At 11:55 am on this day, October 10, 2025, Licensing Program Analyst (LPA) Delmundo arrived unannounced to conduct an annual required inspection. LPA met with Emily Aliping, administrator (ADM), and informed the reason for visit. LPA toured the facility inside out with ADM. LPA inspected the kitchen, dining area, living room, bedrooms, bathrooms, garage, front, side and backyards. There's supplies of 2 days of perishables and 7 days of non-perishables food. Central storage for medications was observed locked. Cleaning supplies were also locked. Facility has smoke and carbon monoxide detectors that were tested and observed in working condition. Hot water temperature in the ensuite bathroom was tested, and measured at 106.1 degrees Fahrenheit. Facility conducts disaster drills every quarter, and records showed last conducted September 1, 2025. LPA reviewed 3 staff and 3 residents files, and interviewed 1 resident. Medications inspected and compared with doctor's orders and LIC622 Centrally Stored Medication and Destruction Record. Resident's P&I checked and compared with last recorded balance. ........continued on 809C (page 2) LPA observed the following: -at 12:10 pm, half bed rail in resident's bed. LPA verified, and ADM stated there's no doctor's order. -at 12:15 pm, staff medications in drawer without lock in unlocked staff bedroom. -at 2:50 pm, resident (R1) has 4 medications listed on After Visit Summary dated 6/24/25, but facility does have have these. Facility has 2 medications for R1 but not listed on the order. LPA received on this day copies of following updated/current documents: 1. LIC308 Designation of Facility Responsibility 2. LIC610E Emergency Disaster Plan (9 pages) 3. $3M Liability Insurance certificate Administrator to submit the following updated/current documents by October 24, 2025: 1.LIC500 Personnel Report 2. Surety Bond coverage Deficiencies are cited from Title 22 California Code of Regulations, and listed on 809Ds. On 08/26/2024 around 09:30 AM, LPA L Holmes arrived at the facility. LPA Telephone and left a message for Emily Aliping, Administrator (ADM) and explained the purpose of the visit. ADM arrived about 15 minutes later after returning from physician with R1. LPA and ADM toured the facilely including but not limited to the common areas, dining room, bathroom, kitchen, bedrooms. The facility consists 3 residents but two were at the day program. All outdoor and indoor passageways were free of obstruction. There were no bodies of water present. A comfortable temperature was maintained at 73 degrees Fahrenheit (F). LPA observed lighting in all areas to be adequate for the comfort and safety of the residents. The hot water temperature in the shared restrooms were measured at 105.2 and 106.5 degrees (F). The shared restroom had paper towels, soap and garbage cans; all areas were safe and sanitary. PPE, sanitizer, and paper goods remain sufficient. There is a 2-day supply of perishable foods and a 7-day supply of non-perishable foods. ...continued on LIC9099C. ...continued from LIC9099. Safety drills are rotational between monthly, last on 08/10/24. LPA reviewed three (3) staff files, and three (3) resident files. On 12/13/2023 at 3:30pm, Licensing Program Analyst (LPA) L. Hall conducted an unannounced 1-Year Required inspection. LPA met with Emily Aliping, Administrator and LPA explained the purpose of the visit. The Administrator currently holds a certificate (#6007022740) that expires on 8/25/2024. The facility’s fire clearance was approved for four (5) non-ambulatory and two (2) ambulatory residents. LPA toured the facility with Administrator including but not limited to bedrooms, bathrooms, kitchen, common area and back yard. The facility consists of four (4) total bedrooms and three (3) bathrooms. One (1) bedroom is occupied by staff. All outdoor and indoor passageways are kept free of obstruction. A comfortable temperature is maintained at 70 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 111.5 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars and non-skid mats. There is a minimum of 7-day supply of non-perishable and 2-day of perishable foods. Smoke detectors/ carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 6/13/2022. Emergency Disaster Plan was last posted on 10/24/2022. First aid kit was observed to be complete. Fire drill was last conducted on 5/13/2023. LPA observed the following deficiencies: At 4:05pm, LPA observed during record review licensee did not maintain accurate records of cash resources. At 4:25pm, LPA observed medication in unlocked kitchen drawer in cups. At 4:25pm, LPA observed knives in unlocked kitchen drawer. At 4:27pm, LPA observed comet and Windex in unlocked kitchen cabinet underneath sink LPA requested the following documents to be submitted to CCLD by 12/20/2023.
