On 08/05/2026, Licensing Program Analyst (LPA) V Gorban arrived at the facility unannounced to conduct a required unannounced Annual Inspection visit. LPA introduced self, stated purpose of visit, and was allowed entrance by staff Isaiah Gordon. Administrator Krystyl Ibanez, certification number 7035602740 and expiration date 10/01/2027 was contacted and notified of Licensing visit. LPA toured the facility inside and out conducting health and safety checks. The facility was observed to be at a comfortable temperature of 76 degrees, clean, in good repair, and no passageway obstructions or fire hazards observed. Fire extinguisher was observed with a service date of 04/22/2026. Last fire drill recorded in July 9th, 2026. LPA observed residents after lunch in common area, others in rooms resting. Kitchen was toured and observed functional appliances, with refrigerator reading of 3 degrees F and freezer with -18 degrees F. The facility observed with substantial amount of perishable and nonperishable food stored. Medications are stored in a locked cabinet in the kitchen. Hot water temperature tested at 118 degrees F. LPA observed securely fastened grab bar and non-skid mat in shower area. Facility has a capacity of 6 with a current census of 5. Smoke alarms are in working condition. Carbon monoxide detector was tested and in working condition. Water temperature was checked in the kitchen and read at 116 degrees Fahrenheit. LPA was not able to review disaster, infection control plan, and staff and residents files. LPA provided staff with annual fees billing and PIN. On 07/22/2025, Licensing Program Analyst (LPA) V Gorban arrived at the facility unannounced to conduct Required Annual Inspection. LPA met with Administrator (AD) Krystyl Sheen Ibanez, certification number 6053811740 and expiration date 10/01/2025. LPA conducted tour inside and out of facility with AD. Residents observed at the facility after lunch time. The facility was observed to be at a comfortable temperature of 76 degrees, clean, in good repair, and no passageway obstructions or fire hazards observed. Fire extinguisher was observed with a service date of 03/28/2024. Last fire drill recorded on 6/1/25. Dining area and Kitchen were toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in freezer, refrigerator, and pantry. Refrigerator and freezer observed operational as required. LPA toured resident bedrooms. Residents' rooms were toured and observed with adequately furnished with bed, dresser, and adequate lighting. Hot water temperature tested at 111 degrees F. LPA observed securely fastened grab bar and non-skid mat in shower area. Medications were stored in a locked medication room. Medications records were reviewed. First Aid Kit was stored in medication room and observed with all required items. LPA toured laundry room and garage. LPA observed chemicals were stored and locked in the garage for staff use only. Facility courtyard was toured and observed to be free from debris. There was outdoor seating available for the residents. LPA reviewed facility Plan of Operation, Infection Control Plan, Emergency Disaster Plan and number of staff and residents files requested and reviewed during the inspection visit. On 7/23/24, Licensing Program Analysts (LPAs) B. Miranda & M. Vega arrived at the facility unannounced to conduct a required unannounced Annual Inspection visit. LPAs introduced themselves, stated purpose of visit, and was allowed entrance by staff. Administrator Krystyl Ibanez was contacted and arrived later. Upon arrival LPAs observed S1 who was not properly associated to the facility to be working with residents, Administrator stated S1 has been rehired as of 7/6/24. LPAs toured the facility inside and out including to include kitchen, dining, living room, bedrooms, bathrooms, and exterior. LPAs observed one outside exit from the back had items on the ground and a plant covers half of the walkway. R1's exit from the room is obstructed by a chair and fan, and screen door needs to be fixed. Medications are stored in a locked cabinet in the kitchen. LPAs observed hygiene products to be accessible to residents in care, R2's physician report states the products should not be accessible to resident. LPAs observed knives and sharp objects to be secured. LPAs observed expired sandwich meat and food items not properly labeled in the refrigerator. LPAs observed dishwasher to have an odor and mildew buildup. Staff stated dishwasher is not used except to air dry clean dishes. LPA also observed dishwasher pods unlocked and accessible to residents. Facility has a capacity of 6 with a current census of 5. Smoke alarms are in working condition. Carbon monoxide detector was tested and in working condition. Water temperature was checked in the kitchen and read at 114.8 degrees Fahrenheit. LPA was not able to review current infection control plan and staff files were not current. LPAs did not observe S1's file at the facility. Exit interview was conducted and a copy of this report LIC809, LIC809D, and appeal rights were provided to Staff Elisa Cervera.
