On 03/13/2026 at 10:45 AM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Caregiver, Christopher Mendoza and explained the purpose of the visit. Christopher phoned, Administrator/Licensee, Victoria Lingbanan, to inform. Johnny and Victoria Lingbanan arrived shortly after. The facility’s fire clearance was approved for capacity of twenty-two (22) residents. Where all can be non-ambulatory . Hospice waiver approved for five (5) residents. Administrator certificate #7001801740 expires 11/25/2027. LPA toured the facility with Licensees Johnny & Victoria Lingbanan including but not limited to the residents’ apartments, bathrooms, multiple activity rooms, kitchen, common area and courtyard. There are no bodies of water observed. LPA observe lighting in all rooms are adequate for the comfort and safety of the residents. Hallway temperature was maintained at 76 degrees F. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in a sample of residents’ shared bathroom were measured at 106.7 and 109 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one-week supply of nonperishable and 2-day of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care. Emergency Disaster Plan was last posted on 01/07/2026. First aid kit was observed to be complete. Emergency disaster drill was last conducted on 01/15/2026. LPA reviewed residents records. LPA reviewed ten (10) staff records and 10 of 10 have current first aid training and associated to the facility. On 03/20/2025 at 9:45 AM, Licensing Program Analyst (LPA) L. Alexander arrived unannounced to conduct 1-Year Annual Required inspection. LPA met with Caregiver, Anabelle Mendoza and explained the purpose of the visit. Anabelle phoned Administrator/Licensee, Victoria "Vickie" Lingbanan to inform. Vickie arrived to the facility approximately 1 hour later. Facility’s fire clearance was approved for capacity twenty-two (22) residents in which all may be non-ambulatory. Hospice waiver approved for five (5) residents. Administrator certificate #6006625740 expires 11/25/2025. LPA toured the facility with Vickie including but not limited to five (5) residents’ apartments, bathrooms, multiple activity rooms, kitchen, common area and courtyard. There are no bodies of water observed. LPA observe lighting in all rooms are adequate for the comfort and safety of the residents. Hallway temperature was maintained at 73 degrees F downstairs and 75 degrees F upstairs. The hot water temperature in a sample of residents’ shared bathroom were measured at 102.4, 102, 103.3 degrees Fahrenheit. Administrator raised hot water temperature and now water is measuring at 115 degree Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care. Type of visit: Case Management - Deficiencies

On 05/03/2024 at 11:10 AM, Licensing Program Analyst (LPA) Lori Alexander arrived unannounced to conduct a Case Management visit. LPA met with Licensee/Administrator, Victoria Lingbanan, and explained the purpose of the visit. Back on 04/25/2024 during Plan of Correction (POC) Inspection visit, LPA L. Alexander assessed civil penalties due to deficiencies not cleared for CCR 87303(a). On 04/26/2024 Administrator and Licensees, Johnny and Victoria Lingbanan, submitted photos of the side and back yards. On 05/03/2024 LPA L. Alexander toured the outside backyards with Johnny Lingbanan and observed that all the excess items in the back yards were removed. LPA L. Alexander observed that the yard was cleaned. Assessing civil penalties will stop on 04/25/2024. No deficiencies issued during the visit. On 04/25/2024 at 10:45 AM, Licensing Program Analyst (LPA) Lori Alexander arrived unannounced to conduct Proof of Correction (POC) visit. LPA met with Caregiver, Dorris, and informed the reason for visit. Dorris phoned the Licensee/Administrator, Victoria Lingbanan to inform. Victoria arrived around 11:15 AM. On 03/26/2024, LPA conducted an Annual visit in which deficiencies were cited. The POC due date was 04/16/2024. Administrator failed to submit the POC by the due date and this is why LPA came to make a POC visit. Facility has the following deficiencies that was not cleared: 87303(a) $100.00 x 9 Days = $900.00 Civil Penalties in the total amount of $900.00 is assessed today for failure to meet POC date for deficiencies. Facility is subject to ongoing daily civil penalties until deficiencies is corrected. Exit interview conducted. LPA toured the facility with Victoria including but not limited to 4 residents apartments, bathrooms, multiple activity rooms, kitchen, common area and courtyard. There are no bodies of water observed. LPA observe lighting in all rooms are adequate for the comfort and safety of the residents. Hallway temperature was maintained at 74, 74 degrees F. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in a sample of residents’ shared bathroom were measured at 110.1, 108.4, 120.00 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of one week supply of nonperishable and 2-day of perishable foods. Centrally stored medications, sharps and toxic are locked and inaccessible to residents in care.
