Type of visit: Required - 1 Year
On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a required annual. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. The facility is licensed for six non-ambulatory residents and has a hospice waiver for one. The facility is a single story home with five resident bedrooms, one of which is shared, one staff bedroom, three shared resident bathrooms, a living room, a dining room, a kitchen, a laundry room, a atrium, and an attached two car garage. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors and testing hot water temperature in the bathrooms. The hot water temperature measured between 105.2 degrees Fahrenheit and all smoke detectors were operational. The facility conducted an emergency drill on May 20, 2026. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed secured medication storage in locked cabinet located in dining room area. LPA observed emergency food and water in a closet in the entryway of the facility. LPA reviewed two of two staff training and fingerprint records and conducted a complete review of six out of six client files. LPA confirmed that administrator has a current administrator certificate which expires on May 19, 2027 . Based on observations no deficiencies noted. Jul 16, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On July 16, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. House Manager (HM) Luisa Cruz was present and assisted LPA on today's inspection. LPA observed that Administrator (AD) Anna Mallari has a valid Administrator certificate which expires on December 26, 2025. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for one. The facility is a single story home with five resident bedrooms, one of which is shared, one staff bedroom, three shared resident bathrooms, a living room, a dining room, a kitchen, a laundry room, a atrium, and an attached two car garage. LPA, accompanied by the HM, conducted a tour of the interior portions of the facility. On today's visit, LPA observed six resident in care and two staff present. LPA observed residents preparing to departure for their respective Day Programs. LPA observed the See Something, Say Something poster, (PUB 475) mounted on the wall in the dining room. LPA inspected all five resident bedrooms and observed them to be free of hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway closet. LPA inspected the three shared resident bathrooms and observed them to be clean. Bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 111.3 and 111.5 degrees Fahrenheit. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. CONTINUED ON 809-C LPA observed the facility has a two day perishable and a seven day nonperishable food supply on hand in the kitchen. LPA observed kitchen appliances to be clean and operational. LPA observed the four burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed fire extinguishers to be mounted on the wall in the kitchen and it was observed to be charged and purchased on May 14, 2025. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on May 20, 2025. LPA observed the centrally stored medication to be kept in a locked cabinet located in the dining room. LPA observed the facility has a First Aid Kit stored in a hallway closet and it had all the required components. LPA observed the facility has a three day emergency food and water supply stored in the hallway closet. LPA observed the laundry room to be clear of any hazards. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage. LPA observed chemicals and toxins to be stored in a locked closet in the garage. LPA, accompanied by the HM, conducted a tour of the exterior portion of the facility. The exterior portion was observed to be free of hazards and obstructions. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gate of the facility to be self latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all six resident files. All the required documentation were present and current in the resident file reviewed. The facility is responsible for Personal and Incident expense monies (P&I) for five residents. LPA reviewed the P&I for all five residents. LPA reviewed the residents' medication and medication records. LPA reviewed six staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Dwayne Mason Jr. arrived at the facility unannounced for the purpose of conducting a required annual inspection. LPA was greeted at the facility by Luisa Cruz, House Manager/DSP. LPA met with Administrator Anna Mallari and explained the purpose of the inspection. The facility is a one-story home with five resident bedrooms, three resident bathrooms, kitchen, dining room, living room, laundry room, atrium, caregiver room, backyard and attached two-car garage. Facility appears clean, safe and sanitary. LPA observed the facility has the necessary postings posted on the walls. LPA noted two residents were lounging in the living room and their own bedroom. Other residents were noted as being away at day program. All resident rooms had the required elements, including bed, chair, closet space and ample lighting. Facility has extra linens and hygiene supplies for residents in the hallway closets. Restrooms are stocked with soap and paper towels and have hand washing postings. Hot water measured between 105 and 120 degrees F. LPA observed facility has emergency food and water supply as well as additional emergency supplies. LPA observed the fire extinguisher was last serviced on May 13, 2024. LPA observed smoke/carbon monoxide detectors to be operational. LPA observed hazardous items such as knives, chemicals and cleaners to be locked up in the kitchen and a closet in the laundry room. Knives are locked up separate from toxic chemicals. Medication for each resident is kept locked in a cabinet in the dining room. Exit gate was observed to be unlocked. LPA observed exit gate to be unobstructed. LPA reviewed three resident files and three staff files. LPA also reviewed medication for three residents. LPA interviewed one staff and one resident. Based on today's inspection, no deficiencies are being issued.
