On 11/25/25, Licensing Program Analyst (LPA) Greg Clark arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator, Victor Rodriguez and explained the purpose of the visit. LPA toured facility including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ in the kitchen sink was measured at 113.2 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of non-perishable and 2 day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 11/05/25. Emergency Disaster Plan was last reviewed on 11/25/25. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 11/01/25. LPA reviewed 3 residents records and 3 staff records; all were complete. LPA also reviewed a sample of resident’s medications. On 12/10/2024 at 11:10 AM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Administrator/Licensee, Sara Abraham and explained the purpose of the visit. Co-Administrator, Annette Sanchez arrived shortly after. The facility’s fire clearance was approved for capacity eight (8) non-ambulatory. Hospice waiver for two (2) residents. Administrator Certificate #6006207740 expires 04/16/2025. LPA toured facility with Annette including but not limited to bedrooms, bathrooms, kitchen, common area and backyard. The facility consists of ten (10) total bedrooms which eight (8) bedrooms are occupied by the residents and two (2) bedrooms is occupied by staff. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 75 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at degrees 106.5 Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was last serviced on 12/10/2024. Emergency Disaster Plan was last posted on 12/10/2024. First aid kit was observed to be complete. Type of visit: Case Management - Other

On 05/09/2024 at 1:30 PM Licensing Program Analyst (LPA) Lori Alexander conducted an unannounced Case Management visit regarding an Resident's (R1) Physician's Report that was received by CCLD. LPA met with Blanca Cuestas and explained the purpose of the visit. Blanca phoned, Co-Administrator, Annette Sanchez, who arrived to the facility shortly. During Annual Visit on 01/03/2024 R1 was missing her Physician's Report with physician's signature. Administrator submitted R1's Physician's Report to LPA L. Alexander via email on 03/05/2024 and the report had a restricted health condition; Foley Catheter. LPA responded to Administrator and asked who was caring for the catheter and Administrator stated that R1 has home health. LPA emailed Administrator on 04/08/24 to advise that R1 will need an exception request for the restricted health condition and to submit the required documents by 04/12/24. LPA spoke with Administrator who stated that R1 no longer has the Foley Catheter and only had a temporary issue with her kidneys. LPA observed R1 sitting in the TV room and she did not have a catheter. LPA advised Administrator to update R1's Physician's Report and also Appraisal Needs and Services. All outdoor and indoor passageways are kept free of obstruction. There are no bodies of water observed. A comfortable temperature is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the kitchen sink was measured at 107.5 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of non-perishable and 2 day of perishable foods. Centrally stored medication and sharps were locked and inaccessible to residents. Smoke detectors and carbon monoxide detectors were in operating condition during visit. Fire extinguisher was observed to be operational during visit. Emergency Disaster Plan was last posted on 3/24/24. First aid kit was observed to be complete. LPA met with Caregivers Yanci Morales and Zaharah Nansimbi, and explained the purpose of the visit. Yanci phoned the Administrator to inform. Annette Sanchez arrived approximately 15 mins later.
