Type of visit: Required - 1 Year

On March 20, 2026, Licensing Program Analyst (LPA) Eboni Bentley arrived unannounced for the purposes of conducting a required 1-Year annual visit using the CARE Inspection Tool. LPA Bentley was greeted and granted entry into the facility by staff, after stating the reason for the visit. Administrators (Admins) Socorro Villanueva Sabino and Arjan Banez arrived shortly to assist with the visit. Socorro Villanueva Sabino has an Administrator Certificate which expires on March 29, 2027 and Arjan Banez has an Administrator Certificate which expires on October 19, 2026. The facility is licensed to operate for age 60 and over for (6) non-ambulatory residents, of which six (6) may be bedridden, and has a Hospice waiver for six (6). The building is a single story structure located in a residential neighborhood, which consists of the following: four (4) resident bedrooms, one (1) staff bedrooms, three (2) bathrooms, living area, dining area, kitchen, an outdoor covered seating areas, and an attached two car garage. LPA Bentley toured the inside and outside of the physical plant with Admin. There were no bodies of water or obstructions on the premises. All rooms were inspected and the facility was observed to be appropriately furnished at the time of visit. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Additional linens, comforters, and bath towels were adequately stocked and available. Bathrooms were found to be clean and operational with water temperatures measured between 105 degrees F to 114.6 degrees F. The kitchen was observed clean, all appliances were operational, and there is a two-day supply of perishable and seven-day supply of non-perishable food available. Toxins, disinfectants, sharps, and medications were secured and inaccessible to residents in care. The fireplace was covered with a screen and secure. The garage and backyard were observed free of clutter and debris and the facility has two exit gates that are operational Continues on LIC 809-C.... The smoke alarms and carbon monoxide detectors were operable. An emergency safety drill was last conducted on February 18, 2026 and are conducted quarterly. Emergency food, emergency water, and emergency supplies were stored in the garage. The facility has two (2) fire extinguishers that were charged, mounted, and serviced on September 5, 2025. First aid kit is maintained and contains all the necessary elements. A working telephone (714-968-9688) remains available and the facility has a device that can be used for video teleconference purposes. LPA Bentley conducted an audit of four (4) resident files (R1-R4), four (4) staff files (S1-S4), and medication and medication administration review were all in order and complete. LPA Bentley conducted four (4) resident interviews and two (2) staff interviews. Liability Insurance is effective May 30, 2025, and expires on May 30, 2026. Based on today’s observations, no deficiencies were cited during the visit, per Title 22, Division 6, Chapter 8 of the California Code of Regulations. Type of visit: Required - 1 Year

On April 28, 2025, at 1:00 PM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced required 1-Year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim was greeted and granted entry by Caregiver (CG) Evelyn Aransado. Administrator (AD) Socorro Villanueva Sabino arrived at the facility and LPA Kim explained the purpose of the visit. The facility is licensed to operate for six (6) nonambulatory residents, of six (6) which may be bedridden, and has a hospice waiver for six (6) residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: four (4) resident bedrooms, one (1) staff bedroom, three (3) bathrooms, living area, dining area, living room, kitchen, outdoor covered patio, and an attached two car garage. LPA Kim toured inside and outside of the physical plant with AD Villanueva. There were no bodies of water or obstructions in the facility. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Resident Room 1, Resident Room 2, Resident Room 3, and Resident Room 4. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 116.2 degrees F to 116.6 degrees F. A comfortable temperature of 74 degrees maintained in the facility. Evaluation Report Continues on LIC 809-C LPA Kim observed the facility to be appropriately furnished at the time of visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food both were available and maintained properly. Emergency food, emergency water, and emergency supplies are stored in the garage. During the visit, LPA Kim observed the facility's infection control practices, plan of operation, and screening protocols for visitors, staff, and residents. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). All mandated inspection control posters were posted. A working telephone (714-968-9688) remains available. Last emergency drill was conducted on March 22, 2025. First aid kit is maintained and contains all the necessary elements. Evidence of liability insurance is effective May 30, 2024, and expires on May 30, 2025. LPA Kim conducted an audit of resident files (R1-R4), staff files (S1-S4), and medication and medication administration record that were all in order and complete. LPA Kim conducted two (2) staff interviews and two (2) resident interviews. No deficiencies were cited during this inspection visit. Type of visit: Required - 1 Year

On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz conducted an unannounced visit for the purpose of conducting a required Annual inspection. LPA was greeted and granted entry into the facility by Caregiver 1 (CG1). Licensee/Administrator (L/AD) Socorro Villanueva Sabino arrived during today's visit and discussed purpose of today's visit. This is a a Residential Care Facility for the Elderly, licensed to provide services to six (6) Non-Ambulatory residents, of which (6) six may be bedridden and has a hospice waiver for (4) four residents. There are no active COVID-19 cases in the facility at this time. There are currently six (6) residents in care at this time, of which (5) five are receiving hospice care services. (SEE LIC 809-D) AD Socorro Villanueva Sabino has an Administrator certificate with expiration date of March 29, 2025. LPA Quiroz along with CG1 toured the interior and exterior of the facility. During today's inspection tour, LPA Quiroz observed Resident 1- Resident 6 in their bedrooms resting with staff conducting frequent round checks. LPA Quiroz interacted and interviewed with Caregivers and 1 of 6 residents during today's visit. LPA Quiroz inspected resident's bedrooms and bathrooms. The water temperature in 2 of 2 resident's bathrooms were recorded to be within normal limits. LPA Quiroz inspected resident's bedrooms and appeared to be clean. Facility temperature in resident's bedrooms and throughout the facility was recorded to be within normal limits. LPA Quiroz observed the emergency and disaster and evacuation plan. Facility has supply of emergency food, water and PPE in the garage area readily available for staff and residents in care. The Fire extinguisher observed last serviced on September 18,2023. LPA Quiroz observed functional and operational washer and dryer in the laundry room area. During today's visit, on or about 9:42am LPA Quiroz observed CG1 walking away from secured sharps drawer where knives are stored leaving the key in the cabinet, but was observed to go back and locked and secured drawer. On or about 11:46am, LPA Quiroz observed CG2 walking away from medication cabinet leaving the medication cabinet opened to assist a Resident, and was observed to go back to medication cabinet to lock and secure it. (SEE LIC 9102 TV) LPA Quiroz toured the outside of the facility and observed seating and shaded area in the backyard for residents and visitor's enjoyment. CONTINUED ON LIC 809-C PAGE... CONTINUED...LPA Quiroz reviewed 6 of 6 resident records and 3 of 3 personnel files. Faciity cited during today's visit. During today's visit, LPA Quiroz provided Consultation on Title 22 and Infection control.
