Licensing program Analyst, Noel Wolf Petersen Arrived unannouced to the facility to conduct a case management related to eviction procedure. LPA met with Administrator Heather McClosky, to explain the purpose of the visit. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Jason Lund arrived unannounced to conduct an one year annual inspection. LPA Lund met with Administrator Ariel Thomas and explained the purpose of the visit. LPA Lund & Heather McClosky, Administrator toured/inspected the facility the common areas, the kitchen, bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents. Wheelchair ramps were observed at the front and back entrances. Furniture and furnishings were sufficient to meet the needs of residents. The facility temperature was 76 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 110 degrees Fahrenheit, which is within the required range of 105 and 120 degrees. The backyard was inspected by LPA Lund and pathways and exits were found to be clear. LPA Lund inspected the first aid supplies, a fully charged and up-to-date fire extinguisher, and working carbon monoxide/smoke detectors. LPA Lund inspected 2-day supply of perishable food and a 7-day supply of nonperishable food. LPA Lund inspected locked closets and cabinets for the storage of medication. LPA Lund inspected locked storage areas for the storage of cleaning solutions and knives as well. LPA Lund reviewed three residents and three staff files and were in compliance. Per California Code of Regulations (CCR's) - Title 22, Division 6, Chapter 8, no deficiencies are being cited. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Renee Campbell arrived unannounced to conduct an annual inspection. LPA Campbell met with Heather McClosky, Administrator and explained the purpose of the visit. As observed by LPA Campbell, there are seven residents currently in the facility. The facility is a single story building with four bedrooms and two bathrooms. It is licensed to serve 6 ambulatory residents and 2 non-ambulatory residents in the northwest bedroom only. The facilities receive referrals state-wide and are not currently contracted with Valley Mountain Regional or Central Valley Regional. LPA Campbell toured the facility and inspected common areas, the kitchen, bedrooms, bathrooms, and backyard areas to ensure there are no safety hazards for residents. Wheelchair ramps were observed at the front and back entrances. Furniture and furnishings were sufficient to meet the needs of residents. The facility temperature was 75 degrees Fahrenheit, which is within the required range of 68 and 85 degrees. The facility's water temperature measured 112 degrees Fahrenheit, which is within the required range of 105 and 120 degrees. Jan 25, 2024Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On 01/25/24, Licensing Program Analyst Renee Campbell made an unannounced visit to the facility at approximately 0900. LPA Campbell met with Heather McClosky, Administrator and stated the purpose of the visit. LPA Campbell toured the facility with the administrator including the living room, bedrooms, bathrooms, outdoor storage and pantry. The facility currently has 8 ambulatory residents. LPA Campbell observed grounds were maintained and paths were clear of debris. Windows and screens were intact. The facility was observed to be sanitary and free of odor. The fire extinguisher was last checked on 01/23/24. Both the carbon monoxide and smoke alarm were tested successfully during today’s visit. The first aid kit was observed with bandages, tweezers and a thermometer. Client medication was observed to be locked in the medcart in the containers they came in and organized by client. The bathroom contained grab bars and the hot water from the faucet was measured at 109 degrees Fahrenheit. The thermostat was measured at 72 degrees Fahrenheit. The facility maintains a 2 day supply of perishable food and a 7 day supply of non-perishable food. There was fresh fruit and vegetables available for residents in care. All knives, toxins and chemicals were observed to be locked and inaccessible to residents in care. LIC 610E, Emergency and Disaster Plan is posted and current. Of the 8 residents, 2 client files were reviewed. Of the 10 staff, 2 staff files were reviewed. Per California Code of Regulations, Title 22, no deficiencies were observed during this visit.
