Licensing Program Analyst (LPA) K. Hiratsuka conducted this visit to complete the complaint. LPA met with Assistant Administrator Ivy Garner. LPA interviewed the resident in question, staff, and reviewed the resident's file. The item in question is a bottom set of dentures. LPA observed the resident wearing the top ones and moving it around with their tongue during an interview. The resident in question does have a diagnosis of dementia and it is difficult to determine if the resident is capable to know when they take out the dentures and where they put it. LPA also cannot determine how much the resident comprehends regarding keeping the dentures clean. Staff stated the resident is able to clean them with assistance and that the dentures are put in a solution when not in use per the instructions. Licensing Program Analyst (LPA) K. Hiratsuka conducted this visit to complete the complaint. LPA met with Assistant Administrator Ivy Garner LPA has spoken with residents, staff, reviewed documentation. Title 22 regulations requires an alert system for residents to request assistance. The regulations do not state a specific time frame the calls for assistance have to be answered. LPA was unable to determine if the resident was able to articulate time. Staff stated there were times they were notified by a witness the resident used the call button and the resident did not. LPA interviewed a couple of residents and the response times varied. Staff stated they do frequent checks as well as respond to call lights within ten minutes. The facility is not required to have a call system log and this facility does not. LPA cannot determine whether staff responded to the resident alerts or not or in what time frame. Allegation investigated: Staff mistreating residents.

On 4/24/2024 LPA Tryon went to the facility to complete the complaint. LPA met with Assistant Administrator Ivy Garner LPA has spoken with residents, staff, reviewed documentation. Regarding the allegation that staff are mistreating residents, LPA has interviewed staff involved with the incident mentioned in the complaint, spoke with residents involved and briefly with other residents. At this time, there is no proof that the staff actually mistreated the residents involved. One resident did fall when staff tried to open the bedroom door, but it's not completely clear if staff pushed the door hard enough to push the resident; or if she just lost her balance and fell. Stories related by staff differ from story related by residents involved. LPA has spoken with other residents and they have not seen any staff mistreat other residents or themselves, and related that staff is helpful and respectful.
