At approximately 1:30 p.m. on 04/08/26 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit. LPA met with the assistant administrator and disclosed the reason for the visit. Today’s case management visit was conducted to ensure the health and safety of all residents. LPA toured the facility inside and out at 1:35 p.m. today, attempted interviews with three (03)-out of three (03) residents between 1:40 p.m. and 2:00 p.m., and requested pertinent records, including but not limited to admission agreements, medical assessments, care plans, and staff and client rosters at 3:00 p.m. No immediate health or safety concerns were observed during today’s visit. Type of visit: Required - 1 Year

On 08/21/25, 10:30 am, Licensing Program Analyst, (LPA) Raymond Comer, conducted an unannounced annual inspection visit at the facility. LPA met with the Administrator, and reason for the visit was discussed. Facility is licensed as a single-story residence, fire clearance for five (5) non-ambulatory. Hospice waiver for five (5). Facility has three (3) total bedrooms, and two (2) bathrooms. LPA conducted a tour of the physical plant with the Administrator and observed the following: Physical plant was inspected for cleanliness and condition. Facility’s main door is the primary entry/exit access. Screening area is located immediately upon entrance. Auditory alarm sensors are present on all exits. Visitor Sign-in sheet, hand sanitizer, gloves and masks are available. Room temperature is comfortable; wall thermostat displays a setting of 72.0°F., within the required range. An approved Mitigation and Infection Control plan is on file. Hand washing, coughing etiquette, and other necessary signage are prominently displayed throughout the facility. Required postings observed to be current. Disaster drills were last conducted: July 2025. KITCHEN area is clean and clear of clutter. LPA observed refrigerator, microwave, stove/oven, dishwasher and sink to be operational. Knives/Sharps are stored in a locked top kitchen drawer inaccessible to residents. A two-day supply of perishable food was observed as properly stored and labeled. A seven-day supply of nonperishable food is stored and available to residents. Dish Soap, cleaning solutions, and toxins are stored in a locked shed near the kitchen. [LIC 809C-continued]--- Fire Detection/Protection system is present in the facility. Multiple dual smoke alarm/ carbon monoxide detectors are installed, hardwired, and interconnected. Detectors were tested and function properly. LPA observed two (2) fire extinguishers located at front and rear exits. Extinguishers display a full charge; Service date: 01/07/2025. Medications are stored in a secured medication cart near the kitchen area and are inaccessible to residents. Medications are listed on a centrally stored medication and destruction record log. A First Aid kit is complete and stored inside the living room cabinet. Laundry: LPA observed laundry area, located in room adjacent to the kitchen. Laundry soaps, and other cleaning agents, are stored and inaccessible to residents. Hallway area storage cabinets contain fresh towels, linens and blankets, sufficient for residents. Commons: LPA observed living room, and dining area, finding them clean, organized, and clear of obstruction. Furnishings observed to be in good condition and provide adequate seating for residents. Activities were observed stored in living room cabinet, with contains arts and crafts, board games, puzzles etc. Bedrooms are observed as clean with sufficient lighting, properly furnished with sufficient closet space, bedding, linens, at least one chair, and night stand. Bathrooms were observed to be clean and sanitary with necessary personal hygiene supplies and required safety fixtures (grab bars, anti-slip floor striping). Hot water temperature measured at 117°F. Within the required range. Towels and washcloths are not shared. Garage is detached from the house and observed to be inaccessible to residents. Garage is also used as storage, extra water and PPE supplies. [LIC 809C-continued]--- Outdoor (backyard) area observed to have a shaded patio, with tables and seating sufficient for residents and visitors. Outdoor furniture was observed to be in good condition. There are no bodies of water in the facility. Resident records are stored in staff office and inaccessible to residents. Records were reviewed for current IPP and/or Needs and Services plans, physician report, and admission agreements. Resident records appeared to be complete and current. Staff records are stored in staff office and inaccessible to residents. Staff files contain criminal record clearances and are associated to this facility. Staff records appear to be complete and current. There were no health and safety hazards observed during the day of inspection. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Abeye Duguma met with Arman Ahangarzadeh for a Required One (01) Year visit. LPA explained the reason for the visit. A tour of the physical plant was conducted at around 1:30 PM and the following was noted: There is one entrance being utilized at the facility. The facility has a total of three (03) bedrooms and two (02) bathrooms. The facility is fire cleared for five (05) non-ambulatory. The facility is currently occupying three (03) residents. The facility has outdoor furniture with a covered shaded area for residents and visitors. The facility does not have a swimming pool/body of water. Laundry detergents, cleaning agents and other toxins are locked away. Kitchen is sufficiently stocked with at least two (02) days perishable and seven (07) days non-perishable food. Frozen foods are wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked and inaccessible to residents. The living and dining room are neat and clean. The facility maintains a comfortable temperature at 76°F. The smoke and carbon monoxide detectors are hardwired, interconnected and observed to be operational. Fire extinguisher is located in the kitchen, observed to be fully charged and last inspected on 02/01/2024. (continued on LIC 809-C) The residents' rooms are adequately furnished with appropriate lighting system. Hallways are well lit. Residents have enough personal hygiene product provided by the licensee. The bathroom was checked for cleanliness and proper operations. The hot water temperature was measured at 111.3°F. Towels and washcloths are not shared. There was enough clean linen available in the cabinets. LPA observed medication to be locked and inaccessible to residents. Facility maintains a complete first aid kit. No health and safety hazards noted during the visit. Exit interview conducted. Allegation investigated: Staff raped resident. Staff are having sexual intercourse during work hours. Facility air conditioning is in disrepair. Staff not providing a comfortable room temperature for residents. Staff not keeping facility free from pests.

Licensing Program Analyst (LPAs) Chris Alemoh and Michael Cava conducted a subsequent complaint visit to the facility to conclude the investigation regarding the above allegations. LPAs met with the administrator, Brian Rosales, and advised him of the complaints. Staff raped resident: In regards to the allegation, the 10 day visit was made by LPA Laqueena Lacey on 08/22/23. Complaint was also accepted by Investigations Branch (IB) Investigator, Juan Lozana, as an assignment. The investigation by both LPA and IB consisted of interviews, record review and a physical plant inspection. On 09/13/23, interviews held with the facility manager and staff do not corroborate with the allegation as no witnesses and dates of this incident could be identified. Interviews were also made with Resident 1 (R1) and Staff 1 (S1), and both deny the allegation. Furthermore, R1 stated the allegation is made up. Unsubstantiated On 09/18/23, IB made contact with the Los Angeles Police Department (LAPD) dispatch. According to dispatch, there were no calls generated to report that R1 had been raped. Based on the information obtained, there was insufficient evidence to prove that S1 raped R1. Therefore, the allegation is deemed Unsubstantiated at this time. Staff are having sexual intercourse during work hours: In regards to the allegation, it was alleged that staff were having sex during their shifts. The complaint received did not identify any staff, witnesses, dates or times. Interviews made with five (5) of five staff deny the allegation. Interview with one (1) of three residents also deny the allegation. The other two (2) residents were non-responsive to the LPA's questions, therefore, could not be interviewed. Based on the information obtained, there was insufficient evidence to corroborate the allegation of staff having sexual intercourse during work hours. Therefore, the allegation is deemed Unsubstantiated at this time. Facility air conditioning is in disrepair/Staff not providing a comfortable room temperature for residents: In regards to the allegation, it was reported that the facility air conditioning has been in disrepair for months, causing the residents to complain of the facility being too hot. It was also reported that administrator refuses to repair the air conditioning unit. Interviews with one (1) of three (3) residents deny the allegation. Resident 1 (R1) states room temperature is always maintained at a comfortable level, and expressed no complaints of the air conditioning being broken. The other two (2) residents were non-responsive to the LPA's questions, therefore, could not be interviewed. Interviews made with the administrator and maintenance staff reveal that the air conditioning unit was never in disrepair. Both stated the air conditioning unit just went under it's usual annual service for maintenance, but the air conditioning wasn't shut down for more than an hour or two. Maintenance was made in the morning to avoid the heat during the day. Air conditioning has been running alright since. During the day's investigation, the room temperature appeared to be maintained at a comfortable level. Based on the information obtained, there was insufficient evidence to corroborate the allegation of the air conditioning being in disrepair, causing room temperatures to be kept at an uncomfortable level. Therefore, the allegation is deemed Unsubstantiated at this time. Staff not keeping facility free from pests: In regards to the allegation, it was reported that the facility is infested with roaches in the living room and resident rooms. During the course of the investigation, LPAs Alemoh and Cava conducted an inspection of the physical plant, which included all five (5) bedrooms, and three and one half (3 1/2) bathroom, common areas, living room and dining room. No roaches were observed. Interview with one (1) of three (3) residents deny ever seeing any roaches. The other two (2) residents were non-responsive to the LPA's questions, therefore, could not be interviewed. Interviews with the administrator and staff also deny the allegation, stating they haven't received any resident complaints or concerns of having roaches. Furthermore, the administrator stated pest control provides service at least twice a month for preventive measures. Based on the information obtained, there was insufficient evidence to prove that staff is not keeping facility free from pests.
