On 05/22/2026 at 09:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this Required One Year inspection. Upon entry, the LPA stated the purpose of the visit to Administrator Joy Manalang-Enriquez, RN. At approximately 01:30 PM Administrator Annette Sanchez arrived. The LPA toured the facility, including but not limited to, residents’ rooms, bathrooms, kitchen, common areas, and the yard. The LPA observed adequate lighting for the comfort and safety of residents in all rooms. Inside and outside areas are free of obstruction and no bodies of water. The temperature in the living room was measured at 72.5 degrees Fahrenheit. The maximum hot water temperature was measured at 112.8 degrees Fahrenheit in the kitchen. The residents’ bathrooms are equipped with grab bars and slip-resistant mats. There is more than the minimum of a one week supply of nonperishable foods and 2 days of perishable foods. The LPA observed the required postings in the facility. Smoke detectors and carbon monoxide detectors were tested and found to be in operating condition. The fire extinguisher was fully charged and last serviced on 12/15/25. The Emergency Disaster Plan was reviewed within the past year; the most recent review was on 5/22/2026. Emergency, disaster, and fire drills were conducted on a quarterly basis; the most recent was 4/9/2026. First aid kit was observed to be complete. Liability insurance expires on 7/30/2026. On 5/9/2025, at 9:00 AM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Required Annual Inspection of the facility. Upon arrival, LPA stated the purpose of the visit to Caregiver Jasmin Sanchez. Licensee / Administrator (ADM) Joy Manalang-Enriquez arrived at approximately 10:00 AM. The LPA toured the interior and exterior of the facility, inspecting the kitchen, food and emergency supply storage areas, dining area, shared restrooms, community living spaces, client rooms, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. The maximum hot water temperature was 127.0 degrees Fahrenheit and the temperature in the common area was 80.3 degrees Fahrenheit. The carbon monoxide and smoke detectors were fully operational. The fire extinguishers were last replaced on 5/7/2025. The LPA observed postings in the facility that included a complaint poster, Ombudsman, and Personal Rights posters. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. Type of visit: Post Licensing

On 7/23/2024 at 12:45 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct the Post Licensing Inspection. Upon entry, LPA stated the purpose of the visit to Licensee Joy Manalang-Enriquez. The LPA inspected the interior and exterior of the facility. The inspection of the physical plant included the kitchen, dining area, restrooms, community living spaces, resident rooms, storage areas, and the grounds of the facility. More than the required minimum of 7 days of nonperishable and 2 days of perishable foods were appropriately stored. Temperature in the dining room was measured at 76.4 degrees Fahrenheit at 1:33 PM. The fire extinguisher was fully charged and last replaced 3/13/2024. The carbon monoxide and smoke detectors were fully operational. The LPA observed required postings in the facility, including the Residential Care Facility for the Elderly Complaint Poster, Ombudsman and Personal Rights posters, and the Theft and Loss Policy. An administrator is on site more than the minimum of 20 hours a week to oversee the proper business operations. The LPA reviewed facility records, records of 5 residents, and records of 5 staff members. The LPA interviewed 2 staff members and 2 residents. Type of visit: Prelicensing

On 05/10/2024 at 12:45 PM, Licensing Program Analyst (LPA) J. Sampair arrived unannounced to conduct a change of ownership prelicensing visit. Upon entry into the facility, the LPA informed Applicant Joy Manalang-Enriquez of the purpose of the visit. The LPA toured the inside and outside of the facility. The LPA inspected the kitchen, common areas, bedrooms, bathrooms, and storage areas of the facility. The facility was clean, appropriately furnished, and well lit. More than the 2 days of perishable and 7 days of nonperishable food supplies were available. No body of water was on the facility grounds. Medications are centrally stored. The LPA reviewed the staff, resident, and facility records stored at the facility. Bathrooms and showers were observed to be fully functioning and clean. Carbon monoxide and smoke detectors operational and the fire extinguishers were last replaced on 03/13/2024. First aid kit inspected and complete. Facility has adequate emergency lighting. Facility passed pre-licensing inspection and completed Component III training with the Applicant. Final review of application and license to be granted by Central Applications Bureau analyst.
