Type of visit: Required - 1 Year

On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility and explained the reason for the visit. The facility is licensed for six non-ambulatory residents and has a hospice waiver for six. The facility is a single story home with four resident bedrooms, two of which are shared, one staff bedroom, three shared bathrooms, a living room, a dining room, a kitchen, a laundry room, and an unattached two car garage. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in resident bathrooms. The hot water temperature measured between 109 to 118.7 degrees Fahrenheit and all smoke detectors were operational. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. LPA observed seventeen eggs left on the kitchen counter, staff confirmed it was placed on the counter since the morning. LPA observed over the counter medications on the top shelf of the kitchen cabinet that was unsecured. LPA observed Resident 1 and Resident 3 had half rails on their hospital beds without a physician's order. LPA observed Resident 2, Resident 4 and Resident 5 with full rails on their hospital beds and who were not on hospice. LPA reviewed two out of two staff fingerprint records. LPA confirmed staff are background cleared and associated to the facility. LPA reviewed five out of five resident files. LPA confirmed Administrator has a current Administrator certificate that expires on November 11, 2026. Based on observations made during today's visit deficiencies are being cited per Title 22. Type of visit: Required - 1 Year

On July 24, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Administrator (AD) Maria Mirabueno was notified via telephone but could not arrive to assist with the inspection. LPA observed Maria Mirabueno has a valid Administrator certificate which expires on November 1, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for six. The facility is a single story home with four resident bedrooms, two of which are shared, one staff bedroom, three shared bathrooms, a living room, a dining room, a kitchen, a laundry room, and an unattached two car garage. LPA, accompanied by a care giving staff, conducted a tour of the interior portions of the facility. On today's visit, LPA observed five resident in care and two care giving staff present. LPA observed residents relaxing in their respective bedrooms. LPA observed the See Something, Say Something poster, (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected all four resident bedrooms and observed them to be free of hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway closet. LPA inspected the two shared resident bathrooms and observed them to be clean. Bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured 107.6 degrees Fahrenheit. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. LPA observed the facility has a two day perishable and a seven day non-perishable food supply on hand. LPA observed kitchen appliances to be clean and operational.CONTINUED ON LIC809-C LPA observed the five burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed a fire extinguishers to be mounted on the wall in the kitchen and it was observed to be charged and purchased on September 5, 2024. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on July 6, 2025. LPA observed the centrally stored medication to be kept in a locked kitchen cabinet. LPA observed a First Aid Kit to be stored in a locked hallway closet and it had all the required components. LPA observed oxygen signs to be posted in the appropriate areas. LPA inspected the laundry room and observed it to be free of hazards. LPA observed chemicals and toxins to be stored in a locked cabinet in the laundry room. LPA, accompanied by a care giving staff, conducted a tour of the exterior portion of the facility. The exterior portion was observed to be free of hazards and obstructions. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gates of the facility to be self latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA observed the door leading to the unattached two car garage to be kept locked and inaccessible to clients in care. LPA observed the garage to be used for storage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA reviewed all five resident files. All the required documentation were present and current in the resident file reviewed. LPA reviewed the residents' medication and medication records. LPA reviewed two staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA explained the reason for the visit. Staff contacted Administrator Maria Mirabueno and informed her of the visit. Maria Mirabueno's Administrator's Certificate expires on November 1, 2024. Facility is a one story home with 5 bedrooms (1 is for staff), 3 bathrooms, dining room, eat in kitchen, living room and a detached 2 car garage. LPA observed the See Something, Say Something poster (PUB 475) posted in the main entry of the facility. LPA and staff toured the facility. The smoke detectors/carbon monoxide detectors tested operational. LPA observed the fire extinguisher in the kitchen is fully charged. LPA observed a 2 day perishable and 7 day non-perishable food supply on hand in the kitchen. LPA observed the kitchen is clean and organized. Knives are kept locked in a kitchen drawer. Medication is kept locked in a kitchen cabinet. LPA observed the 5 burner cook-top lights unassisted. LPA and staff toured the resident rooms. LPA observed the resident rooms had the required furnishings. LPA observed the hall closet is kept locked and used for storage of supplies. LPA observed all 3 bathrooms were clean and operational. Hot water measured 130.0 degrees Fahrenheit in bathroom 1. LPA and staff toured the backyard and the garage. The garage is used for storage and kept locked. LPA observed extra supplies and furniture stored in the garage. There are 2 sheds in the garage. Both sheds are kept locked and used for storage. LPA observed old furniture and tools are kept in both sheds. No bodies of water observed There are 3 exit gates in the backyard. All exit gates are operational. The Administrator arrived during the visit. LPA reviewed 5 resident files and 5 resident medications. No discrepancies observed. LPA reviewed 2 staff files. Both staff members had CPR training and the required annual training. No discrepancies observed. Both staff members are background cleared and associated to the facility. LPA inspected the first aid kit. The first aid kit has all the required elements. LPA consulted with the Administrator concerning reporting requirements and resident rights. Deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
