On 1/12/2026 at 9:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required Annual Inspection. Upon entry, the LPA stated the purpose of the visit to Administrator Erlinda Tuazon. The LPA toured the facility including but not limited to residents’ rooms, bathrooms, kitchen, common areas and the backyard. The LPA observed adequate lighting for the comfort and safety of residents in all rooms. Inside and outside areas are free of obstruction and no bodies of water. The temperature in the dining room was measured at 70.1 degrees Fahrenheit at 10:18 AM. The maximum hot water temperature was measured at 137.3 degrees Fahrenheit, not in the safe range of 105 to 120 degrees Fahrenheit. The residents’ bathrooms are equipped with grab bars and slip-resistant mats. No smoking signs posted for residents on oxygen. There is more than the minimum of a one-week supply of nonperishable food and 2 days of perishable food. Centrally stored medications, sharps, and toxic cleaners are inaccessible to residents in care. The LPA observed the required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. Smoke detectors and carbon monoxide detectors were tested and found to be in operating condition. The fire extinguishers were fully charged but the replacements were purchased on 9/26/2024, which was longer ago than the one year requirement. The Emergency Disaster Plan was not reviewed within the past year; the most recent review was on 7/5/2023. Facility cited because emergency, disaster, and fire drills were not conducted on a quarterly basis. The most recent drill was conducted November 2025. First aid kit was observed to be complete. Liability insurance expires on 8/1/2026. Continued on LIC 809-C . . . . . . On February 26, 2025 at 01:00 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct an annual required inspection. Upon entrance, the LPA informed Administrators Gabriel and Erlinda Tuazon of the purpose of the visit. The LPA inspected the inside and outside of the facility. The inspection included the kitchen, dining area, living room, bedrooms, bathrooms, storage sheds, and yard. An adequate amount of food supplies were observed, more than the required minimum of 2 days perishable and 7 days non-perishable food. The central storage for medications was locked. The dangerous objects were stored in locked cabinets, but toxic cleaners and paint supplies were not stored in a locked cabinet. Facility has working smoke and carbon monoxide detectors. Facility conducts disaster/emergency and fire drills on a quarterly basis; records showed that the most recent drill was conducted on January 12, 2025. The fire extinguishers were fully charged and replaced July 26, 2024. The indoor temperature was 69.7 degrees Fahrenheit, in the acceptable range, but the maximum hot water temperature was 122.3 degrees Fahrenheit, which was not in the acceptable range. The LPA reviewed 5 resident and 5 staff records. Citations for 2 A-Type and 2 B-Type deficiencies were issued (for details refer to LIC 809-D). On 02/20/2024 at 9:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct a required annual inspection. LPA explained the purpose of the visit to Licensee Erlinda Tuazon. LPA toured the interior and exterior of the facility. LPA inspected the kitchen, dining area, restrooms, community living spaces, bathrooms, resident rooms, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Temperature in the dining room was measured at 70.8 degrees Fahrenheit at 10:50 AM. Fire extinguisher was fully charged and last serviced on 10/3/2023. Carbon monoxide and smoke detectors were fully operational. The LPA observed postings in the facility that included a complaint poster, Ombudsman and Personal Rights posters, Theft and Loss Policy, Rights to Resident Council, and Rights to Family Council, which were posted in a prominent location. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA reviewed facility records, the records of 5 staff members, and the records for 4 residents. The LPA interviewed 2 staff members and 2 residents. No citations issued during inspection.
