On 06/09/2026, Licensing Program Analysts (LPAs) Arielle Pascua and Sulma Lopez arrived unannounced to this facility to deliver complaint findings for the allegations above. LPAs met with Facility Designated Administrator (FDA), Ariel Thomas and explained the purpose of the visit. Current census was 78. A brief interview with FDA Thomas was conducted. Allegation: Staff do not allow resident to possess personal belongings. It was alleged that staff do not allow resident to possess personal belongings. During the course of this investigation, the department conducted interviews and reviewed facility records. Based on interviews conducted, it was determined that the facility confiscated a resident's cell phone due to the resident's failure to comply with established house rules. Further interviews revealed that the resident acknowledged retaining the cell phone beyond the agreed-upon time and using it in a manner that violated facility guidelines. Allegation investigated: Illegal eviction

Licensing Program Analyst (LPA) Jason Lund arrived unannounced to complete a complaint investigation. LPA Lund met with Administrator Misty Speegle and explained the reason for the visit. Census: 72 Illegal eviction- Based on records reviewed, staff interviewed, and attempted interviews with reporting party. On 5/20/2024 Clinet (C1) was aggressive towards staff swinging and punching at staff, due to C1’s behaviors C1 was sent out to the hospital. On 5/22 at 3:58pm facility management was having a meeting, during the meeting the facility called C1’s conservator who gave a verbal order for the bed to be discharged as of 5/20/24 when C1 went to the hospital. After the call, conservator order was executed at the facility. LPA Lund attempted to call and email the Tulare County conservator office and got no response. On 09/15/2023, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a complaint visit. LPA Pascua was greeted by Facility Designated Administrator, Zak Davis and explained the purpose of the visit. The purpose of this visit is to deliver complaint findings for the allegations above. Current census was 78. A brief interview with FDA Davis was conducted. Allegation: Facility staff are over medicating resident. It was alleged that facility staff are over medication resident. During the course of this visit, LPA Pascua conducted interviews and reviewed facility records. Based in interviews conducted, it was learned that R1 has been residing at the facility since 2018 and needs consistent prompting when asked to get ready to leave the facility, go to the bathroom, and take medication. It was denied by all staff that have been interviewed that R1 has been overmedicated. LPA reviewed Medication Administrator Records and medications for R1. LPA Pascua was greeted by Facility Designated Administrator, Zak Davis and explained the purpose of the visit. The purpose of this visit is to deliver complaint findings for the allegations above. Current census was 78. A brief interview with FDA Davis was conducted. Allegation: Facility is not adequately staff to meet the needs of the residents in care. It was alleged that the facility is not adquately staffed to meet the needs of the residents in care. During the course of this investigation, LPA conducted interviews and reviewed facility files. LPA conducted 7 staff interviews. 7 out of 7 staff memebrs state that they are able to meet the residents needs and feel that they have enough staff on all shifts. 7 out 7 staff members deny that they are understaffed at this time. Licensing Program Analyst (LPA) Victoria Brown arrived unannounced on 7/10/23 at 10:20am to conclude the complaint investigation. LPA met with Zachary Davis, Administrator and stated the purpose of the visit. Upon arrival, LPA observed residents participating in a group activity. LPA reviewed documents submitted to Community Care Licensing (CCL). LPA conducted a random amount of interviews during this visit. LPA observed there is a sink not working in 1 of the public restrooms, however, there are 3 others that are working during this visit. LPA also observed there is 1 showerhead leaking, however, there are 6 others to be used by residents. Administrator has been working on repairing those items prior to todays visit and scheduled repair for this week. Based on interviews with a random amount of Public Guardian offices, Conservators, Residents and staff the investigation revealed that there is an insufficient amount of evidence.
