On 08/21/2026, at 01:45 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required 1 Year inspection. Upon arrival, the LPA explained the purpose of the visit to Administrator (ADM) Victor Rodriguez. The LPA and ADM toured the community inside and outside. The tour of the community included, but was not limited to, residents’ rooms, bathrooms, kitchen, common areas, garage, and yards. The community was clean, appropriately furnished, and well lit. There are no bodies of water on the grounds of the community. The interior and exterior passageways of the community were free from obstructions. The LPA observed the required postings in the community. The residents’ bathrooms are equipped with grab bars and slip-resistant mats. There are more than the required minimum 7-day supply of nonperishable foods and 2-day supply of perishable foods on hand. The temperature of the two refrigerators were below 40 degrees and the two freezers were below 0 degrees Fahrenheit. The maximum hot water temperature was measured at 114 degrees Fahrenheit in the kitchen. At approximately 2:40 PM, the inside temperature was measured at 72 degrees Fahrenheit in the kitchen. The first aid kit was observed to be complete. The smoke and carbon monoxide detectors were operational. The fire extinguisher was last serviced on 03/16/2026. The Emergency Disaster Plan was reviewed by the ADM on 06/16/2026. Emergency and disaster drills were conducted for the sufficient number of staff each quarter and the most recent one was conducted on 08/11/2026. The liability insurance is current through 06/13/2027. An administrator is on site more than the minimum of 20 hours a week to ensure proper business operations are being conducted. The LPA reviewed community records, 5 resident records, and 5 staff records. No citations were issued during this inspection. On 08/05/2025 at 01:20 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required Annual Inspection. Upon entry, the LPA stated the purpose of the visit to Administrator Annette Sanchez. The LPA toured the facility including but not limited to residents’ rooms, bathrooms, kitchen, common area and courtyard. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The temperature in the kitchen was measured at 73.9 degrees Fahrenheit. The hot water temperature was measured at 132.2 degrees Fahrenheit, above the safe range of 105 to 120 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2 days of perishable foods. Centrally stored medications, sharps, and toxic cleaners are inaccessible to residents in care. The LPA observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was fully charged and serviced. The Emergency Disaster Plan was last reviewed on 04/01/2025. Emergency, disaster, and fire drills are conducted monthly; the most recent drill was conducted on 08/01/2025. First aid kit was observed to be complete. The LPA reviewed 5 resident and 5 staff records; all were complete. 1 Type-A citation was issued during the inspection. On 8/6/2024 at 8:45 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required Annual Inspection. Upon entry, LPA stated the purpose of the visit to Supervisor Bertha Uriarte. Administrator Alex Sanchez arrived at approximately 11:00 AM. The LPA inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Temperature in the dining room was measured at 74.6 degrees Fahrenheit at 9:51 AM. The fire extinguisher was serviced 7/17/2024. The carbon monoxide and smoke detectors were fully operational. The LPA observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA reviewed facility records, records of 5 residents, and records of 5 staff members. No citations were issued during the inspection.
