Licensing Program Analyst (LPA) Bernadette Allen made an unannounced visit to the facility to commence a complaint investigation and deliver findings. LPA identified herself to Marlene Santillanes support staff who was informed of the purpose of the visit. Marlene then called Delcie Meadows-Administrator who arrived about 10:05 AM. Delcie was informed of the purpose of the visit and allegations. The investigation consisted of interview with clients, staff members, records review and observations. During the investigation LPA conducted a random audit of the Medication Administration Records (MARS) and found that staff are administering medications as prescribed by the physicians. LPA observed staff ensuring that medications are swallowed at the time given. Interviews with both clients and staff indicated that clients are receiving assistance with their hygiene needs and are not left soiled for extended periods. Licensing Program Bernadette Allen conducted an unannounced visit to the facility to commence a complaint investigation. LPA Allen was granted entrance by acting administrato Kara Richardson. At 12:45 PM the director Niaz Khalid arrived LPA Allen identified herself and discussed the elements of the visit regarding the above allegations. The allegation #1 states that the facility did not properly appraise residents and allegation #2 medical documents were falsified. LPA interviewed (3) staff members (3) residents and reviewed (6) six residents files all of which were confirmed that files are in compliance. LPA identified physicians’ orders and records which were also in compliance.LPA was unable to corroborate the above allegations.LPA has determined that the allegations above are Unsubstantiated. Licensing Program Analyst (LPA) Christine Le conducted an unannounced visit to the facility to investigate the above allegations. LPA met with administrator Ginnie Bauer. LPA conducted interviews and reviewed files. The allegations indicate that Resident 1 (R1) is not allowed phone calls or visitors due to the resident's responsible party. During the file review and interviews, LPA was informed that R1 is conserved. R1 was in the process of obtaining legal counsel for an ongoing situation and requested to not have contact with specific individuals. During the interviews, LPA was informed that in general R1 is allowed to have phone calls and/or visitors. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated at this time. LPA conducted interviews and reviewed facility files. The first allegation indicates that Resident 1 (R1) and Resident 2 (R2) are being over-medicated with Medication 1 (M1). During the interviews and file review, LPA was informed that M1 was discontinued for R1. For R2, LPA was informed that there was a misunderstanding with staff members where it appeared R2 was not receiving M1 as prescribed, but upon further review, LPA observed that M1 was being administered correctly. LPA reviewed the residents' MARS log and observed that the medications are being administered as prescribed. The second allegation indicates that staff are not qualified to administer medications.
