Type of visit: Case Management - Health Checks
At approximately 2:00 p.m. on 04/08/26 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit. LPA met with the assistant administrator and disclosed the reason for the visit. Today’s case management visit was conducted to ensure the health and safety of all residents. LPA toured the facility inside and out at 2:05 p.m. today, attempted interviews with two (02) out of two (02) residents between 2:15 p.m. and 2:35 p.m., and requested pertinent records, including but not limited to admission agreements, medical assessments, care plans, and staff and client rosters at 3:00 p.m. No immediate health or safety concerns were observed during today’s visit. On 12/01/25, at 11:01am, Licensing Program Analyst (LPA) Gina Saucedo conducted an unannounced case management visit at this facility for the purpose to ensure Health and Safety. LPA met with Kevin Moscoso, Scheduling Coordinator and explained the purpose of the visit. 1. LPA conducted a physical tour-living/dining room, resident rooms, bathrooms, kitchen area checked facility temperature which was set at 74 degrees. 2. There are cameras in the common areas of the facility. 3. There was two (2) staff providing care and supervision. 4. Some residents were on one (1) side of the facility sleeping and the others were around the dining hall waiting for lunch. No immediate health and safety issues observed. Type of visit: Case Management - Deficiencies
Licensing Program Analyst (LPA) Raymond Comer conducted a Case Management (CM) visit to the facility to follow up on a facility building alteration made by the licensee. LPA met with Los Angeles Fire Inspector, Herbert Reddick, facility administrator Brain Robles, and advised Administrator of the visit. Today's CM visit consisted of a physical plant inspection and Inspector Reddick's assessment of the altered interior wall. Community Care Licensing-Woodland Hills Office received information that licensee made an un-permitted alteration to the interior wall separating Facility#197607979 (licensed for a capacity of 4; current census is 2) and Facility#197607055 (licensed for a capacity of 5; current census is 3) Deficiency cited; LPA requested for the licensee to submit the following in order to comply: · Los Angeles Department of Building and Safety (LADBS) permit allowing the wall's alteration, while maintaining the proper fire resistive rating, OR, to have the wall restored to its original condition at the time of that the facility fire clearance (STD 850) was approved and issued. There are cameras in the common areas of the facility. 3. There was two (2) staff providing care and supervision. 4. The last emergency evacuation drill was conducted on 08/28/25 the Facility Safety Assessment was also conducted on 08/28/25. 2. LPA conducted a physical tour-living/dining room, resident rooms, bathrooms, kitchen area checked facility temperature which was set at 68 degrees and two (2) bathroom's water temperatures within 116 and 120 degrees. 3. Type of visit: Required - 1 Year
On 08/22/25, 8:30 am, Licensing Program Analyst, (LPA) Raymond Comer, conducted an unannounced annual inspection visit at the facility. LPA met with the Administrator, and reason for the visit was discussed. Facility is licensed as a single-story residence, fire clearance for four (4) non-ambulatory. Hospice waiver for four (4). Facility has two (2) total bedrooms, and two (2) bathrooms. LPA conducted a tour of the physical plant with the Administrator and observed the following: Physical plant was inspected for cleanliness and condition. Facility’s main door is the primary entry/exit access. Screening area is located immediately upon entrance. Auditory alarm sensors are present on all exits. Visitor Sign-in sheet, hand sanitizer, gloves and masks are available. Room temperature is comfortable; wall thermostat displays a setting of 73.0°F., within the required range. An approved Mitigation and Infection Control plan is on file. Hand washing, coughing etiquette, and other necessary signage are prominently displayed throughout the facility. Required postings observed to be current. Disaster drills were last conducted: July 2025. KITCHEN area is clean and clear of clutter. LPA observed refrigerator, microwave, stove/oven, dishwasher and sink to be operational. Knives/Sharps are stored in a locked top kitchen drawer inaccessible to residents. A two-day supply of perishable food was observed as properly stored and labeled. A seven-day supply of nonperishable food is stored and available to residents. Dish Soap, cleaning solutions, and toxins are stored in a locked shed near the kitchen. [LIC 809C-continued]--- Fire Detection/Protection system is present in the facility. Multiple dual smoke alarm/carbon monoxide detectors are installed, hardwired, and interconnected. Detectors were tested and function properly. LPA observed two (2) fire extinguishers located at front and rear exits. Extinguishers display a full charge; Service date: 01/07/2025. Medications are stored in a secured medication cart near the kitchen area and are inaccessible to residents. Medications are listed on a centrally stored medication and destruction record log. A First Aid kit is complete and stored inside the living room cabinet. Laundry: LPA observed laundry area, located in room adjacent to the kitchen. Laundry soaps, and other cleaning agents, are stored and inaccessible to residents. Hallway area storage cabinets contain fresh towels, linens and blankets, sufficient for residents. Commons: LPA observed living room, and dining area, finding them clean, organized, and clear of obstruction. Furnishings observed to be in good condition and provide adequate seating for residents. Activities were observed stored in living room cabinet containing arts and crafts, board games, puzzles etc. Bedrooms are observed as clean with sufficient lighting, properly furnished with sufficient closet space, bedding, linens, at least one chair, and night stand. Bathrooms were observed to be clean and sanitary with necessary personal hygiene supplies and required safety fixtures (grab bars, anti-slip floor striping). Hot water temperature measured at 116°F. Within the required range. Towels and washcloths are not shared. Garage is detached from the house and observed to be inaccessible to residents. Garage is also used as storage for emergency water and PPE supplies. [LIC 809C-continued]--- Outdoor (backyard) area observed to have a shaded patio, with tables and seating sufficient for residents and visitors. Outdoor furniture was observed to be in good condition. There are no bodies of water in the facility. Resident records are stored in staff office and inaccessible to residents. Records were reviewed for current IPP and/or Needs and Services plans, physician report, and admission agreements. Resident records appeared to be complete and current. Staff records are stored in staff office and inaccessible to residents. Staff files contain criminal record clearances and are associated to this facility. Staff records appear to be complete and current. There were no health and safety hazards observed during the day of inspection. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Abeye Duguma met with Arman Ahangarzadeh for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at around 9:30 AM and the following was noted: There is one entrance being utilized at the facility. The facility has a total of two (02) bedrooms and two (02) bathrooms. The facility is fire cleared for four (04) non-ambulatory. The facility is currently occupying one (01) residents. The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. Laundry detergents, cleaning agents and other toxins are locked away. Kitchen is sufficiently stocked with at least two (02) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. The living and dining room are neat and clean. The facility maintains a comfortable temperature at 76°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located in the kitchen, observed to be fully charged and last inspected on 02/01/2024. (continued on LIC 809-C) The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. Residents have enough personal hygiene product provided by the licensee. The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 110.7°F. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. LPA observed medication to be locked and inaccessible to residents. Facility maintains a complete first aid kit. No health and safety hazards noted during the visit. Exit interview conducted. At 12:30 p.m. on 02/01/2024, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced complaint visit. LPA met with Administrator and disclosed the reason for the visit. To investigate the allegation above, LPA conducted an initial visit on 08/24/2023 and toured the facility inside and out at 12:15 p.m. and conducted a record review at 1:00 p.m. including but not limited to the resident list, staff list, admission agreements, physician reports, care plans, and care notes. LPA interviewed the administrator at 12:30 p.m. on 11/16/23, Staff #1 (S1) at 1:00 p.m. on 11/16/23, Staff # (S2) at 4:00 p.m. on 01/23/24, Staff #3 (S3) at 4:15 p.m. on 01/23/24, and Staff #4 (S4) at 2:30 p.m. on 01/29/24. LPA conducted a subsequent visit today and conducted a medication review at 12:50 p.m., interviewed Resident #3 (R3) at 1:10 p.m., and Staff #5 (S5) at 1:25 p.m., and toured the facility at 1:15 p.m. Unsubstantiated ***This page was amended to include additional pertinent information *** - LPA Reed, 10:20 a.m., 06/21/24 Regarding the allegation “Resident sustained unexplained bruises while in care” it was alleged that staff did not know how Resident #1 (R1) had gotten bruises on their chin, abdomen, and arms. Also, R1 stated someone was attacking them. R1 was unavailable for interview. Interview with the administrator revealed they were not aware of any bruises on R1, and R1 occasionally spoke incoherently. No instances of R1 being attacked were reported to the administrator. Interview with S1 revealed R1 had some difficulty with mobility, and staff supported R1’s needs for mobility assistance by following R1 when they are outside of their room. Injuries on R1’s chin may have been resulted from a fall. However, no recent falls were reported to S1. Three (03) out of the three (03) other staff interviewed were not aware of any reports of R1 being attacked or any bruising. Record review revealed that R1 was prone to wandering, confusion, and some visual impairment. R1’s care plan noted they were a low fall risk. Internal daily log for R1 did not reveal any information to verify the allegation. There were no reports of bruising on R1’s chin, abdomen or arms or any reports of R1 being attacked. Based on interviews and record review, R1 may have suffered from fall resulting bruises. However, there is not sufficient information to verify the allegation. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. Regarding the allegation “Staff do not ensure facility is clean” it was alleged the bathroom had feces on the wall and the floors were covered in trash. Facility tours on 08/24/2023 and today revealed no signs of trash or feces in the facility. The resident bathroom was sanitary, and no hazards were observed. S2, S3, and S4 all confirmed that they were not aware of any feces in bathrooms or trash on floors. S2 stated when staff are not attending to residents, they are trained to maintain the facility in an orderly manner. Staff clean unsanitary conditions “right away”. R3 stated the facility is clean to their liking and they have not seen feces in the bathroom. LPA observed staff sweeping the main entrance around 12:45 p.m. today. Based on interviews and observations, the staff have ensured the facility was clean. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. Regarding the allegation “Staff left resident soiled for an extended period of time” it was alleged staff do not change Resident #2 (R2) enough which led to multiple Urinary Tract Infections (UTIs). Record review revealed R2 experienced bowel and bladder impairment. R2’s care plan stated they required “extensive” and “total” assistance with toileting. R2’s care notes revealed staff routinely assisted with showering, grooming, and toileting. S1 stated R2’s UTIs may have been caused by their lack of hydration as R2 often refused staff assistance. S2 and S4 noted R2 was never left in a soiled diaper, and they checked R2’s diapers every 2 hours and changed them if needed. Based on interviews and record review, staff did not leave R2 in soiled diapers. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. Regarding the allegation “Staff overmedicated residents” it was alleged Staff #5 (S5) was overheard claiming they overmedicate residents. Interview with S5 revealed they were a nurse but rarely assist with medications at the facility. They have never overmedicated a resident or mismanaged medication. Interview with the administrator and four (04) out of four (04) other staff revealed they had never heard S5 say anything regarding overmedicating residents. S3 stated S5 trained them on medication procedures and was a credentialed nurse. S4 confirmed S5 was a Certified Nursing Assistant (CNA) and was “on top of it” and “the best”. R3 stated they have never been overmedicated or had any medication issues. Based on interviews, S5 has not overmedicated residents. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. Regarding the allegation “Staff are mismanaging residents’ medication” it was alleged some Norco medications were missing. LPA and the administrator completed a medication review and noted all Norco medications were accounted for. Interviews with the administrator and five (05) out of five (05) staff revealed they were unaware of any missing medications. S3 stated they had never assisted with Norcos and staff are trained to have hospice nurses or a supervisor assist with Norco medications. Based on interviews and observations, staff are not mismanaging resident medications. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. No immediate health and safety hazards were observed during this visit. Exit interview conducted. Appeal rights discussed.
