Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to open a complaint investigation. LPA met with Licensee, Lindsey Lamerson and explained the purpose of today's visit. Regarding the allegation that resident sustained bruises and fractures due to physical abuse, interviews and records reviewed did not reveal evidence that facility staff physically abused the resident. Medical records documented the resident had multiple chronic medical conditions, mobility impairment, chronic knee and hip pain, edema, fall risk, use of blood thinners, and a history of bruising easily. Records further documented prior falls, fractures, weakness, and the resident’s need for assistance with ambulation and transfers. Home health documentation reflected bruising concerns were evaluated and monitored, including recommendations to pad bedrails and wheelchair components to reduce accidental bruising. LPA reviewed multiple over the counter PRN medication administration records maintained by the facility for the resident. Based on facility records reviewed the facility staff did administer resident’s medication as prescribed. On 02/07/2022, Licensing Program Analyst (LPA) Walton arrived unannounced to deliver findings on the above allegation. LPA introduced self, stated the purpose of the visit and reqeusted to meet with the Administrator. Administrator was not present upon LPAs arrival, LPA contacted Administrator via telephone. Administrator gave verbal permission to meet with Caregiver. LPA met with Caregiver, Estella Mendoza. On 01/10/2022, Administrator was notified that LPAs, Ombudsman, Hospice nurses, Physical Therapist, and Occupational Therapists are exempt from testing for COVID-19 on site, providing test results and/or providing documentation of proof of vaccination prior to entry as they are considered essential visitors. On the same date, Administrator updated facility policies to reflect the new information.
