On Tuesday, 12/10/2024, Licensing Program Analyst, (LPA) Raymond Comer, conducted the initial10-day complaint visit at the facility. At 10:05 am, LPA met with facility representative, Saul Aranda, and the purpose of the visit was disclosed. A physical plant tour of the facility was conducted. No health and safety issues were observed. Allegation: Licensee is not addressing pests at the facility- The Reporting Party (RP) alleges that faciity is not addressing incidents of bed bugs and roaches observed in resident bedrooms. Licensing Program Analyst (LPA) Tihesha Smith conducted an unannounced complaint investigation visit to investigate the above allegation. LPA Smith met with Alexis Peralta (Human Resources Director) and disclosed the purpose of the visit. Staff yells at residents To investigate this allegation LPA requested facility documents,and conducted interviews with staff and resident between 11:05 -1pm. LPA interviews with six (6) of six (6) staff revealed they have not yelled at residents or witness any staff yelling at residents in care. Staff #4 (S4) and Staff # 6 (S6) revealed staff and residents have a bound of respect. Staff #2 (S2) revealed have yelled and talk firmly to a vendor that had disrespected residents in the past but have not yelled at the residents. LPA met with lead staff, Alexcis Peralta, and explained the reason for the visit. --- Facility staff are restraining clients. It was alleged that staff restrain residents using a four-pin restraint mechanism. To investigate the allegation on 11/15/2023, LPA Duguma made observations during the physical plant tour, interviewed three (03) staff from 11:00 AM - 12:00 PM and six (06) residents from 1:00 PM – 2:30 PM. Allegation investigated: Due to inadequate supervision resident wander away from the facility Kitchen hood exhaust is in disrepair.

Licensing Program Analyst (LPA) Rosaura Valenzuela conducted an unannounced complaint investigation visit for the above noted allegations. LPA Valenzuela met with Human Resources Director, Alexis Peralta and explained the reason for the visit. It was reported that due to inadequate supervision resident wander away from the facility. It was alleged that a male resident in their late thirties was wandering away from the facility at night. To investigate this allegation on 10/17/2023 between 3:45pm and 4:00pm, staff interviews were initiated. Interviews revealed that a resident with dementia walked out of the main building and into the patio. Staff found resident #1 (R1) on the facility premises, in the patio area. In the other adjacent facility there are no residents present who are under 65 years of age. In addition LPA conducted a physical plant tour between 12:30pm and 1:00pm and made a note that the facility has alarms and camaras around the facility premises. Licensing Program Analyst (LPA) Rosaura Valenzuela conducted an unannounced complaint investigation visit for the above noted allegation. LPA Valenzuela met with Human Resources Director, Alexis Peralta and explained the reason for the visit. It was reported that staff did not ensure facility was free from bed bugs. To investigate this allegation on 10/17/2023 between 12:00pm and 12:30pm, staff interviews were initiated. Interviews revealed that facility does have a bed bud investation in room 221A. In order to get rid of the bed bugs the facility replaced all the furniture including the mattresses, and washed all the clothes and linens in the shared room, but the bed bugs continued to be presesnt. Between 12:30pm and 01:00pm, LPA toured the facility and went inside room 221A. LPA did observe bed bugs on the walls and one bug on the bed. Staff told LPA that a fumigating company was scheduled to come today to provide an estimate. Upon arrival LPA LaQueena Lacy met staff Alexcis Peralta and explained the purpose of this visit. #1. Resident was left in soiled diapers for an extended amount of time. It is alleged that residents are left in soiled diapers. To investigate the above allegation LPA conducted interviews with staff and residents on 01/11/2023 at approximately 2:15pm- 3:50pm. Four out of six residents interviewed are incontinent. Interviews with three (03) out of four (04) incontinent residents confirm they do not wait for long periods of time when needing assistance with a diaper change, and staff comes during the day and night to check if they need to be changed. LPA Lacy met with staff Alexcis Peralta and explained the purpose of the visit. It is alleged that resident #1 (R1) was sexually abused by staff #1 (S1). S1 threw R1 to the floor and got on top of them. To investigate the above allegation LPA requested documents relevant to the investigation on 08/26/2022. Interviews conducted with residents on 08/26/2022 between 3:00pm- 3:45pm revealed that five (05) out of (05) residents have not witnessed or been told by another resident that R1 have been sexually abused by staff. Residents stated that they have not witnessed staff hit any residents in care and staff treat them well and assist them when they need help. Licensing Program Analyst (LPA) LaQueena Lacy conducted an unannounced subsequent complaint visit to deliver findings on 05/17/2023. Upon arrival LPA LaQueena Lacy met staff Alexcis Peralta and explained the purpose of this visit. #1. Facility has insufficient staffing for the residents. It is alleged that after 10pm there were no staff at the facility to watch the residents. To investigate the above allegation, on 08/10/2022 LPA requested documents relevant to the investigation at 3:38pm. LPA interviewed staff and resident on 08/10/2022 at approximately 2:18pm and 01/11/2023 at approximately 2:15pm- 3:50pm. Interviews with six (06) out of six (06) residents confirm that at least two (02) staff members work at night, and staff is available when they need assistance. Licensing Program Analyst (LPA) LaQueena Lacy conducted an unannounced subsequent complaint visit to deliver findings on 05/17/2023. Upon arrival LPA LaQueena Lacy met staff Alexcis Peralta and explained the purpose of this visit. It is alleged that witness #1 (W1) witnessed R1 being sexually abused by a staff member at the facility. To investigate the above allegation, on 01/11/2023 LPA requested documents relevant to the investigation at 12:30pm. LPA began interviews with staff and residents at approximately 2:15pm- 3:50pm and additional interviews with staff and residents on 01/13/2023 at approximately 11:45am. Interviews with five (05) out of (05) resident revealed they have not witnessed any staff members sexually assaulting or touching any residents inappropriately. On 04/05/2023 at 10:10 a.m. Licensing Program Representative (LPA) Evelin Rios conducted an unannounced complaint investigation at the location mentioned above. LPA was greeted by Staff #1 (S1) and granted access. Alexcis Peralta met LPA in the Lobby. LPA explained the reason for the visit. Administrator Kandice was not available for todays visit and designated Alexcis to sign this report. At 10:20 a.m. a physical plant tour was conducted to ensure no immediate health and safety issues were present. No immediate health and safety issues were noted. Allegation: Facility is not providing adequate supervision to residents in care. It is alleged that due to a lack of supervision a resident from the facility left the facility unsupervised and exposed themselves and masturbated in public. To investigate the above allegation LPA conducted interviews with Alexics, and 3 staff from 10:45 a.m. to 11:34 a.m. From 11:35 a.m. to 12:00 p.m. LPA interviewed 6 out 59 residents. Licensing Program Analyst (LPA) LaQueena Lacy conducted an unannounced ten (10) day complaint visit to the facility to investigate the above allegations. LPA met with staff Alexcis Peralta and explained the purpose for the visit. This agency has investigated the complaint alleging that staff #1 (S1) was financially abusing R1, and did not accord R1 privacy while on the phone. During the investigation it was determined that R1 does not live at Melrose Villas. R1 resides currently at the sister facility Melrose Chateau. We have found that the complaint was without a reasonable basis. We have therefore dismissed the complaint. No health and safety hazard is noted during this visit. Licensing Program Analyst (LPA) LaQueena Lacy conducted an unannounced subsequent complaint visit to complete an investigation of the above noted allegations. Upon arrival LPA Lacy met staff Alexcis Peralta and explained the purpose of this visit. It is alleged that the facility smells like urine and poop. Upon arrival in the facility, LPA could smell a strong order of urine throughout the first floor of the facility. During this visit LPA conducted a physical plant tour at 10:56am. While conducting a physical plant tour, LPA questioned staff #1 (S1) regarding the odor in the facility. S1 responded, “I’ve observed staff cleaning resident’s rooms and changing the linens, I know they are cleaning”. The mattresses have been changed and plastic placed on them. During the physical plant tour, LPA observed the second floor to be free from odor of urine. During the investigation S1 informed LPA the first floor is mainly housing incontinent residents. Licensing Program Analyst (LPA) LaQueena Lacy conducted a subsequent complaint visit for the above allegation on 04/18/2022 at 11:10am for the purpose of delivering investigative findings. LPA Lacy met with Alexcis Peralta and explained the purpose of the visit. LPA Lacy conducted a physical plant tour at 11:21am, no immediate health and safety concerns observed. It is alleged that resident #1 (R1) was involuntarily moved to another facility. To investigate the above allegation, LPA conducted an initial complaint visit on 10/27/2021. LPA requested documents relevant to the investigation at 10:39am and began interviews with the Administrator and staff at approximately 10:41am. Interviews revealed that R1 was transferred from the facility due to exhibiting uncontrollable behaviors. Administrator indicated that R1 was placed in the facility by a placement agency operating under the Department of Health Services (DHS). Licensing Program Analyst (LPA) Gary Tan conducted an unannounced subsequent complaint visit to this facility to deliver the findings for the above allegation. LPA met with Assistant Administrator Alexis Peralta and explained the reason for the visit. On 04/20/21, then LPA Margaryan conducted the initial complaint visit. During her visit, LPA Margaryan conducted physical plant tour at 11:30 AM, interviewed administrator and staff between 9:15 AM to 10:30 AM, requested facility documents at 11:00 AM. It was alleged that the resident #1 (R1) was admitted to the hospital where R1 was observed to have four (4) unstageable pressure injuries. The injuries were located on R1’s lower back, buttock, hip and shoulder.
