On 10/09/2025 at 2:30 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a Case Management - Annual Continuation inspection to complete the Required Annual Inspection that was partially completed on 9/22/2025. Upon entry, The LPA stated the purpose of the visit to Staff Member Gina Jimenez. Administrator Victor Rodriguez arrived at approximately 2:45 PM. The LPA completed a review of the resident, staff, and facility records. The LPA reviewed the records of 5 residents and 5 staff members. No citations issued during the inspection. On 09/22/2025 at 2:15 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required Annual Inspection. Upon entry, the LPA stated the purpose of the visit to Caregivers Dafne Gala and Gina Jimenez. Administrator Victor Rodriguez arrived at approximately 3:00 PM. The LPA toured the facility including but not limited to residents’ rooms, bathrooms, kitchen, common areas and the backyard. The LPA observed adequate lighting for the comfort and safety of residents in all rooms. The temperature in the kitchen was measured at 74 degrees Fahrenheit. The hot water temperature was measured at 136 degrees Fahrenheit, not within the safe range of 105 to 120 degrees Fahrenheit. The residents’ bathrooms are equipped with grab bars and non-skid mats. There is more than the minimum of a one week supply of nonperishable food and 2 days of perishable food. Centrally stored medications, sharps, and toxic cleaners are inaccessible to residents in care. The LPA observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. Smoke detectors and carbon monoxide detectors were tested and found to be in operating condition. Fire extinguisher was fully charged and serviced on 11/21/2024. The Emergency Disaster Plan was last reviewed on 9/15/2025. Emergency, disaster, and fire drills are conducted monthly; the most recent drill was conducted on 9/1/2025. First aid kit was observed to be complete. Liability insurance certificate expires on 6/13/2026. 1 Type-A citation was issued during the inspection (refer to citation in LIC 809-D). The Required Annual Inspection was not completed. The LPA will return to complete inspection on another date and time. On 11/25/2024 at 12:30 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a Case Management - Annual Continuation inspection to complete the Required Annual Inspection that was partially completed on 11/06/2024. Upon entry, The LPA stated the purpose of the visit to Staff Member Mariela Cuaresma Medina. Administrator Annette Sanchez arrived at approximately 1:45 PM. The LPA inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, garage, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. At approximately 2:25 PM, the kitchen hot water temperature was 107.4 and the living room temperature was 72.5 degrees Fahrenheit. The fire extinguisher was last serviced on 11/21/2024. The carbon monoxide and smoke detectors were fully operational. The LPA observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA completed a review of the resident, staff, and facility records. The LPA reviewed the records of 5 residents and 5 staff members. The LPA interviewed 4 staff members. No citations issued during the inspection. Upon entry, the LPA stated the purpose of the visit to Staff Member Mariela Cuaresma Medina. Administrator Annette Sanchez arrived at approximately 1:45 PM. The LPA inspected the interior and exterior of the facility. During the inspection, the LPA measured the kitchen hot water temperature at 107.4 degrees Fahrenheit. The fire extinguisher was serviced on 11/21/2024. Those cleared the citations from 11/6/2024. No citations issued during the inspection. On 11/06/2024 at 11:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPA stated the purpose of the visit to Staff Member Gina Jimenez. Administrator Annette Sanchez arrived at approximately 11:55 AM. The LPA inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, garage, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. At approximately 2:45 PM, the kitchen hot water temperature was 125.1 degrees and the dining room temperature was 75.5 degrees Fahrenheit. The fire extinguisher was last serviced on 12/23/2022. The carbon monoxide and smoke detectors were fully operational. The LPA observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA reviewed facility records and records of 5 residents. 2 Type-A and 1 Type-B citations were issued during the inspection. Administrator will provide LPA copies of the following documents by 10/21/2024: 1. LIC 308 Designation of Facility Responsibility 2. LIC 500 Personnel Report 3. LIC 610E Emergency Disaster Plan (9 pages) 4. $3M Liability Insurance certificate The annual inspection is incomplete. On 11/01/2023 at 9:30 AM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct an annual inspection. Upon arrival, LPA explained the purpose of the visit to Caregiver Gina Jimenez. Administrator (ADM) Annette Sanchez arrived at approximately 10:30 AM. During the Inspection, the LPA inspected the facility inside and outside. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 68 degrees Fahrenheit. The LPA observed that the lighting in all rooms is adequate for the comfort and safety of the residents. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detector were in operating condition. Fire extinguishers was observed to be fully charged. First aid kit was observed to be complete. LPA interviewed 2 staff members and 2 residents, and reviewed the records of 4 staff and 3 residents. No citations issued during visit.
