On March 19, 2026, Licensing Program Analyst (LPA) Eboni Bentley arrived unannounced for the purposes of conducting a required 1-Year annual visit using the CARE Inspection Tool. LPA Bentley was greeted and granted entry by staff, after stating the reason for the visit. Administrator (AD) Vivian Ann Ortiz- Luis was contacted by telephone and permission was granted for Caregiver to sign today’s inspection report. The facility is licensed to operate for age range 60 and over and approved for (6) non-ambulatory, of which one (1) may be bedridden in Room #3 only, with a Hospice waiver for six (6). The building is a single story structure located in a residential neighborhood, which consists of the following: six (6) resident bedrooms, one (1) staff bedrooms, two (2) bathrooms, living area, dining area, kitchen, an outdoor covered seating areas, and an attached two car garage. LPA Bentley toured inside and outside of the physical plant with staff. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for each resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked and available at the time of visit. Bathrooms were found to be clean and operational. The water temperatures in bathrooms measured at 112.8 degrees F to 113.2 degrees F. A comfortable temperature of 74 degrees F was maintained in the facility. CONTINUE TO LIC-809-C..... LPA Bentley observed the facility to be appropriately furnished at the time of visit. The kitchen was inspected and there is a two-day supply of perishable and seven-day supply of non-perishable food available. LPA observed four non-operational stove burners that do not self ignite and four cabinet doors in need of repair, with two hanging off the hinges and two unable to close. A deficiency is being cited. The smoke alarms and carbon monoxide detectors were operable. An emergency safety drill was last conducted on January 20, 2025, and are conducted quarterly. Emergency food, emergency water, and emergency supplies were stored in the garage. The facility has two (2) fire extinguishers that were charged, mounted, and serviced on January 5, 2026. First aid kit is maintained and contains all the necessary elements. A working telephone (714-963-0026) remains available, however the facility does not have a device that can be used for video teleconference purposes. A Technical Violation was provided. LPA Bentley conducted an audit of four (4) resident files (R1-R4), three (3) staff files (S1-S3), and medication and medication administration review were all in order and complete. LPA Bentley conducted four (4) resident interviews and two (2) staff interviews. Liability Insurance is effective August 8, 2025, and expires on August 8, 2026. Based on today’s observations, a deficiency is being cited during the visit, per Title 22, Division 6, Chapter 8 of the California Code of Regulations. Licensing Program Analyst (LPA) Ruth Martinez is conducting this unannounced visit for the purpose of completing an annual required inspection. LPA arrived at the facility and was greeted and granted entry by staff and LPA explained the nature of the visit. There are four residents at the facility and there is two residents receiving hospice services currently. LPA began the tour of the inside and outside of the facility. Facility stays within the capacity limitations. There is a minimum of one week of non-perishables foods and two days of perishables foods available. There is additional food storage in storage in a refrigerator located in the garage. The facility is maintained at a comfortable temperature. LPA inspected that medication is centrally stored in a safe locked storage cabinet in the kitchen. LPA reviewed medication and observed medication was labeled and stored inaccessible to residents in care. LPA inspected the bathroom and LPA measured the hot water temperature which measured 109.5 Fahrenheit degrees. All bathrooms observed to have a supply of soap, toilet paper and towels. Bathrooms are equipped with required safety measures such as non-skid mats and grab bars. Lighting is sufficient to ensure safety and comfort. The facility is equipped with sufficient hand hygiene, cleaning, and disinfecting supplies. LPA observed that toxic chemicals, cleaning solutions and disinfectants are stored locked underneath kitchen sink and locked storage in the garage. The facility has an available clean supply of linens. LPA inspected residents’ bedrooms which has sufficient lighting to ensure the safety and comfort. All bedrooms observed to have all required components. Storage space is provided for residents in their bedroom. Smoke detectors were tested and found to be operational. LPA toured the outside of the facility and observed outdoor passageways are free of obstructions. LPA observed there are shaded Continued on LIC809-C seating areas for residents’ enjoyment. LPA observed a fire extinguisher with service date of December 26, 2024, in kitchen. LPA began review of records. LPA reviewed two resident records. All the required documentation was present and current in the residents’ files reviewed. LPA reviewed two employee records. All employees present have a criminal record clearance and are associated to the facility. LPA observed records reviewed have a current First Aid certificate. Based on the observations made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. On today's date, Licensing Program Analyst (LPA) LPA Rosie Quiroz conducted an unannounced visit for the purpose of conducting a required Annual inspection. LPA was greeted and granted entry into the facility by Caregiver 1 (CG1). LPA Quiroz called Administrator (AD) Vivian Ann Ortiz-Luis and explained the nature of the visit. (AD) Ortiz-Luis arrived during today's visit. This facility is licensed to provide services to age range 60 and over, approved for six (6) Non-Ambulatory Residents of which 1 (one) may be bedridden in Room #3 only, and has a hospice waiver for six (6) residents. There are four (4) residents in care of which (3) three residents are receiving hospice care services There are no active COVID-19 cases in the facility at this time. Administrator Ortiz-Luis has an Administrator Certificate with expiration date of 9/8/2025. LPA Rosie Quiroz along with (CG1) toured the interior and exterior of the facility. During today's inspection tour, LPA Quiroz observed two of four residents in their bedroom resting and 2 of four residents in the dining area with staff supervision. LPA Quiroz interacted and interviewed with staff and residents during today's visit. Between 10:06am-10:20am while conducting inspection tour of the kitchen area, LPA Quiroz observed expired condiments, food items and expired canned food with expiration dates varying from 6/1/2020- 10/19/2023. This was verified with (CG1) and (AD) Ortiz-Luis. (See LIC 809-D) LPA Quiroz inspected resident's bedrooms and bathrooms. Water temperatures were recorded to be between 107.7-109.4 degrees Fahrenheit. LPA Quiroz inspected resident’s bedrooms and appeared to be clean. Facility temperature in resident's bedrooms and throughout the facility was recorded to be within normal limits. LPA Quiroz observed the emergency and disaster and evacuation plan. Facility has a supply of emergency food, water and PPE in garage area readily available for staff and residents. LPA Quiroz toured the outside of the facility and observed seating and shaded area backyard for residents and visitor's enjoyment in backyard area. CONTINUED ON NEXT LIC 809-C PAGE...
