Type of visit: Required - 1 Year
On 12/22/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Elizabeth Gonzalez. LPA was granted entry. 6 residents were present during inspection. Administrator (A1) Doderlein Anaya Fernandez was present during LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 38 degrees F. Fire extinguisher was observed with a purchase date of: 11/24/25. Fire drill last completed on 10/2/25. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature at 109.9 degrees F. in 1 bathroom. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. First aide kit observed with all of the required items. All residents and samples of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 1/5/26: Lic 308, Lic 500, Lic 610E, Current Liability Insurance and current Administrator’s certificate. Type of visit: Required - 1 Year
On 12/26/24 Licensing Program Analyst (LPA) M Vega entered the facility. LPA arrived at the facility unannounced to conduct an unannounced annual inspection. LPA met with Assistant Administrator Kirstenkay Kaye Yu and also Administrator (AD) Doderlein Anaya Fernandez. Facility has 4 bedrooms and 2.5 bathrooms. The facility has a capacity of 6 and there are currently 3 residents living at the facility. Facility has live in staff at the facility. 1 of the 3 residents are in a share a room and the other 2 residents have a private room each. Upon entering the facility LPA observed residents 2 residents in common area watching TV, 1 resident with their family for the holidays. The facility was observed to be at a comfortable temperature, of 75 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. Department phone number and infection prevention information signs were posted in facility. LPA observed residents rooms to be properly furnished. LPA toured the facility inside and out. Facility is clean, odor free, and clear from obstruction. LPA observed knives/sharps, disinfectants, and chemicals to be locked and inaccessible to residents. Kitchen LPA observed the required 7 day supply of non-perishable food and 2 day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. LPA observed water temperature in a main resident bathroom and read at 107.4 degrees F. LPA observed fire extinguisher to be services 11/22/24 and in good standing. Smoke detector and carbon monoxide reader was tested and is in good standing. LPA toured facility with Administrator outside. LPA observed no external bodies of water. Outdoor seating area was observed in good repair and available to residents. Continuation on LIC 809C LPA observed resident's medications. LPA observed resident files and staff files complete. Resident files observed to have updated information. An emergency disaster plan was observed. No deficiencies were observed and cited. Exit interview conducted. Allegation investigated: Staff left resident in a soiled diaper for an extended time. Staff did not ensure resident had privacy.
On 9/9/2024 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to deliver the findings for the allegations listed above. LPA introduced herself and explained the reason for the visit. LPA met with Administrator (AD) Doderlein Anaya Fernandez. 1. The Department investigated the allegation: Staff left resident in a soiled diaper for an extended time. LPA conducted multiple interviews. During the interviews LPA was informed R1 was found on 4/10/24 with soiled brief and clothes, R1 also had bowel movement (BM) and was double briefed. On 4/11/24 R1 was found with dried BM & double briefed. LPA observed pictures provided which showed R1 with bowel movement going through the first brief to the second brief and with BM stuck to the buttocks of R1. Substantiated 2. The Department investigated the allegation: Staff did not ensure resident had privacy. LPA was informed during multiple interviews the administrator’s small child goes in and out of the resident’s rooms. On 4/16/24 LPA observed the small child going in and out of R2’s room during treatment of the roommate. LPA also observed the small child throwing a ball inside the facility which can be hazardous for resident’s using a walker, cane, or unbalanced. Based on LPAs observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 6 & Chapter number 8, are being cited on the attached LIC 9099D. LPA introduced herself and explained the reason for the visit. LPA met with Administrator (AD) Doderlein Anaya Fernandez. 1. The Department investigated the allegation: Staff did not ensure that resident's hygiene needs were met while in care. During the interviews LPA was informed and pictures were provided showing R1 to have bowel movement (BM) dried to the buttock area. Interviewees stated when they saw R1 they smelt of BM and urine. Based on LPAs observations, interviews, which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. On 9/9/2024 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to deliver the findings for the allegations listed above. LPA introduced herself and explained the reason for the visit. LPA met with Administrator (AD) Doderlein Anaya Fernandez. 1. The Department investigated the allegation: Staff did not prevent resident from harming themself while in care. Multiple interviews were conducted. LPA was not able to verify facility did not prevent resident from harming themself. LPA observed bedrooms with double beds have the beds against one side of the wall. 2. The Department investigated the allegation: Staff did not safeguard resident's personal possessions. LPA conducted interviews and reviewed records. LPA observed a signed and dated "Discharge" form by R1's family indicating R1's personal possessions were returned when R1 left the facility. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. Type of visit: Case Management - Other
On 8/9/2024 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management visit. LPA introduced herself and explained the reason for the visit. LPA met with Administrator Doderlein Anaya Fernandez. On 9/5/2024 at 3:44 pm LPA requested AD to provide an explanation for the bruising on R1's right arm. On 9/5/2024 LPA searched for the incident reports regarding R1's bruises but was unable to locate. AD responded on 9/6/2024 at 8:44 am stating incident reports were previously submitted via fax. LPA did not observed a fax confirmation on the incident reports included in the email. The incident reports included on 9/6/2024 email indicated the family was informed of the bruising verbally. According to Title 22, Division 6, Chapter 8 the Dept. and responsible party should be notified in writing within 7 days of the occurrence. AD stated verbal notice was given to family members.. 87211 Reporting Requirements (a) Each licensee shall furnish to the licensing agency such reports as the Department may require, including, but not limited to, the following: (1) A written report shall be submitted to the licensing agency and to the person responsible for the resident within seven days of the occurrence of any of the events specified in (A) through (D) below. This report shall include the resident's name, age, sex and date of admission; date and nature of event; attending physician's name, findings, and treatment, if any; and disposition of the case. Type of visit: Required - 1 Year
On 12/15/23 at 11:58 a.m. Licensing Program Analyst (LPA) B. Miranda entered the facility. LPA arrived at the facility unannounced to conduct an unannounced annual inspection. LPA met with Administrator (AD) D. Anaya Fernandez. Upon entering the facility LPA observed 2 residents in the living room watching TV and 1 resident in their room. Facility has 4 bedrooms and 2.5 bathrooms available for residents. The facility has a capacity of 6 and there are currently 3 residents living at the facility. Facility has live in staff at the facility. 2 of the 3 residents share a room and the 3rd resident has a private room. LPA observed staff and residents interacting with one another. LPA toured the facility inside and out. Facility is clean, odor free, and clear from obstruction. LPA observed knives/sharps, disinfectants, and chemicals to be locked and inaccessible to residents. LPA observed exits to be free from obstruction. Water temperature was check in a bathroom and read at 111.6 degrees F. LPA observed fire extinguisher to be services 11/29/23 and in good standing. Smoke detector and carbon monoxide reader was tested and is in good standing. LPA observed residents rooms to be properly furnished. LPA observed resident files and a sample of staff files. LPA observed resident's medications. No citations were issued at this time.
