On 12/18/2025 at 1:15 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required Annual Inspection. Upon entry, the LPA stated the purpose of the visit to Caregiver Salve Regina Piguing and Applicant Dennis Sayson. The LPA toured the facility including but not limited to residents’ rooms, bathrooms, kitchen, common areas and the backyard. The LPA observed adequate lighting for the comfort and safety of residents in all rooms. Inside and outside areas are free of obstruction and no bodies of water. The temperature in the living room was measured at 70.7 degrees Fahrenheit. The hot water temperature was measured at 112.1 degrees Fahrenheit, which was within the safe range of 105 to 120 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and slip-resistant mats. There is more than the minimum of a one-week supply of nonperishable food and 2 days of perishable food. Centrally stored medications, sharps, and toxic cleaners are inaccessible to residents in care. The LPA observed the required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. Smoke detectors and carbon monoxide detectors were tested and found to be in operating condition. The fire extinguisher was fully charged and replaced within one year on 11/7/2025. The Emergency Disaster Plan was last reviewed on 12/18/2025. Emergency, disaster, and fire drills are conducted quarterly; the most recent drill was conducted on 9/27/2025. First aid kit was observed to be complete. Liability insurance certificate expires on 5/30/2026. The LPA reviewed 5 resident records and 5 staff records. On 1/17/2024 at 1:00 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct an annual required inspection. The LPA informed Staff Members Fe Alolod and Salve (Regina) Piquing of the reason for visit. At approximately 1:30 PM, Executive Director (ED) Racquel Flores and Licensee Steve Chou arrived at the facility. The LPA inspected the facility inside and outside with the staff and licensee. The inspection included the kitchen, dining area, living room, bedrooms, bathrooms, and yards. An adequate amount of food supplies were observed, more than the required minimum of 2 days of perishable and 7 days of non-perishable food. The central storage for medications and cleaning supplies were stored in locked cabinets. Facility has a fully functioning 2-in-1 smoke and carbon monoxide detector. Facility conducts disaster/emergency and fire drills on a quarterly basis; records showed that the most recent drill was conducted on 10/24/2024. The fire extinguisher was fully charged and last serviced on 11/18/2024. The indoor temperature in the living room was 68.4 degrees Fahrenheit and the maximum hot water temperature was 110.4 degrees Fahrenheit, both within the acceptable temperature range. The LPA reviewed 4 resident and 5 staff records. No citations were issued during this inspection. Type of visit: Case Management - Health Checks

On 4/9/2024 at 3:15 PM, Licensing Program Analyst (LPA) J. Sampair arrived at facility unannounced to conduct this case management visit. Upon arrival, the LPA stated the purpose of the visit to Licensee Steve Chou, which was to verify the health and safety of their new resident who was admitted on 4/6/2024 due to the fire at Casa Blanca Homes (071441125). At 3:15 PM, the LPA observed Resident R1 in their room. They were safe and healthy in their bed. The Licensee stated that the last of their belongings were brought only an hour earlier. On 1/22/2024 at 10:30 AM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct the Required Annual Inspection of the facility. Upon arrival, LPA stated the purpose of the visit to Caregiver Arnel Corpuz. Licensee Steve Chou and Administrator Raquel Flores arrived at approximately 11:45 AM. The LPA reviewed the records of 5 staff members; inspected the kitchen, living room, 1 bathroom, and 1 resident bedroom. All indoor passageways were free of obstruction. LPA observed a 7 day supply of nonperishable and 2 day supply of perishable foods on hand. 1 Type-A citation and 1 Type-B citation were issued (for details refer to LIC809-D). By 1/29/2024, Licensee will send updated forms to LPA: · LIC500 - Personnel Report · LIC308 - Designation of Facility Responsibility · LIC610E - Emergency/Disaster Plan · Evidence of Liability Insurance Required Annual Inspection incomplete.
