Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. made an unannounced visit to the facility to conduct a Plan of Correction (POC) visit for deficiencies issued during the required annual inspection that was conducted on July 17, 2026. LPA was greeted and granted entry by (AD) Marieta Marquez. LPA explained the purpose of the visit. Regarding the Type B citation 1569.625(b)(2). LPA reviewed 2 of 2 updated staff trainings files dated July 23-26, 2026. Therefore, the citation has been cleared. Regarding Type A citation 1569.605. The Licensee has not obtained current liability insurance; therefore, a Type A citation will issued today. Based on today's observations one citation is being issued per Title 22 Division 6 of the California Code of Regulations. One of two citations issued during the required annual inspection conducted on July 17, 2026, has been cleared. Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. conducted an unannounced visit for the Required 1 Year Inspection. LPA explained the purpose of today’s visit, and was greeted and granted entry by Administrator (AD) Marieta Marquez. For today’s visit, LPA observed a total of three residents in care and two staff members on duty. LPA observed the Administrator Certificate for AD Marieta Marquez which expires on October 30, 2026. LPA Ramirez toured the interior and exterior portions of the facility with AD Marquez. The facility is a one-story home and is licensed for six non-ambulatory resident. The facility has a Hospice waiver approved for two. There are a total of six bedrooms of which one is for staff. LPA Ramirez toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. LPA observed all windows were screened. Smoke and carbon monoxide detector alarms were tested and operational. There are a total of three restrooms. Restrooms were observed to be in good repair and toilets were operational. Water temperature tested 106.1 degrees Fahrenheit. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Fire extinguisher was observed to be fully charged and mounted by the hallway. Gas stove, microwave, washer, and dryer were all inspected and observed to be operable. CONTINUED ON LIC809-C... LPA Ramirez observed the emergency disaster and evacuation plan located by the dining room. Licensee was advised to use the update Emergency Distaste Plan (LIC610E) dated December 2025. Facility had back-up emergency food and water supply. LPA observed that the First Aid Kit had all the required components. LPA observed that medications and toxins were locked and inaccessible to residents in care. Medications are locked in a cabinet by the residents' bedroom hallway. For the exterior portion, LPA Ramirez observed a shaded area, patio furniture, and the grounds were free of any hazards. There are two gates in the backyard. No bodies of water were observed. LPA reviewed three of three resident files and three staff file. LPA observed that two of two caregivers completed their last training on June 2025, a citation is being issued today. LPA interviewed three residents and three staff. For today's visit two deficiencies were issued per Title 22 Division 6 of the California Code of Regulations. On July 11, 2025, Licensing Program Analyst (LPA) Samer Haddadin conducted an unannounced annual inspection at Sunshine Retirement Home. The facility holds a license to care for six non-ambulatory residents and a hospice waiver for two individuals, although none were receiving hospice services at the time of inspection. During the visit, four residents were in care. Administrator Marieta Marquez maintains a valid certificate through October 30, 2024. A thorough tour of the physical plant and grounds revealed the home to be clean, safe, and sanitary. The facility comprises five resident bedrooms, two common restrooms, one dedicated resident restroom, a staff room, a living room, a dining room, and a kitchen. Each resident bedroom was appropriately furnished with required bed linens, adequate closet and drawer space, and functional furniture. Bathrooms were fitted with secure and operational toilets, faucets, and grab bars; showers were free of mold and mildew; and water temperatures ranged from 117.9 °F to 120.5 °F, within acceptable limits. Supplies of bath towels, toiletries, and personal hygiene products were ample, and all common areas and doorways were clear of hazards or obstructions. The kitchen was well stocked with both perishable and non-perishable foods, and all appliances operated correctly. Safety systems—including smoke and carbon monoxide detectors, fire extinguishers, and first aid kits—were tested and found fully functional and properly maintained. All toxic substances were stored securely. Outdoor areas featured ample shaded seating, and exit gates were unlocked but equipped with self-latching mechanisms. A records review encompassed three resident files and four staff files. Each resident file contained the required admission agreement, physician reports, and resident appraisal. Medications were stored securely and administered in accordance with physician orders. The facility’s emergency disaster plan and infection control plan were comprehensive and up to date. Documentation showed the most recent fire drill occurred on June 25, 2025.No violations of Title 22, Division 6, Chapter 8 of the California Code of Regulations were identified. LPA was greeted and granted entry into the facility and explained the reason for the visit. Deficiency cited under Health and Safety Code 1569.605 pertaining to Liability Insurance has been cleared. Licensee provided proof of insurance. Licensee has complied with the POC. Deficiency cited under Health and Safety Code 1569.695(c) pertaining to Emergency Drills has been cleared. Licensee provided proof of emergency drill. Licensee has complied with the POC. Deficiency cited under Title 22 Regulation 87608(a)(3) pertaining to Postural Supports has been cleared. Licensee provided proof of physician orders for bed rails. Licensee has complied with the POC. During today's visit, Licensee has ample emergency water. Licensing Program Analysts (LPAs) Kimberly Lyman and Samer Haddadin conducted an unannounced visit to Sunshine Retirement Home. The purpose of today’s visit was to conduct the Annual Required inspection. LPAs were allowed entry into the facility and explained the reason for the visit. Facility is licensed for 6 non-ambulatory residents with a hospice waiver for 2. There are no residents on hospice during today's visit. Administrator Marieta Marquez has an administrator certificate valid until 10/30/2024. LPAs along with Administrator Marieta Marquez toured the facility at 1:35 PM. LPAs toured the physical plant, checked food service, first aid kit and reviewed records. Facility appears to be clean, safe, and sanitary. The home consists of five resident bedrooms, 2 common restrooms, resident restroom, one staff room, living room, dining room, and kitchen. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. LPAs observed two residents with half bed rails. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 118.7 and 122.5 degrees F in all facility bathrooms. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the elements including thermometer, tweezers and scissors. LPAs observed toxins are secured during today's visit. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Smoke detectors and Carbon Monoxide tested operational during today's visit. Fire extinguisher is fully charged. Kitchen appliances are operational during today's visit. LPAs toured the outside grounds and there is ample shaded seating for residents. Exit gates are unlocked and self latching. LPAs observed ample emergency food supply. LPAs reviewed the emergency disaster plan and infection control plan during the visit. Plans are thorough and complete. Facility does not have documentation of last fire drill conducted. CONT ON LIC809-C DATED 07/31/2024. Facility provides activities per resident request. At 2:20 PM, LPAs reviewed three resident files and four staff files. Resident files contained required documents including admission agreements, physician reports and resident appraisals. Two out of two residents with bed rails do not have corresponding physician orders for rails. Staff files reviewed contained required documentation of medical clearance/ TB, CPR training and criminal record clearance. At 2:45 PM, LPAs reviewed medication storage and administration. Medications are stored in a locked cabinet. Medications are being administered per physician order. Based on the observations made during today's visit, the following violations are being cited per California Code of Regulations, Title 22, Division 6, Chapter 8.
