Unannounced Annual visit made out to this facility on 05/01/2026 by Licensing Program Analyst (LPA) Charlie Yang. This LPA was met by the facility caregiver, Maria Zavala, who was briefly interviewed at this time. This LPA requested that she go ahead and contact the facility designated Administrator, Paula Turpo, to inform her that CCL was present at this time. The facility designated Administrator, Paula Turpo, arrived later to this facility while this LPA was conducting this annual visit. Current census was 6 residents. It was learned that there weren't any residents under the care of hospice at this time. This facility does have an approved waiver to be able to accept and retain up to (5) residents under the care of hospice at any given time. It was learned that this facility has a program to be able to accept and retain dementia residents at any given time. It was learned that there were (2) residents diagnosed with dementia at this time. It was learned that there were (2) residents receiving services through home health at this time. It was learned that this facility does not have a bedridden fire clearance to be able to accept or retain any residents deemed to be bedridden at any given time. Tour of this facility was conducted. Dining area, living area, and all other areas intended for resident use were toured. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Linen closet, located in facility hallway closet, was reviewed and observed to contain a sufficient supply of towels, sheets, and bedding able to meet the needs of the residents at this time. Kitchen area was toured. Kitchen drawers and cabinets were opened and reviewed. Food supply for 2-day perishable and 7-day nonperishable quantities was reviewed to make sure that they were in compliance at all times. Pantry area was toured. Administrator certificate # 7020702740 for Paula Turpo which was observed to expire on 10/31/2027 and in compliance at this time. Medication cabinet, located in the facility kitchen cabinets, was observed to be locked and made inaccessible to the residents at this time. First aid kit, located in the kitchen cabinet, was reviewed. This LPA observed that it did contain all of the required components at this time. Fire extinguisher located hanging on the wall adjacent to the kitchen was observed to have been annually inspected by the local fire extinguisher company, Touch Down Fire Inc, with the inspection date of 04/01/2026 and in compliance at this time. Facility resident bedrooms were toured. Furniture and furnishings were observed to be sufficient and able to meet the needs of the residents at this time. Facility resident restrooms were toured. Grab bars and non skid mats were observed to be present and in good repair at this time. Hot water temperatures were taken to make sure that they were within the allowed range of 105-120 degrees at all times. A tour of the facility exterior grounds was conducted. A review of the facility perimeter fence, side gates, and all other exits was conducted. A review of (3) facility personnel records was conducted and noted on the following LIC 859. A review of (6) facility resident records was conducted and noted on the following LIC 858. The following forms and documents were requested to be updated and submitted into CCL for review by this LPA: LIC 308 LIC 400 LIC 500 LIC 610 The following deficiencies were observed and cited on the following LIC 809-D pursuant to Title 22 Rules and Regulations, Health and Safety Codes. On 04/10/2025, Licensing Program Analysts (LPAs) Arielle Pascua and Triel Lindstrom arrived unannounced to this facility to conduct an annual visit. LPAs were greeted by Staff Member (SM), Maria Zavala and explained the purpose of the visit. LPA Pascua asked SM Zavala to call the Facility Designated Administrator (FDA), Paula Turpo to inform them that CCL was present. Shortly after, LPAs met with FDA Turpo. The purpose of this visit was to conduct an annual visit. Current census was 5. A brief interview with FDA Turpo was conducted. LPAs reviewed staff and resident files. Resident files were complete; admission agreements are stored in a separate binder. Staff files are complete as well, and everyone is compliant with First Aid requirements. The LPAs toured the facility with Administrator Turpo. The interior is clean and free of odors. The lighting and furnishings are adequate. The inside temperature and water temperature in the bathrooms are within the acceptable range. The bathrooms have secure grab bars and non-slip mats. The facility is stocked with 2-days worth of perishable and 7 days of nonperishable foods. Medications and sharps are in locked cabinets and drawers, and cleaning products are in the garage and laundry room with a locked door. There are two fire extinguishers, both were service March 26, 2025. There are two video cameras in common areas, one in the hallway and one in the living room; the plan of operations covers this. The front and backyards are well maintained and walkways are free from obstructions. The side gate is self-closing and latching. A technical assistance is being provided for Section 87465(H)(6) for medication management. Based on the observations made during today's visit, no deficiencies were cited pursuant to the California Code of Regulations (CCR) Title 22, Division 6. On 4/2/24 at approximately 10:20am Licensing Program Analyst (LPA) Maja Jensen arrived at facility unannounced to conduct a required 1 year annual inspection. LPA Jensen met with Administrator Paula Turpo and explained the purpose of today's visit. There are currently 5 residents. The facility is licensed for 6 non-ambulatory residents and has a hospice waiver for 5. The facility serves clients that require dementia care. LPA Jensen toured the interior and observed the facility to be sanitary and free of odor. There was adequate furnishings and lighting throughout. The bathrooms were equipped with grab bars and non-slip flooring in the bath or shower. There is a paper towel dispenser in the bathrooms to avoid the sharing of towels. The facility maintains an adequate supply of linen. The water temperature was measured at 105.1 degrees and is in compliance. The thermostat was set at 71 degrees which is within the required range of 68-85 degrees Fahrenheit. LPA Jensen toured the kitchen and observed a 2 day supply of perishable food and a 7 day supply of non-perishable food. LPA Jensen observed the facility sketch to accurately reflect the facility layout and room usage. Knives, medications and cleaning supplies are locked and inaccessible to residents in care. LPA Jensen observed the first aid kit to be complete. There is emergency lighting available. The emergency disaster plan is posted and in compliance. LPA Jensen observed all required postings to be prominently displayed. The fire extinguishers were newly purchased and in compliance. The smoke detector and carbon monoxide detector were tested and found to be in good working order. LPA Jensen toured the grounds and observed the grounds to be well maintained with all paths free of obstruction. There is outdoor furniture and and a shaded area available for resident outdoor activities. All window screens were observed to be in good repair. The facility was observed to have a back gate that is not self latching. LPA Jensen reviewed the liability insurance and determined it to be in compliance. A copy of the staff schedule was provided in lieu of the LIC 500. LPA Jensen interviewed 1 staff member and determined that that additional training on personal rights would be beneficial. Technical assistance was provided. LPA Jensen interviewed 2 residents who both stated they are satisfied with all aspects of their care. LPA Jensen reviewed 5 of 5 resident files and determined them to be compliant. LPA Jensen reviewed 5 of 5 staff files and observed 2 of 5 staff files to lack a health screening. Technical assistance was provided. Technical assistance was also provided with a recommendation to treat dietary supplements as a PRN and obtain physician's approval for use. LPA Jensen conducted a random audit of 1 medication and determined the pill count to be off by 1 with no record to account for the discrepancy. Deficiencies were cited pursuant to the California Code of Regulations (CCR) Title 22, Division 6. Failure to correct deficiencies may result in the assessment of civil penalties.
