficiencies within a 12-month period may result in civil penalties.
